This visit was conducted by Pam Hauser, Licensing Supervisor, with Cheryl Rodgers, lead teacher. Your program currently operates with a five-star license effective February 7, 2019. Restrictions include 1st shift, meets all enhanced requirements and reduced ratios minus one, and school-age equipment cannot be used. The program is operated by Surry County Schools. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. Children were engaged in outdoor play, group time and activity centers. There were no new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded May 6, 2024. A fire drill was recorded April 15, 2024. A shelter in place drill for the facility was recorded 3/19/2024. Ms. Rodgers stated she will conduct an emergency drill prior to the end of the school year this week. The most recent fire inspection was on September 7, 2023. I observed a sanitation inspection was last conducted on April 5, 2024, earning a superior classification. The program transports three children and the DPI transportation documents were monitored were on file and available for review during the visit. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were no safety concerns observed indoors. Outdoors there was one bungee cord attached the the fence holding a white, PVC structure to the fence. 10A NCAC 09 .0601(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. .0803(2)(a) 853 Incident logs were not completed and maintained as required. There was an incident report for one child that was not logged on the incident report log. .0802(g)(1-6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send Audrey Davis documentation verifying compliance on or before June 3, 2024. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Technical assistance discussed during today’s visit: Item #807 - A safe indoor and outdoor environment was not provided for the children was cited during the visit. During today’s visit there were no safety concerns observed indoors. Outdoors there was on bungee cord attached the fence holding a white, PVC structure to the fence. The bungee cord was removed from the fence during the visit. Bungee cords attached to a fence pose a risk to strangulation safety for children. In the future Ms. Rodgers stated she will utilize a zip tie to ensure the safety of the children. Item #844 - Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. During today’s visit there was one child enrolled with an inhaler in a labeled Ziploc bag and not in the original container. The original container is designed to protect the medicine from heat, air, light, and moisture which may impact the effectiveness and safety of medication. Ms. Rodgers stated moving forward she will ensure all prescription medication is in the original labeled container when received from the parent. Ms. Rodgers stated this was not mentioned during the annual compliance visit at the beginning of the school year, if so, it would have been corrected immediately. Item#853- Incident logs were not completed and maintained as required. During today’s visit there was an incident report for one child that was not logged on the incident report log. Incident reports and logs are a vital tool for ensuring safety within an early childhood environment. By documenting incidents, and injuries, on the incident log you can track incident data and take proactive measures to prevent further reoccurrence. Ms. Rodgers stated she is going to get another clipboard to separate the incident form log from Surry County School student and employee injury log. This visit was conducted by Audrey Davis, childcare consultant with Cheryl Rodgers, lead teacher. Nicole Hazelwood, principal, and Ashley Queen, assistant principal were on site and available to assist as needed. The school operated on a two-hour delay today due to weather conditions. Your program currently operates one classroom with a five-star license issued 2/7/19. Restrictions include daytime care only; meets enhanced space; enhanced ratios and enhanced ratios minus 1; and school age outdoor play equipment off-limits to preschool children. The license was observed, and the restrictions were found in compliance. This program is owned and operated by Surry County Schools. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. During this visit, children had lunch, gym play, and free-choice classroom activity areas. I observed developmentally appropriate materials and activities were available to the children in the classroom. The last annual compliance visit was conducted 1/25/23. A sanitation inspection was conducted 10/5/23 with a Superior classification. The last fire inspection was conducted 9/7/23. You provided me with a copy of the fire inspection today. Program records and required postings were monitored. A fire drill was last documented on 12/18/23, a lockdown emergency drill on 11/9/23. A playground inspection was last completed on 12/5/23. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous products was monitored and are stored in a locked classroom cabinet. Emergency medications were stored appropriately with signed authorization on file. Three children are transported by bus to and/ or from school daily. Transportation requirements were monitored. NC Pre-K requirements were monitored as follows: The program operates 7 hours per day, Monday through Friday, 10 months per year. You offer parent/teacher conferences two or more times per year; daily communication with parents by way of daily planners and the TS Gold app. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher Cheryl Rodgers and Hope Elliott, teacher assistant. Ms. Rodgers has a NC B-K license and the administrator has a Level III NCECAC. Two of 18 children’s files were monitored for completed health assessments and developmental screenings. Your program was monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled.
