This visit was conducted by Audrey Davis, childcare consultant with Donna Flippen, lead teacher. Maria Galicia, Head Start Family Advocate served as a substitute teacher assistant today in the absence of Whitney Mendez. Your program currently operates one classroom with a five-star license. Restrictions include daytime care only; meets enhanced space; enhanced ratios, enhanced ratios minus 1; and no cooking allowed. The license was observed, and the restrictions were found in compliance. This program is owned and operated by YVEDDI, Inc. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. I conducted a walk-through of the facility observing indoor and outdoor areas. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision of children was adequate during my observations. I observed positive interactions today. I observed children working in a variety of activity areas, outdoor play, lunch, and rest. I observed developmentally appropriate materials and activities were available to the children in the classroom. Lunch consisted of hotdog on bun, potato wedges, sliced apples, and milk. We reviewed the importance of safety when eating foods such as hotdogs and fresh apples including not encouraging a lot of laughing when eating and teachers positioned to visually supervise each child. After lunch children brushed their teeth to the Toothbrushing Song by Blippi. The last annual compliance visit was conducted 3/9/23. A sanitation inspection was last conducted 12/15/23 with a Superior classification. . The last fire inspection was conducted 9/25/23. You provided me with a copy of the fire inspection report today. Program records and required postings were monitored. A fire drill was last documented on 3/1/24 and a lockdown emergency drill on 2/21/24. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous products was monitored and are stored in a locked closets No medications were observed or reported. The program does not provide transportation to and from the facility and does not provide any off-premises activities. Children are not transported for any reason. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance during your September 20, 2023 routine unannounced visit. This classroom uses The Creative Curriculum. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet not in use beside the sand table was not covered. 10A NCAC 09 .0604(c) 1043 All staff records, except financial records, were not made available for review. The substitute provider did not have the required records on file upon request. She was able to contact her supervisor who brought her file to the site. G.S. 110-91( 9) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. A staff employed in October 2023 did not have documentation of orientation within the first six weeks on file. .1101(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A teacher employed 10/20/23 did not have a training certificate on file verifying completion of the Recognizing and Responding to Suspicions of Child Maltreatment on file. .1102(g) Child Care programs are expected to be in compliance at all times and are required by NC GS 110-90(4) (d) to maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Substitute files - I recommend subs have a travel file to carry from site to site. Consultation • Resuming the Rated License - Your facility is in Cohort 2 to resume the rated license assessment meaning your preparation year will begin July 1, 2024 through June 30, 2025. Suggested activities during your preparation year include - make sure all staff have an up-to-date WORKS account, - decide whether or not you will request the Environment Rating Scale to maintain your points in the program component of the rated license; - encourage staff to participate in quality-related trainings sponsored by the CCR&R and Surry County Partnership for Children; - request technical assistance from me and other local partners; - if staff members need to take additional college coursework to maintain your points in the education component, contact Surry Community College to discuss educational opportunities; - review the NCRLAP website for Environmental Rating Scales resources at www.ncrlap.org; and - you have the option to request the Environmental Rating Scale during your preparation year and either process your rated license during your prep year or save the scores to be used during your assessment year. Donna Flippen, Lead Teacher, assisted me with the visit. The classroom is operated by YVEDDI, Inc. with a five-star license effective 2/18/19. You operate one NC Pre-K Head Start classroom. Restrictions include first shift care, enhanced space, enhanced ratios, enhanced ratios minus one, and no cooking. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS (n/a) • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having outdoor play, lunch, toothbrushing and rest. Lunch consisted of chicken sandwich, potato tots, raw carrots and ranch dressing, apple slices, and low-fat unflavored milk. One new staff was reported at this program, transferred from Red Hill Creek Head Start. Files for new staff was reviewed. Limited monitoring of continuing staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. Chemicals are stored in a locked closet. A playground inspection was recorded 9/1/23. A fire drill was documented 9/18/23 and a shelter-in-place drill for the facility was documented 8/29/23. The most recent fire inspection was on 3/9/23 which you provided a copy for me today. I observed a sanitation inspection was last conducted on 4/26/23 earning a Superior classification with no demerits. The program does not provide transportation. Pre-K requirements were monitored as follows: The first day of school for this group was 8/14/23. The classroom operates from 8 to 2:30 daily ten months per year. You use the Creative Curriculum. You said you provide at least three parent conferences per year. You stated you communicate with parents through Ready Rosie. Family engagement opportunities include activities in the weekly folders, and reading activities. I observed two children's files and observed medical and shot records and vision, dental, and hearing screenings. You have completed Dial 4 assessments for each child. One violation was observed and documented today as follows: Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Medical documents for the food service staff were maintained in the employee's general personnel file and not in a separate file as required. .0701(d) Child Care programs are expected to maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. (This includes the decision to become incorporated or dissolve a corporation.) • Please send me your staff and training worksheet by February 1, 2024 to help me prepare for your upcoming annual compliance visit. • You are in Cohort 2 for resuming the rated license reassessment meaning your preparation year will be June 30, 2024 through June 30, 2025. Beginning July 1, 2025, we will begin the rated license reassessments for Cohort 2. We recommend all facilities take advantage of this time to make sure staff has similar education levels as your last assessment and all staff have an up-to-date WORKS account. Also, if you have staff who have not participated in an Environment Rating Scale assessment, this is a good time to familiarize them with the ECERS-R by making available copies of the ECERS-R and providing trainings in the ECERS.
