The visit was conducted with you, Carrie Roselli, Operator. You provided me with applicable program, staff, and children’s records for review. This facility currently operates with a Four star rated license issued on April 28, 2020. The permit restrictions include maximum of 5 preschool children at any time, daytime care, 2nd shift, overnight care and serves no more than 2 infants under age 1. This facility has earned two points in program standards, seven points in education, and one quality point for infant capacity restricted to no more than two children under one year of age. The license was posted, and the restrictions were in compliance. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. You use Creative Curriculum. The program’s compliance history was 98 percent as of December 2, 2025. The most recent annual compliance visit was conducted on January 9, 2025. You stated your FCCH is not incorporated. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form were completed and signed during today’s visit. Your signature on these forms serves as verification the information provided was accurate and complete. A checklist was used to note the requirements monitored today. Four children, one to five years of age, were present today. I observed children engaged in gross motor activities outdoors, free play activities indoors, lunch and nap. You were actively involved with the children. One child, one year of age, two children two years of age, two children four years of age, one child five years of age, and three school age children were enrolled. One child two years of age is a drop-in. You stated no children are enrolled on second or third shifts. Vaccination records for pets were current and on file for review. You stated children enrolled do not have any medications. The most recent outdoor inspection checklist was completed on November 26, 2025. The most recent fire drill was conducted on November 12, 2025. The most recent quarterly drill was conducted on October 16, 2025. Your EPR Plan was updated on July 23, 2025. Criminal Background Check Qualifying letters were reviewed and were in compliance for you and household members. You and household members were listed on the ABCMS Provider Portal roster. The analysis date for the most recent lead water test was May 24, 2024. You may review your facilities results by visiting https://www.cleanwaterforuskids.org/en/carolina/. This website also indicated your facility is “exempt” from required asbestos and lead-based paint testing. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. In the kitchen, a bag of Halls cough drops was observed in an unlocked cabinet above the counter beside the refrigerator. The cough drops belonged to the operator. .1719(a)(5) The violation observed was corrected during the visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance was provided on the following: Storage of medication- You moved the bag of Halls cough drops to locked storage during the visit. I suggested you keep all medications that are not emergency medications in the locked closet at all times. I also suggested that you immediately place medications back in the locked closet after use to ensure they are stored properly at all times. Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container. Designated emergency medications shall be stored out of reach of children at least five feet high but are not required to be in locked storage. Consultation: (The following items discussed were in compliance today.) -We discussed the environment during nap time. I discussed with you that nap time should not be too dark and that lighting should be provided to allow for adequate supervision and offer assistance to a child if needed. Also, music or sound machines played during nap time should not be so loud that you are unable to hear children when assistance is needed. During today’s visit, plenty of light was available in the rooms where children napped and you were using the exhaust fan for white noise, which was not too loud. QRIS Pathways to the Stars: -We reviewed Pathway #1, #2, and #3, Family and Community Engagement Foundational Practices, Continuous Quality Improvement, approved curriculum, formative assessment tools, and capacity options. I encouraged you to review this information more thoroughly by visiting https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization. -I suggested that you review Environment Rating Scale information and resources at www.ncrlap.org for FCCERS-3, should you choose Pathway #1. -Reach out to Lincoln and Gaston County Partnership for Children for additional technical assistance and support at 704-922-0900. -You stated you intend to follow Pathway #2 for your upcoming rated license reassessment. Reminders and Resources: -Providers can speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in their classroom by calling 1-888-600-1685 Option 1. Providers may also submit questions to a Behavior Support Advisor online and receive a call or email in response. In addition, providers may post questions in the ‘Talk to the Expert’ Group on an online network, Social Emotional Connections, for early childhood educators. There are also opportunities to join free webinars on challenging topics or classroom activities. The following areas were monitored during the visit: supervision, capacity, CPR, first aid, ITS-SIDS training, health and safety trainings, CBC qualification, general health, administering of medication, storage of hazardous products, storage of medication, general safety, general licensing requirements, program records, license posted, fire safety, and sanitation. The visit was conducted with you, Carrie Roselli, Operator. Your program currently operates with a Four star rated license effective April 28, 2020. The program’s compliance history was 95 percent as of August 28, 2024. You stated your FCCH is not incorporated. Five (5) preschool children were present today. I observed you and the children engaged in free play activities indoors, handwashing routines, and snack. You were actively involved with the children. Five (5) preschool children, one to five years of age, and three (3) school age children were enrolled. Vaccination records were monitored for both pets and were in compliance. You completed CPR and First Aid training on February 13, 2024, and ITS-SIDS training on April 11, 2024. The most recent outdoor inspection checklist was completed August 26, 2024. The most recent fire drill was conducted on August 21, 2024. The most recent quarterly drill was conducted on July 8, 2024. Your EPR Plan was documented as reviewed on July 24, 2024 Criminal Background Check Qualifying letters were reviewed for you and household members during today’s visit. The qualifying letter for one household member expires on September 30, 2024. The most recent lead water testing results were completed on May 24, 2024. The following violation of child care requirements observed today was reviewed with you and documented in the visit summary given to you at the conclusion of the visit. Violation Number Comment Rule 1404 Medication remaining after the course of treatment is completed, after authorization is withdrawn. or after authorization has expired, was not returned to the parent/guardian. Any medication not retrieved by parent/guardian within 72 hours of completion of treatment, or withdrawal of authorization, was not discarded. Authorization from the parent to administer Blue Lizard sunscreen for one child expired on July 11, 2024. 10A NCAC 09 .1720(c)(5) The violation documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how the violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. I must receive your compliance statement by September 11, 2024. Please send your letter to Kristen.Mauney@dhhs.nc.gov. Failure to correct the violations and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your violation correction letter that corrections have been made when they have not, it will be considered falsification of information. Technical Assistance regarding the violations observed: Medication remaining after the course of treatment is completed, after authorization is withdrawn, or after authorization has expired, must be returned to the parent/guardian. Any medication not retrieved by parent/guardian within 72 hours of completion of treatment, or withdrawal of authorization, must be discarded, unless a new medication permission form is obtained. You must return the sunscreen to the parent or request an updated medication permission form be completed. I suggested that you review medication permission forms monthly to ensure the authorization to administer the medication, sunscreen, etc. has not expired. Consultation: -You may visit http://www.cleanwaterforuskids.org/carolina to check the results of asbestos and lead paint testing.
