This visit was also used to monitor compliance with applicable childcare requirements located in Child Care Rule Section .3000 in spaces 1 and 2, where children participating in the NC Pre-K program are cared for. This visit was conducted with Victoria Spriegel and Mary Beth Overby, lead teachers. Your program currently operates with a Five-star license. Restrictions include first shift only, meeting all enhanced requirements and reduced ratios minus one, preschool may not use school age equipment and Certified Developmental Day. The license was observed, and the restrictions were found in compliance. The facility is operated by Surry County Public Schools. The program’s compliance history was 91% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children engaged in blocks, manipulatives, art, and dramatic play. I observed materials and activities that were available to the children in the classrooms. Children were served a lunch consisting of chicken sandwich, broccoli, fruit cup and milk. The last annual compliance visit was conducted on August 22, 2024. A sanitation inspection was completed April 24, 2025, with a superior classification. The last fire inspection was conducted on March 3, 2025. Program records and required postings were monitored. A fire drill was conducted on May 9, 2025. A shelter in place drill was documented on March 18, 2025. An outdoor inspection was documented on May 16, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored for diaper creams. No emergency medication was reported The program does provide transportation through the public school. Required DPI Transportation forms were current and on file verifying all requirements are monitored. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. In addition, a Ready Rosie is used as an additional means to communicate with parents. You verified that you incorporate family engagement by field trips, opportunities for parents to participate in the classroom, eat lunch with their child, etc. The environment rating scale score received on 12/3/2019 was 5.78. I have observed two new staff files today. The storage and administration of medications was monitored. Medication authorization was monitored. The storage of hazardous items was monitored. One child has seizure medicine on site, however, the permission slip for the medicine has no date. A fire drill was documented on 04-18-2024. A shelter in place drill was documented on 03-18-2024. An outdoor inspection was documented on 04-30-2024. A sanitation inspection was completed on 04-14-2024. A fire inspection was completed on 02-01-2024. The following violations were documented during this visit: Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. The allergies were not posted in space 1. .0901(g) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not posted in space 1. 10A NCAC 09 .0901(b) 860 Balloons were accessible to children. There are hundreds of broken balloons on the ground on the playground and on the path to the playground within reach of children; these are leftover from yesterday’s water balloon day. .0604(q) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. There is one seizure medicine on site that has no dates on the permission slip for the time period that the medicine may be given to the child. This matches rule number .0803(6)(i). .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) I have asked Ms. Inscore to send me written documentation by June 4, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Inscore that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current Developmental Day staff are lead teacher, Victoria Spriegel and teacher assistants Marissa Inscore and Dawn Shupe. Lead teacher for the NC Pre-K classroom is Mary Collins and teacher assistant Sarah Hicks. Please follow up with parents regarding children’s health assessments which are required on file within 30 days of enrollment. This along with developmental screenings for NC Pre-K children will be monitored at a future visit. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Second Step Curriculum and are implementing Letter Land. The first day full day of school was this Monday, August 28th and the classrooms operate from 8 am to 2:50 pm. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Tiny binder clips, a push pin and other small objects that could be a choking hazard were stored in an unlocked drawer within children's reach in the Pre-K classroom. 10A NCAC 09 .0601(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet accessible to the children in the bathroom used by the Pre-K children was not covered with a safety plug. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two gallon-size containers of Clorox were stored on a high, open shelf in the Developmental Day classroom and not in locked storage. .2820(b) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Written permission to transport children on the school bus did not include when and where the child was to be transported and the expected time of departure and arrival. .1003(i)(j) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. In the DD classroom, written authorization indicated "topical ointment" and did not name the product or even the product type. The authorization form referenced bug bites although the product provided was Desitin diaper cream. 10A NCAC 09 .0803(2)(b)(i-v) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 13, 2023. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • accurate, detailed description of how and when you corrected each violation You may email the letter to Audrey.Davis@dhhs.nc.gov or mail two copies of the letter to: Audrey Davis PO Box 17273 Winston-Salem, NC 27116 If you state in your letter that corrections or changes have been made when they have not yet been made, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: • We discussed today that the backpack containing food fed to a child using a G-tube should be labeled with the child’s name. • Due to the public school schedule, your next annual compliance visit will most likely occur prior to summer break although it will be less than a year. You will also receive a Routine Unannounced visit sometime during this school year. • Reminder: Health and Safety training topics are required to be completed every 5 years, and can also count as on-going training in the year they are taken. * In preparation for your rated license assessment when the time comes, make sure all staff is registered and up to date in WORKS.
