The center operates with a Five- Star license and an 18-month compliance history score of 99%, effective September 3, 2019. The Secretary of State website was checked and the status of NORTH MECKLENBURG CHILD DEVELOPMENT ASSOCIATION was current and active. The restrictions include: Daytime care only; Meets enhanced ratios; Meets enhanced space; Children under 2 ½ years old in rooms with direct exits only. Upon arrival, I was greeted by A. Rivens, Director. I informed Ms. Rivens the reason for my visit. I discussed the Annual Compliance Monitoring guidelines with Ms. Rivens. I completed the walkthrough of the center alone and Ms. Rivens assisted me with the rest visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed during indoor/ outdoor free play and transitions. There were forty-nine (49) children present today. There are sixty-three (63) children enrolled in the center. Outdoor Learning Environment: The outdoor learning environment was monitored today and in compliance. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 3/31/26. The centers last shelter in place/ lockdown drill was conducted on 3/30/26. The last monthly playground inspection was conducted on 3/27/26. The center last had a sanitation inspection on 11/14/25. The center received ten (10) demerits and received a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 11/4/25. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There were three (3) violations cited during today's visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #4, I observed Arm & Hammer handsoap near the sink, accessible to children that says " keep out of reach of children". .2820(b) 843 A drug or medicine was administered after its expiration date. During the walkthrough of the center I observed five (5) OTC creams that were expired. 10A NCAC 09 .0803(1)(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #6, I observed one (1) expired medication authorization form. .0803(12) All violations were corrected during the visit. Therefore no further action is required. Technical Assistance: Today I discussed pathway to the stars with Ms. Rivens, the center has decided to pursue pathway #1 and will be applying by September 2026. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 3/28/25. The centers last shelter in place/ lockdown drill was conducted on 3/31/25. The last monthly playground inspection was conducted on 3/17/25. The center last had a sanitation inspection on 4/1/25. The center received eleven (11) demerits and received a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 11/18/24. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 3/26/24. The centers last documented lockdown was conducted on 3/28/24. The last monthly playground inspection was conducted on 3/28/2024. The center last had a sanitation inspection on 1/11/2024. The center received 5 demerits and received superior classification. The center last had a fire inspection on 11/15/23. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and not in compliance. There were four (4) violations cited during today's visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In space R3, I observed peeling paint on the blue cubbies, and through out the building, I observed peeling paint on the baseboards of the classrooms. I also observed peeling paint on the handrails on the steps near the preschool classrooms. 15A NCAC 18A .2825(a) 721 All equipment and furnishings were not in good repair. In space R1, I observed a torn blue couch. The brown wooden fence on the playground was not in good repair. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space R5, I observed drops for a fish tank on the counter, not properly stored/ accessible to children. Also, in space R5, the container that holds the sunscreen was broken and not properly locked. .2820(b) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Three out of seventeen staff members did not have an annual staff evaluation or a staff development plan. (TL, GH, BM) 10A NCAC 09 .0514(f) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 5/7/2024, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information.
