The center currently has a Four-Star rated license, the 18-month compliance history score is 96%. The center license and Summary of the Law was prominently posted. I was greeted by N. Abdullah, Administrator. I informed Ms. Abdullah the reason for my visit. Ms. Abdullah was on her way to take the children outside for outdoor play time, so I began the walkthrough of the center alone. Ms. Abdullah assisted me with the visit as needed. Permit information-the program operates with a Four- Star license, issued March 15, 2022. The center has seven (7) points for education, three (3) points for program standards and one (1) quality point. License restriction include: Daytime care only; School age children only; Meets enhanced space. The program is still located at: 635 South ST, Davidson, NC 28036. The phone number is (980) 343-3900. The center had twenty-three (23) children present, forty-six (46) children enrolled. The following items were monitored: Supervision, S/C Ratio, Storage of Hazardous Substances, Storage of Medication, Adequate Approved Space, Program Records, License Posted and Permit Restrictions. The center has not hired any new staff members since their last visit. The center’s last fire drill was documented for 5/23/2024. The center’s last lockdown drill was on 1/8/2024. The last sanitation inspection was completed on 5/15/2024 with 4 demerits, receiving a superior rating. The center playground inspections were monitored and in compliance. There were two (2) violations cited during todays visit. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. In space 100, I observed a container of magnetic letters with a broken top. 10A NCAC 09 .0604(p) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The cneter's last documented lockdown drill was on 1/8/2024. .0604(u);.0302(d)(8) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 6/12/24, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. The center operates with a Three-Star license, effective March 15, 2022. The restrictions include: Daytime care only; School age children only; Meets enhanced space. I was greeted by N. Abdullah. Ms. Abdullah assisted me with the walkthrough of the center. I discussed with you, Ms. Abdullah, the Annual Compliance Monitoring guidelines. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed engaged in multi-play opportunities. Teachers were observed nurturing and engaged interactions with children. There were twenty-eight (28) children present today. There are thirty-five children enrolled. Outdoor Learning Environment: The outdoor learning environment was not monitored today due to inclement weather. Inspections: The outdoor inspections and fire drills were monitored. The documents were current and up to date. The last fire drill was conducted on 1/8/24. The centers last documented lockdown was conducted on 1/8/24. The last monthly playground inspection was conducted on 1/8/24. The center last had a sanitation inspection on 11/6/2023. The center received 4 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 5/10/23. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Staff files were monitored and not in compliance. Student files were monitored and not in compliance. There were four (4) violations cited during today's visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. I observed Zyrtec medication with no parent authorization form. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. I observed Benadryl medication with an expiration date of 10/2023. 10A NCAC 09 .0803(1)(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One out of three staff members did not have documentation of completing all the the required hours of on-going training. (J.B) .1103(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two out of three staff members did not have documentation of completing all of the required health and safety trainings within one year of employment.
