You, Ms. Jones, the administrator, assisted me with the visit. Currently this facility operates with a GS 110 Notice of Compliance that was issued 6-29-15. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- seven percent (97%) as of 3/24/26. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed on (3/24/26) and Christian Faith Center of Creedmoor, Incorporated was listed as current- active. A full assessment and walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, art, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Fire (11/10/25) inspection remains current. The last sanitation inspection was completed on 3/3/26. I reviewed one new staff file during today’s visit. Existing staff files were not fully reviewed during the visit, just training and certificates, because a Staff and Training Worksheet was e-mailed to the administrator prior to today’s visit. I received the Staff and Training Worksheet and reviewed it for compliance before today’s visit. A sampling of Children’s files (ten percent of enrollment) was reviewed today. The last fire drill was conducted on 3/18/26. The shelter and lockdown drills were conducted on 3/18/26. The last outdoor inspection conducted was 3/19/26. A checklist was used to note the requirements I monitored today. Four violations were cited during today’s visit. Technical assistance and a consultation were given to the provider to ensure compliance. The provider and I discussed the following topics below to ensure compliance. 10A NCAC 09 .2703 CRIMINAL HISTORY RECORD CHECK REQUIREMENTS FOR CHILD CARE PROVIDERS (f) Provisional child care providers are eligible for employment at a child care facility and are eligible to reside in a family child care home, nonlicensed home, or child care center in a residence and shall be counted in staff/child ratio. Provisional child care providers shall be supervised at all times by an individual who received a qualifying result on a criminal background check within the past five years and may not be left alone with children. Child care facilities found to be in violation of this Paragraph may be issued an administrative action up to and including revocation of their child care license or notice of compliance in accordance with Section .2200 of this Chapter. 10A NCAC 09 .0803 ADMINISTERING MEDICATION IN CHILD CARE CENTERS The following provisions apply to the administration of medication in child care centers: Prescribed medications: (a) shall be stored in the original containers in which they were dispensed with the pharmacy labels; 15A NCAC 18A .2819 DIAPERING AND DIAPER CHANGING FACILITIES (b) (d) Child care center employees shall change a child's diaper as follows: (c) (1) gathering supplies before placing child on diapering surface; (d) (2) donning disposable gloves (if needed); (e) (3) using disposable towelette or moistened paper towel to clean child, wiping front to back; (f) (4) disposing of gloves if used, soiled towelettes and diaper in a plastic-lined, covered receptacle; (g) (5) wiping the child care center employee's hands and the child's hands each with a separate disposable towelette or moistened paper towel; (h) (6) sliding a clean diaper under the child, applying diapering products if needed, using facial or toilet tissue, and discarding the tissue in a plastic-lined, covered receptacle; (i) (7) fastening the diaper and placing clothing on child; (j) (8) washing child's hands in accordance with Rule .2803 of this Section, or, if child is unable to support the child's head, cleaning the child's hands with a disposable towelette or moistened paper towel, then drying the child's hands and returning the child to a supervised area; (k) (9) spraying entire diapering surface with detergent solution and wipe clean, using disposable paper towels; (l) (10) spraying entire diapering surface with an approved disinfectant and allowing to remain on the surface for two minutes or as specified by the manufacturer, or air dry; and (m) (11) washing hands in accordance with Rule .2803 of this Section even if disposable gloves are used by the child care center employee. 15A NCAC 18A .2803 HANDWASHING (c) Child care center employees shall ensure that children wash their hands as follows: (1) upon arrival at the child care center; (2) after each diaper change or visit to the toilet; (3) before eating meals or snacks; (4) before and after water play; (5) after being outdoors; and (6) after handling animals or animal cages. 10A NCAC 09 .0604 SAFETY REQUIREMENTS FOR CHILD CARE CENTERS (f) Electrical cords shall not be accessible to infants and toddlers. SECTION .0600 - SAFETY REQUIREMENTS FOR CHILD CARE CENTERS 10A NCAC 09 .0601 SAFE ENVIRONMENT (c) Equipment and furnishings shall be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions (excluding nuts and bolts on sides of fences), and pinch and crush points. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- seven percent (97%) as of 3/24/26. A full assessment and walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, art, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Fire (11/10/25) inspection remains current. The last sanitation inspection was completed on 3/3/26. I reviewed one new staff file during today’s visit. Existing staff files were not fully reviewed during the visit, just training and certificates, because a Staff and Training Worksheet was e-mailed to the administrator prior to today’s visit. I received the Staff and Training Worksheet and reviewed it for compliance before today’s visit. A sampling of Children’s files (ten percent of enrollment) was reviewed today. The last fire drill was conducted on 3/18/26. The shelter and lockdown drills were conducted on 3/18/26. The last outdoor inspection conducted was 3/19/26. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety- eight percent (98%) as of 4/14/25. Children throughout the facility were participating in group time, free play in activity areas, art, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Fire (11/6/24) inspection remains current. The last sanitation inspection was completed on 4/29/24. The sanitation inspector is behind and hasn’t been to do an inspection of the facility yet. I reviewed two new staff files during today’s visit. Existing staff files were not reviewed during the visit because a Staff and Training Worksheet was e-mailed to the administrator prior to today’s visit. I received the Staff and Training Worksheet and reviewed it for compliance before today’s visit. A sampling of Children’s files (ten percent of enrollment) was reviewed today. The last fire drill was conducted on 4/8/25. The shelter and lockdown drills were conducted on 3/5/25. The last outdoor inspection conducted was 3/25/25. A checklist was used to note the requirements I monitored today. Three violations were cited during today’s visit. Two violations were corrected during today’s visit. Technical assistance and a consultation were given to the provider to ensure compliance. The administrator in formed me that it had been removed because it is going to be repaired on 4/25/24. During today's visit maintenance placed a temporary ceiling tile to cover the opening. The administrator informed me that the new one piece will be repaired tomorrow. 15A NCAC 18A .2825(a) Technical Assistance Any products labeled “keep out of reach of children” and doesn’t have any other warnings on the label need to be kept inaccessible to children.
