The annual compliance monitoring checklist for Child Care Centers was used to note requirements monitored today. Upon my arrival, I was greeted by Kelly Nance, Assistant Director. The Administrator, Tammy Pait, arrived shortly afterwards. The program operates as a Religious-Sponsored program operating with a GS 110 Notice of Compliance. I was unaccompanied as I completed a general walk-through of the facility. A total of twenty-seven (27) children were present during today’s visit. I observed the children participating in free choice play, completing routine care tasks and participating in gross motor activities indoors. REQUIRED INSPECTIONS/DRILLS: The most recent annual fire inspection was completed on October 14, 2025, with approval for daytime care. The most recent annual sanitation inspection was completed November 6, 2025, and received a Superior classification with 15 demerits. The last documented monthly fire drill was conducted on April 23, 2026, and the last documented quarterly lockdown drill/shelter-in-place drill was conducted on March, 2026. The following items of non-compliance of childcare requirements were observed and documented during today’s visit. You submitted the compliance letter during today’s visit. Violation Number Comment Rule 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff (ML) did not have a signed acknowledgement of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy on file. This was corrected during the visit when the staff member completed the signed policy. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Three staff (AS,KB, ML) did not have verification of completing the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The violations must be corrected immediately. In response to the violations, you shall submit a compliance letter that explains how the violations were corrected. The letter of compliance must be received no later than June 9, 2026. You can email the letter to Miriam.Byrd@dhhs.nc.gov. If you state in your letter that compliance has been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by the date above, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history score is 96% from 11/27/2024 to 5/26/2026. Technical assistance was provided regarding required training for all staff at the center. I reminded you to ensure that all required trainings are completed within the designated timeframes and maintained in staff files for review. Monitoring expiration dates, maintaining current documentation and developing a system to track upcoming training renewals was discussed. Today’s visit was conducted with Tammy Pait, Administrator and Kelly Nance, Assistant Director. The program operates as a Religious-Sponsored with a GS 110 Notice of Compliance. I completed a general walk-through of the indoor and outdoor areas used by the children. I monitored the required postings, attendance logs, program records, equipment and furnishings, staff/child ratios & supervision, and outdoor area & equipment. I observed all children participating in teacher-directed activities, free-choice activities and outside gross motor play. Today’s lunch was in compliance with the Meal Pattern requirements. REQUIRED INSPECTIONS/DRILLS: The last fire inspection was completed on October 14, 2025, with approval for daytime care. The most recent annual sanitation inspection was completed on November 6, 2025, and received a Superior classification with 15 demerits. The Emergency Drill Log form was monitored during today's visit. Three violations of child care requirements were observed and documented. The computer-generated report was printed at the conclusion of today’s visit, and a copy was left with Ms. Pait. Violation Number Comment Rule 404 All staff did not wash their hands thoroughly after diapering each child. The staff providing care to infants did not wash their hands after diapering a child. 15A NCAC 18A .2803(a) 405 A child's hands were not washed after each diaper change. After the child's diaper was changed, the child's hands were not washed. 15A NCAC 18A .2803(c)(2) 619 Diaper changing surfaces were not cleaned with a detergent solution and disinfected after each use. After the child's diaper was changed, the staff did not clean the diaper changing surface with a detergent solution and disinfectant. 15A NCAC 18A .2819(c) The violations must be corrected immediately. In response to the violations, you shall submit a compliance letter that explains how the violations were corrected. The letter of compliance must be received no later than December 9, 2025. I emphasized the role of proper hand hygiene, and surface disinfection in preventing the spread of illness. Staff were reminded to follow the diapering procedures posted to ensure all sanitation steps are completed after each diaper change. I reminded staff that both the child and staff member must wash their hand immediately after each diaper change, even if gloves were worn. During today’s visit, Ms. Pait stated the outdoor learning environment had been extended. I re-measured the outdoor learning environment to update the total amount of usable space following recent changes to the outdoor area. The new measurements confirmed that the outdoor play space now totals 3,555.59 square feet. At 75 square feet per child, 47 children can occupy the outdoor environment. At 100 square feet per child, 36 children can occupy the outdoor environment. The Outdoor Space Calculation worksheet was completed during the visit. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. On or before June 25, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. An Annual Compliance visit was conducted on July 3, 2024. The program currently operates as a Religious-Sponsored program operating with a GS 110 Notice of Compliance. Tammy Pait, Administrator, was not present during today's visit. Kelly Nance, Assistant Director, was on-site, providing care in space #7. REQUIRED INSPECTIONS/DRILLS: The most recent annual fire inspection was completed on August 19, 2024. The most recent annual sanitation inspection was completed November 19, 2024, and received a Superior classification with 12 demerits. The Emergency Drill Log was monitored during today's visit. During today’s visit, a partial assessment of child care requirements was conducted. I was unaccompanied as I completed a general walk-through of the facility, which consisted of all classrooms, the outdoor play area, and the kitchen. A total of thirty-one (31) children were present during today’s visit. I observed preschool aged children participating in free choice play in their classrooms and in gross motor play in the multi-purpose area. I observed infants in daily routines and napping. I monitored the required postings, attendance logs, and safe sleep logs. I also monitored program records, equipment and furnishings, staff/child ratios & supervision, and outdoor area & equipment. This facility does not provide transportation. One violation of child care requirements was observed and documented. Computer-generated report was printed at the conclusion of today's visit and a copy was given to Ms. Nance. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Grass and weeds have grown into the fall zone area of the large outdoor climber. 15A NCAC 18A .2832(a) All violations must be corrected immediately. A compliance letter is due to me within two weeks of today’s visit. By April 29, 2025, please send me a letter addressing how the violation was corrected. I asked Ms. Nance if children are going outside on a daily basis. She stated that all children do go outside on a daily basis, weather permitting. I reminded Ms. Nance of the Child Care rule 10A NCAC 09 .0508(c)-When children are in care and weather conditions permit, there shall be outdoor time, either as part of a small group, a whole group, or individual activity, for no less than the following durations: Children under 2 years of age must have at total of 30 minutes of outdoor play, daily. Children 2-12 years of age must have a total of 60 minutes of outdoor play, daily. Playing outdoors has benefits for both young children and educators. It’s a refreshing pause in the day’s schedule—time set aside to look and listen, explore and observe, move and let loose. Time spent outside can lead to better physical and mental health, improved sleep, and cognitive, social, and emotional gains for young children. Ensuring that outdoor play is an integral part of your child care and education setting’s daily schedule supports early learning across all domains and unleashes a whole lot of joy—for you and for children! Reminder: If you haven't completed ABCMS regarding the CRIMINAL BACKGROUND CHECK SYSTEM, please make sure to complete that as soon as possible. North Carolina Child Care Administrators can now view and edit their facility’s roster in ABCMS, the DCDEE’s criminal background check system. ABCMS allows administrators to see the real-time background check status of staff members, run printable report of the staff roster to assist with compliance visits, and see new background check applicants and add to the staff roster. Today's visit was conducted with Kelly Nance, Assistant Director. Tammy Pait, Director, arrived shortly after my arrival. Limited monitoring of child care requirements occurred during today's visit. During the Annual Compliance visit on July 10, 2023, five (5) violations of child care requirements were documented. A compliance letter was received on July 21, 2023 showing the following violation was corrected: #1897- Recognizing and Responding to Suspicions of Child Maltreatment have been completed and is on file. An extension was granted for the following violations until August 7, 2023: #1048- I verified First Aid certification for one staff was completed on August 3, 2023. #1049- I verified CPR certification for three staff was completed on August 3, 2023. #0106- A fire inspection was conducted on July 11, 2023. The inspection report was received during today's visit. #1032-One staff has not obtained a medical report as of today. An appointment is scheduled for August 12, 2023. #1899-I verified four out of five staff have completed the health and safety trainings. Two outstanding violations were cited during today's visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A medical report for one staff has not been completed. 10A NCAC 09 .0701(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff (out of five) has not completed the health and safety training within the required timeframe. .1103(b) The violations must be corrected immediately. Today's visit was conducted with Kelly Nance, Assistant Director. Tammy Pait, Director, arrived shortly after my arrival. Limited monitoring of child care requirements occurred during today's visit. During the Annual Compliance visit on July 10, 2023, five (5) violations of child care requirements were documented. A compliance letter was received on July 21, 2023 showing the following violation was corrected: #1897- Recognizing and Responding to Suspicions of Child Maltreatment have been completed and is on file. An extension was granted for the following violations until August 7, 2023: #1048- I verified First Aid certification for one staff was completed on August 3, 2023. #1049- I verified CPR certification for three staff was completed on August 3, 2023. #0106- A fire inspection was conducted on July 11, 2023. The inspection report was received during today's visit. #1032-One staff has not obtained a medical report as of today. An appointment is scheduled for August 12, 2023. #1899-I verified four out of five staff have completed the health and safety trainings. Two outstanding violations were cited during today's visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. We discussed changes in the child care requirements regarding Criminal Background Checks. I notified you today that qualification is now valid for five years rather than three. New letters issued will have the five year timeframe, however current letters will not be reissued. I encouraged you to develop a system to track the updated due dates. Facility Specific TA/Consultation/Reminders: Health and Safety Trainings: Health and Safety training requirements were monitored. I reminded you these trainings must be renewed every 5 years.
