Ms. Sheila Dunlap, operator, assisted me with the visit. This program currently operates with a 5-star rated license, issued June 14, 2017. The 5-star license was based on the program earning 7 points in Education Standards, 7 points in Program Standards and 1 point in Quality. License restrictions include Daytime and Overnight, maximum of 5 preschool children at any time, serves no more than 2 infants under age 1; swimming pool not used during operating hours, no more than 3 children less than 1 year old. A walk-through of the home was completed today. All indoor and outdoor areas were monitored. I observed two school-age children arriving from school. Children washed their hands upon entering the home. During today's visit, the following was monitored: program/space requirements, equipment and furnishings, capacity and supervision, medications, outdoor area, and equipment, as well as transportation. The FCCH Annual Compliance Monitoring Checklist was used during today’s visit and a copy was left with Ms. Dunlap. The following violations were observed during today's visit. Computer-generated report was printed at the conclusion of today's visit and a copy was left with Ms. Dunlap. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The rabies vaccination for the cat expired August 2025. .1719(b)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator did not complete the ABCMS Provider Portal. G.S. 110-90.2 & .2703(r) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. 10A NCAC 09 .1725(a)(2) All violations documented above must be corrected immediately. A signed and dated letter of compliance must be submitted to me by mailed to me by December 4, 2025, describing how each of the violations were corrected. The letter of compliance can be emailed to: Miriam.Byrd@dhhs.nc.gov If you state in your letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all applicable laws and rules at all times. The best way for you toe make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development’s website (www.ncchildcare.nc.gov) on a regular basis to find out what’s new in childcare. You could also find any needed child care forms by going on the web site and clicking on the “Provider” tab and then clicking on the “Provider Documents” tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history score is 100% from 5/16/2024-11/15/2025. Rated License Assessment: Today, I reviewed the new QRIS Pathways, and you requested to be assessed through Pathway #2- Classroom and Instructional Quality. We reviewed the FCCH rule requirements to earn a five-star license. You meet the following 5 star requirements: *Serve no more than 4 children under the age of 24 months of the children who are birth -5 years of age *You completed the Family and Community Engagement Foundational Standards form and selected three additional options with one for each category of engagement. I reminded you to document how and when you complete each option. You were provided a copy of your selections. *You completed your CQI and PD plan during today’s visit. *You chose to use the following approved Curriculum and Formative Assessment: The Creative Curriculum for Preschool, 4th Edition and Creative Curriculum Developmental Continuum for ages 3-5. I suggested you share the websites of the curriculum and the formative assessment with your families. They can complete observations at home and partner with you when you share ongoing assessment with them twice a year. Please keep the child’s assessment with anecdotal observations in a file, easily accessible. You can focus on a few each week. *Your education was previously assessed by DCDEE WORKS. You completed an AAS in ECE in 2011, and you have 30+ years of early childhood work experience. Please be reminder that the five-star level also requires you to choose additional training or mentoring. You chose to complete five additional training hours plus your required 8 hours of annual on-going training. The Application packer for a FCCH Rated License was completed and signed. A copy was left with you. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
