This visit was conducted with Melinda Lescar, Child Care Consultant and Sharon Bledsoe, Operator. Your program currently operates with a Two Star license. Restrictions include daytime care only, maximum five preschoolers at any time, children on ground level only, fireplace not in use, serves no more than two infants at a time. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play activities, outdoor play, and lunch. I observed materials and activities that were available to the children. The last annual compliance visit was conducted on May 5, 2025, A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility. I have asked Sharon Bledsoe who lives in the home and recorded her answer on the Verification of Required Information for Operator and Additional Caregiver Form with her signature. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on 2/10/2026. A lock down drill was documented on 1/8/2026. An outdoor inspection was documented on 4/7/2026. There is one (1) pet inside the home and all vaccinations, and parent acknowledgement forms were monitored. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Medications were monitored per DCDEE procedures. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. The last fire drill documented on the log was on February 10, 2026. .1721 (e)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before April 28, 2026. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to Melinda.lescar@dhhs.nc.gov or mail two copies of the letter to: Melinda Lescar P.O. Technical assistance discussed during today’s visit: Item #923- The last fire drill was documented on 2/10/2026. Ms. Bledsoe stated during today’s visit that she did a fire drill in March but forgot to document it on the log. It is recommended to review your fire drill logs regularly to ensure that these are documented monthly. Additional Information/Consultation: The Pathway to the Stars information was discussed today. Ms. Bledsoe stated she is not interested in pursuing any of the Pathways and will request a reduction in stars, from a two-star to one-star family child care home. Ms. Bledsoe plans to send a letter requesting a reduction in stars to me by April 28, 2026. You must notify your consultant 30 days prior to changing the ownership status of your facility. This visit was conducted by Turkessha Casterlow, Child Care Consultant, and Pam Hauser, Licensing Supervisor with Sharon Bledsoe, Operator. Your program currently operates with two-star license effective 9-18-2020. Restrictions include first shift only, maximum of 5 preschool children at any time, children in care on ground level only, fireplace/woodstove not used during operating hours, and serves no more than 2 infants under one year of age. Ms. Bledsoe is the sole proprietor of this licensed family child care home. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. A walk-through of the home was completed today. Indoor and outdoor areas were observed. During the visit the fire alarm was not tested due to children napping. Ms. Bledsoe confirmed all six fire alarms are in working order and this will be be completed on the next annual compliance visit. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. Children were napping during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded November 24, 2024. A fire drill was recorded November 11, 2024, at 9:30am with 15 seconds. A lock down shelter in place drill for the facility was recorded October 9, 2024. There was one pet inside the home and all vaccinations and parent acknowledgement forms were monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The facility EPR plan was last updated June 7, 2022. .1714(e ) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. There was one child's sleeping mat torn in two places with foam exposed and accessible to children. .1719(a)(18) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Lead and Asbestos Testing Information for Family Child Care Homes Effective January 1, 2024, Child Care Rules were amended to include the requirement for testing of all child care facilities for lead in water, lead-based paint, and asbestos testing. An email blast from the Division of Child Development and Early Education detailing the steps for enrollment and participation in this testing was sent out on December 7, 2023. If you have not already, immediately visit the Clean Water for US Kids website at www.cleanwaterforUSkids.org/carolina and sign up for a pre-enrollment webinar, which must be completed prior to enrolling for testing. Water samples shall be collected by the operator and tested within 12 months of the effective date of this Rule. The pre-enrollment webinar will give an overview of the program process and representatives will be able to answer your remaining questions or comments live after the presentation. Sharon Bledsoe, Owner/ Operator of the home assisted me with the visit. Your program currently operates with a two-star license. Restrictions include 1st shift care, minimum of five preschool children at any time, NMT 2 infants under age one; children in care only on ground level; and fireplace/ wood stove not used during operating hours. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Ms. Bledsoe stated the only person living in the home with you is your husband. Be reminded that you and your husband must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. I observed a variety of developmentally appropriate toys for indoor play. You said for outdoor play, you have a sandbox, and you take out balls, frisbees, push toys, and riding toys. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed children getting up from rest; having a snack of animal crackers and apple juice; and playing with toys. I observed positive interactions today as the children set up a pretend ice cream shoppe and took each other’s ice cream order, discussed ice cream flavors, and the cost of the ice cream which cost “five monies”. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed or reported. Storage of hazardous items was monitored today and are stored in a cabinet above the washer and dryer. An emergency lockdown drill was last documented 10/9/23. Another emergency drill is due this month to meet the requirement for conducting quarterly emergency drills. A fire drill was last documented 12/5/23. There is one pet in the home, a basset hound mix with current vaccinations on file. You stated you do not transport child care children for any reason. You said you do not conduct routine tasks and do not use any substitute providers. The following violation was documented during today’s visit: Violation Number Comment Rule 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. The cabinet used to store cleaning supplies was not locked during my walk-through. .1719(a)(6) Child Care programs are expected to be in compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance provided during today’s visit: You stated you did laundry this morning and did not remember to re-lock the cabinet where chemicals are stored. You are using magnetic locks and had some trouble working the lock during my visit. You may want to consider an easier type of lock in order to consistently meet this requirement. Other topics discussed today: You must notify your consultant 30 days prior to changing the ownership status of your facility. Examples of an ownership change include but are not limited to becoming incorporated or dissolving your corporation. Rated License – Your FCCH has been assigned to Cohort 2 to resume the rated license. This means your preparation year will begin July 1, 2024 through June 30, 2025 and your rated license points will be reassessed between July 1, 2025 and June 30, 2026. Although you have not requested the Environment Rating Scale in the past, this is something you could consider for the future if you wish to apply for higher stars. Provider Verification of Information Form: I emailed the family child care provider verification of information form which you completed and sent to me prior to this visit with the exception of the on-going training section. Be sure to resubmit the form to me 30 days prior to last year’s annual compliance visit, by May 14, 2024 so I can prepare for your visit.
