The facility currently has a four-star license and an 18 month compliance history of 100%. The license and NC childcare law summary were prominently posted. The Secretary of State database was monitored, and LEAFSPRING SCHOOL CORNELIUS, INC. is current and active. I was greeted by A. Epps, assistant director. Ms. Epps accompanied me during the walkthrough of the facility. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter in place and/or lockdown drills were documented as required. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 7/22/2025. The center received 8 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 3/11/2025. There were 64 children in attendance today during the walkthrough. I observed the children participating in napping, arrival and handwashing. The playgrounds were monitored during today’s visit. I monitored the facilities 3 transportation vehicles. I monitored all staff files. Ten percent of the enrolled children’s files were monitored at random today. There were 3 violations cited during today’s visit. All violations were corrected during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. There was no current activity plan posted for the school age group of children. GS 110-91(12); .0508(a) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. There was no documentation on file of an annual EMC plan review for 3 staff members. 10A NCAC 09 .0802(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. There was no documentation on file of an annual EPR review for 3 staff members. .0607(e) Technical Assistance: I shared with Ms. Epps to try and place cots at least 18 inches apart and/or use shelving or dividers. Ms. Olajile Montesino, Head of School Director, accompanied me to this location. Ms. Ashley Epps, Program Coordinator, was also present upon our arrival. I stated the purpose of today's visit and began my walk-through of the indoor and outdoor learning spaces. Currently this center operates with a Temporary License, issued on October 30, 2024. The restrictions include daytime care only; Children 3-12 years old only. I also spoke with Ms. Montesino about adding a restriction on the license to indicate that the upstairs school age classroom is designed for Second Graders only. The NC Secretary of State website was viewed before the visit LeafSpring School Cornelius, INC, is current as of 9/14/24. If any changes to the company need to be made or you decide to sell your business, then you must notify your consultant of at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: There are 76 children enrolled in at the facility. Out of the 76 enrolled children, 29 children were present. During the arrival children were engaged in nap routines and transitioning from nap routines. Upon arrival, staff/child ratio was observed and was in compliance. I reviewed all program records requirements. The last fire drill was conducted on 2/7/25. The last playground inspection was completed on 3/31/25. The Sanitation inspection that took place on 11/13/24 with 2 demerits and a Superior Rating. The facility is still waiting for final approval for a Fire Inspection and a Building Inspection. These inspections will be required prior to the expiration of your temporary license of April 30, 2025. Non-receipt of the fire inspection and building inspection will require me to place the facility on a Provisional License Status until receipt. Please make sure you are adhering to deadlines to complete any requirement requested for an approved fire and building inspection. Outdoor Learning Environment: Was monitored and in compliance with Child Care Requirements. I monitored all three (3) transportation buses during today’s visit. All three (3) buses were in full compliance. Records: Staff: The Staff and Training Worksheets were completed prior to today’s visit. This facility did not have any new staff hired since my last visit. I monitored 10% of Children’s records during today’s visit and no violations were observed during this review. Nutrition: Your facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. Meatballs, Hawaiian Rolls, Cole Slaw, Mix Fruit and Milk. The full menu was observed in the hallway but updated in the classrooms. I reviewed, and it met the meal pattern requirements. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A preschool age child whose family enrolled her on 9/9/24, did not have a medical exam/health assessment record on file during today's visit. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Rated License: We discussed the two-component rated license process during today’s visit. The Head of School Administrator (O. Montesino/Owner Representative Rhonda Rivers stated they will continue to remain in the harmless hold until it is no longer available. I explained to the administrator that I will keep the previous score and education from their last ERS assessment/Rated License. It is important to make sure staff are continuing to take EDU course work or work towards completion of an AA degree in Early Childhood. I suggest to research T.E.A.C.H and other community college resources to assist with encouraging staff who will like to continue their early childhood education. I explained and discussed enhanced ratios and enhanced space with you. I explained that if you receive a four or five-star license, then you will be required to have an approved curriculum for children four years of age. The facility has already adopted the Creative Curriculum for Preschool. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. You must have earned enough points for 3-5 stars in order to continue receiving subsidy dollars. To help childcare providers meet the new training requirements, the Division of Child Development and Early Education has Moodle Trainings set up and can be accessed through the Divisions website: ncchildcare.ncdhhs.gov.
