The facility currently has a four-star license and an 18 month compliance history of 89%. The license and NC childcare law summary were prominently posted. The Secretary of State database was monitored and JEEPFROG DEVELOPMENT, LLC is current and active. I was greeted by K. Prokos, director. Ms. Prokos accompanied me during the walkthrough of the facility. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-down drills were documented quarterly as required. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 2/27/23. The center received 20 demerits and received approved classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 10/27/22. There were 103 children in attendance today. The children were observed participating in free play, teacher directed activities, water break and outdoor play. The playground was monitored during today’s visit. The facilities transportation vehicles were monitored today. I monitored all new staff files since my last visit. I monitored ten percent of existing staff files. Ten percent of the enrolled children’s files were monitored at random today. There were 5 violations cited today. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space 11, there was a diaper cream that had expired. There was a diaper cream with expired permission to give dates. 10A NCAC 09 .0803(1)(d) 1030 Application for employment and date of birth was not on file for all staff. There was no application on file for A. Wellman, H. Willhelm, T. Burns and M. Berstein. All completed the application during the visit except for M. Berstein. .0302(d)(1)(A) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. There was no medical form on file T. Burns. 10A NCAC 09 .0701(a) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. There was no documentation on file for the required orientation training for M. Berstein. .1101(a)(b) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child's file had an incomplete medical report.
