This visit was conducted with Melinda Lescar, Child Care Consultant and Deborah Flippin, Lead Teacher. Your program currently operates with a Five Star license. Restrictions include 1st shift, meets all enhanced requirements and reduced ratios minus one, and school age equipment cannot be used. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on January 12, 2025, and Surry County Schools was listed as current and active. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children large group time, free play, and lunch. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on February 6, 2025. A sanitation inspection was completed September 4, 2025, with a Superior classification. The last fire inspection was conducted on September 8, 2025. Program records and required postings were monitored. A fire drill was conducted on December 4. 2025. A locked down drill was documented on September 5, 2025. An outdoor inspection was documented on January 5, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. You stated that you provide transportation in the morning and afternoon for ten (10) children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported. All the required DPI paperwork for transportation was available onsite. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, Deborah Flippin and teacher, Hannah Coble. A selection of files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you were in the process of completing the Dial-4 screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was August 27, 2025. The classroom operates from 7:30a.m. to 3:00 p.m. Parent conferences are held once a year. You stated you provide conferences in the Spring if needed. You stated that you use planners, Class Dojo, Ready Rosie and newsletters to communicate with parents. You verified that you incorporate family engagement by offering breakfast with books and parents are invited to have lunch with the children. The environment rating scale score received on April 24, 2019, was 6.19. The following violations were cited during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility roster was not updated to reflect the current staff and administrator. G.S. 110-90.2 & .2703(r) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last lock down drill documented was on September 5, 2025. A shelter-in-place or lock down drill was not completed in December 2025. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all time7 and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before January 27, 2026. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS is an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This visit was conducted by Audrey Davis, childcare consultant with Deborah Flippin, lead teacher. Your program currently operates one classroom with a five-star license. Restrictions include daytime care only; meets enhanced space; enhanced ratios, enhanced ratios minus 1; and school-age playground equipment off-limits to preschool children. The license was observed, and the restrictions were found in compliance. This program is owned and operated by Surry County Schools. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. I conducted a walk-through of the facility observing indoor and outdoor areas. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision of children was adequate during my observations. I observed positive interactions today. I observed children having group time, transitioning to lunch, outdoor play, and rest. I observed developmentally appropriate materials and activities were available to the children in the classroom. The last annual compliance visit was conducted 3/8/23. A sanitation inspection was last conducted 10/16/23 with a Superior classification. The last fire inspection was conducted 3/7/23 which Ms. Martin provided me with a copy during a visit to your school in September 2023. Another fire inspection should be conducted next month. Program records and required postings were monitored. A fire drill was last documented on 1/31/24 and a lockdown emergency drill on 11/30/23. An emergency drill is due by the end of February. A playground inspection checklist was last completed 2/5/24. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous products was monitored and are stored in a locked cabinet in the bathroom. I monitored emergency medications and parent authorization forms. Transportation: I monitored files for two bus riders today which contained all required information. A transportation form was completed and signed by a school administrator verifying that all vehicles are in good condition and free of hazards; have current registration and inspection; all drivers have proper license and have completed orientation on transporting preschool children; appropriate child restraint devices are used, if required by NC Child Restraint Laws; and all DCDEE procedures are being followed. This Pre-K program operates from 7:35 am until 2:55 pm Monday through Friday, 10 months per year. Staff-child ratios and maximum group sizes required for NC Pre-K classrooms were verified in compliance. Ms. Flippin, lead teacher has a NC B-K license and the assistant teacher has an AAS degree in Early Childhood Education. The principal is a Level III NCECAC. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses The Creative Curriculum. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. .1102(c) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Technical assistance provided during today’s visit: Medication Authorization Child Care Rule .0803(6) states a parent may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. I recommend entering the renewal date on a calendar as a reminder at the time you receive an emergency medication so this does not get missed. First Aid Training Documentation: Prior to taking First Aid and CPR training, verify with the trainer that they can provide you with documentation that lists both pediatric first aid and child CPR upon completion of your training. Also, check the documentation once you receive it and notify your trainer immediately if it does not contain the required information. Consultation • Resuming the Rated License - Your facility is in Cohort 2 to resume the rated license assessment meaning your preparation year will begin July 1, 2024 through June 30, 2025. Suggested activities during your preparation year include - make sure all staff have an up-to-date WORKS account, - decide whether or not you will request the Environment Rating Scale to maintain your points in the program component of the rated license; - encourage staff to participate in quality-related trainings sponsored by the CCR&R and Surry County Partnership for Children; - request technical assistance from me and other local partners; - if staff members need to take additional college coursework to maintain your points in the education component, contact Surry Community College to discuss educational opportunities; - review the NCRLAP website for Environmental Rating Scales resources at www.ncrlap.org; and - you have the option to request the Environmental Rating Scale during your preparation year and either process your rated license during your prep year or save the scores to be used during your assessment year. This visit was conducted by Audrey Davis, child care consultant with Deborah Flippin, lead teacher. Tamara Martin, principal was on site and assisted as needed. The classroom is operated by the Surry County Schools with a five-star license effective 5/8/19. Restrictions include first shift care, enhanced space, enhanced ratios, enhanced ratios minus one, and school-age playground equipment off-limits to preschool children. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. A fire drill was documented 9/21/23 and a lockdown emergency drill on 8/30/23. A monthly playground form was last completed on 9/5/23. I observed emergency medication stored greater than 5 feet up out of children’s reach with parent-signed medication authorization form. Storage of hazardous items was monitored today and are kept in a locked closet in the classroom bathroom. The most recent fire inspection was conducted March 7, 2023. Ms. Martin provided me with a copy during todays visit. I observed a sanitation inspection was last conducted on 1/30/23 with a Superior classification. This classroom operates from 7:35 to 2:55 daily, ten months per year. You use the Creative Curriculum. Ms. Flippen stated you communicate with parents via daily planners and a monthly newsletter. You stated parent involvement opportunities include lunch with their child, literacy night, fall festival, and holiday festivities. You reported that all of the Pre-K children have had vision, hearing, and dental screenings which will be monitored during your upcoming annual compliance visit. A nutritious lunch was served today. You stated all of the children eat lunch provided by the school with no lunches brought from home. The school provides nutritious substitutions for allergies. No new staff were reported at this program. Limited monitoring of continuing staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, and Health and Safety Training. Transportation: I observed a Transportation Requirements form signed on 8/23/23 by Principal Tamara Martin verifying the buses are in good condition with up-to-date insurance and inspections. Ms. Martin maintains driver orientation forms and all required transportation information on file in the school office. I observed parents’ written permission to transport forms on file for children that ride the bus. One violation was observed during today’s visit as follows: Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics.
