This visit was conducted with Ms. Wendy Cunningham, Teacher. This program currently operates with a Five Star license. Restrictions include daytime care only, meets enhanced ratios minus one, meets enhanced ratios, meets enhanced space. The license was posted and the restrictions were being followed. The program’s compliance history was 92% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least 75%. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were playing outdoors and meeting with the dentist, who is on site. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • SIDS training • Criminal Background Checks • General health • Administration of medication • Storage of hazardous products • Storage of medication • General safety • General license requirements • Program records • License posted • Fire safety • Sanitation Limited monitoring of staff files was conducted in accordance with DCDEE procedures for the purpose of reviewing CPR and First Aid, EPR, SIDS training, Health and Safety trainings and Criminal Background Checks. The storage and administration of medications was monitored. Medication authorization was monitored. The storage of hazardous items was monitored. A fire drill was documented on 08-28-2024. A shelter in place drill was documented on 08-29-2024. An outdoor inspection was documented on 09-03-2024. This program does not provide transportation. The following violations were documented during this visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. One child was not signed in this morning. 10A NCAC 09 .0302(d)(4) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Ms. Cunningham's annual Health Questionnaire is more than two years old. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. M.s Cunningham's Emergency Form is more than two years old. .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. htere is no Shaken Baby form on file for Ms. Cunningham. .0608(d)(1-4) I have asked Ms. Cunningham to send me written documentation by October 4, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Cunningham that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. An outdoor inspection was documented on 03-01-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. Ms. Nelums stated that there is no medication on site today. This program does not provide transportation. I documented the following violations during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan from last week was posted. GS 110-91(12); .0508(a) 434 The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There were not three different activities for children to choose in the art activity center. 10A NCAC 09 .0509(2) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter-in-place drill was conducted on 9-11-2023. .0604(u);.0302(d)(8) I have asked Ms. Nelums/Tracy Speight to send me written documentation by April 8, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. Consultation: The site needs to have another fire inspection as soon as possible. This is an NC Pre-K Program. This site completed the last Environment Rating Scale on 05-19-2022 and scored a 6.20. Today, Ms. Nelums and Ms. Lange were present with eleven children. There are thirteen children enrolled in the NC Pre-K Program at this site. Attendance was observed today. I have observed the NC Pre-K children’s files today and the following items were observed: Health Assessments Immunizations Vision Screenings Hearing Screenings Dental Screenings This site currently uses Teaching Strategies Gold for their Developmental Screening. I have observed a sampling of these today. This site currently uses Creative Curriculum for their Curriculum. I have observed the lesson plans today. This site currently uses Dial 4 for their Formative Assessment. I have observed a sampling of these today. Ms. Nelums has stated today that family engagement activities include a parent conferences, parent club monthly meeting, biweekly video to educate parents and a parent board. The teachers stated that they work 40 hours per week. Ms. Nelums has a BS degree in Birth through Kindergarten. Ms. Lange has an AAS degree in Early Childhood Education. I have reviewed this visit summary with Ms. Nelums at the conclusion of the visit and a copy was given to her. Today I provided Technical Assistance in the following areas: I have observed the sign in and sign out sheet today. The center is using a notebook and each child is written on a separate page. When staff is busy during the day, they do not have time to check each separate tab during the day to make sure that each child is signed in and out properly. If a child is not signed in or out by the teacher or the parent, then it would be a violation. Today, I have observed a child’s paper that has the signature on the form with the time that the child was dropped off, but no name of the child and no date. Staff report that this person has three children here and that they do not have time to write each child’s full name and the date along with signature and time on three separate papers. I have suggested that all children enrolled be written on one form with the date at the top so that parents can write either their signature or their initials, and the time. One staff person suggested that the central office was concerned about privacy and the child’s first and last name on the form for all to see, however, only one part of the information could be on the form to meet the rule; for example, K. Healy, Kelly, Healy, or Kelly H. I have reviewed this visit summary with Ms. Simms at the conclusion of the visit and a copy was given to her.
