Last Annual Compliance Visit – 11/12/24 18-month compliance history from 4/1/24 – 9/30/25 = 100%. The FCCH Operator, M. Johnson, was present and assisted me with the visit. The family child care home currently operates with a Four-Star License, issued 11/29/22. The Child Care Commission has adopted new rated license rules called QRIS (Pathway to the Stars) rules in Section .3200 and is moving forward with rated license reassessments using the new rules. Your rated license reassessment will be due by December 2026. Information, training, resources, and updated forms will be shared with you as we transition to the new rules. See information in the QRIS Technical Assistance section. The family child care home is owned as a Sole Proprietorship. M. Johnson reported no changes to owner information, facility contact information (phone, address, email) or license information. Please contact me prior to any information change regarding the ownership of this facility or contact information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 7/1/2025 and Item Number Listing, effective 4/2025. I also reviewed children’s records, operator records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation. The operator stated that she never uses a substitute and closes the family child care home when she is not available to provide care. The Written Plan of Care currently shows that the operator does not perform routine tasks during operating hours. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations I completed a walk-through of the home and monitored all indoor and outdoor spaces used for child care. Four children, one through three years of age, were present and were working on coloring a horse at the table before transitioning to music and movement activities and free play. Permit capacity and supervision were in compliance. The operator interacted positively with the children, providing encouragement and guidance. Lunch was chicken and noodles, collards, fresh orange slices, whole wheat toast, and milk. Children rested on linen-covered cots in the playroom. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A shed used to store paint was not locked, making it accessible to children when on the playground. .1719 (a)(7) 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. The battery-operated refrigerator thermometer was not operating. 10A NCAC 09 .1725(a)(10) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The operator's qualification letter expired on July 27, 2025. A five-year criminal background check was not completed prior to the expiration date, and a current qualification letter was not available until September 26, 2025. G.S. 110-90.2 & .2703(n) & (o) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, M. Johnson. We reviewed the visit summary and violation(s) documented during today’s visit. You corrected two of the three violations and only need to correct #810 – Refrigerator Thermometer. Correct this immediately. Send evidence of correction to me so that I receive it no later than 10/15/2025 Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how the violation has been corrected. Also include the following picture to verify compliance. Include a picture of the thermometer as described in the technical assistance information. Send compliance verification letter in an email from your fcch’s official email address, nannie47@embarqmail.com to my email address, Marjorie.White@dhhs.nc.gov. You may include the compliance verification as an attachment or in the body of the email. I will email a sample compliance verification letter to you. Technical Assistance With Documented Violations: Storage of Hazardous Items - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. The substances most commonly involved in poison exposures of children are cosmetics and personal care products, cleaning substances, and medications. The shed that is accessible to children when they are outdoors was not fully locked. Cans of house paint were stored on the floor inside the door. The key lock was on the shed but was not fully locked. You stated that you will sometimes leave the shed unlocked over the weekend because your son comes over and assists with mowing your yard. Because of the rain, he had not been here this past weekend, and you forgot about relocking the lock. You locked it during the visit. Refrigerator Thermometer – Maintaining food in a refrigerator kept at 45 degrees or colder helps to slow the growth of bacteria, which prevents foodborne illness, spoilage, and food waste. A refrigerator thermometer helps you ensure that the refrigerator is maintaining cold temperatures. The battery-operated thermometer in the door of your refrigerator was not operating and could not show that your refrigerator was at 45 degrees or colder. You stated that the battery must have died, and you will either replace the battery or purchase a non-electronic refrigerator thermometer. For your compliance letter, state the date you purchase a new battery or thermometer and send a picture of the thermometer in your refrigerator, reflecting a temperature of 45 degrees or cooler. Criminal Background Check (CBC) – Criminal Background Checks ensure that child care operators, staff, or household members have not committed any crimes which could potentially make them unsafe to be around children. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Your CBC expired on 7/27/2025 and was not renewed until 9/26/2030. You stated that you had forgotten about it, but when informed that is was expired, you immediately completed an on-line application and fingerprints. Use your FCCH Verification form to track CBC due dates. Schedule five-year Criminal Background Checks so that you have plenty of time to complete fingerprints and resolve any possible errors. I observed your Qualifying Letter, dated 9/26/25. This item has been corrected so no further action is required except to maintain compliance with rules regarding Criminal Background Checks. Repeated violations related to Criminal Background Checks may result in an Administrative Action. We completed the QRIS Conversation Template reviewing documents and resources available to you and you listed the areas where you feel you need support. Reminders: FCCH Verification Form – We reviewed your FCCH Verification Form today that is updated and provided to you with the visit summary. Use it to track items that expire or need to be updated annually, e.g., on-going training, H&S Training, CPR/FA, ITS/SIDS, CBC, EPR Plan, vehicle information, etc. I highlighted items that will expire in the next year and reviewed them with you. *On-going Training – Carry forward 4 hours to 10/1/25 – 10/1/26. ABCMS Portal – ABCMS Portal - The process of notifying the Division when you have new staff (or household members for family child care homes and centers licensed in a residence) has changed and is now captured in ABCMS, the new Criminal Background Check portal. You have completed the ABCMS Provider Portal Training and are now authorized to log in the ABCMS Provider Portal https://ncabcms.nc.gov/DCDEE/ using your Business NCID. You said that you are working through the process of getting yourself and household members linked to your facility in the ABCMS portal. I am emailing the ABCMS Provider Portal Access Guide which should provide you the step-by-step instructions to complete this. • Some providers have completed the Moodle Training again or have gone back to complete the Powerform where you list your business NCID email. • If you need assistance using the ABCMS Provider Portal, contact the Criminal Background Check Unit at (919) 814-6401 or DCDEE_ABCMS_Provider@dhhs.nc.gov and someone will assist you. Emergency Preparedness and Response (EPR) Requirements, Emergency Medical Care Plan (EMCP) and Ready-to-Go File – Your Emergency Preparedness and Response (EPR) Plan needs to be reviewed and updated annually in the NC Risk Management Portal. *Your EPR Plan was reviewed and updated in the NC Risk Management Portal in April 2025. Review in the NC Risk Management Portal and update or document date of review by April 2026. I will monitor at your next visit. *Your Ready-to-Go File was current. Additional Comments: Annual Licensing Fee - All licensed child care facilities, except for school or church-sponsored centers, that have a current license (whether active or inactive) on October 1st of each year will receive an invoice for a licensing fee based on their permitted capacity. 12 or fewer children - $52 This must be paid annually. All licensed child care facilities must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the program. Last Annual Compliance Visit – 11/12/24 18-month compliance history from 4/1/24 – 9/30/25 = 100%. The FCCH Operator, M. Johnson, was present and assisted me with the visit. The family child care home currently operates with a Four-Star License, issued 11/29/22. The Child Care Commission has adopted new rated license rules called QRIS (Pathway to the Stars) rules in Section .3200 and is moving forward with rated license reassessments using the new rules. Your rated license reassessment will be due by December 2026. Information, training, resources, and updated forms will be shared with you as we transition to the new rules. See information in the QRIS Technical Assistance section. The family child care home is owned as a Sole Proprietorship. M. Johnson reported no changes to owner information, facility contact information (phone, address, email) or license information. Please contact me prior to any information change regarding the ownership of this facility or contact information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 7/1/2025 and Item Number Listing, effective 4/2025. I also reviewed children’s records, operator records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation. The operator stated that she never uses a substitute and closes the family child care home when she is not available to provide care. The Written Plan of Care currently shows that the operator does not perform routine tasks during operating hours. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations I completed a walk-through of the home and monitored all indoor and outdoor spaces used for child care. Four children, one through three years of age, were present and were working on coloring a horse at the table before transitioning to music and movement activities and free play. Permit capacity and supervision were in compliance. The operator interacted positively with the children, providing encouragement and guidance. Lunch was chicken and noodles, collards, fresh orange slices, whole wheat toast, and milk. Children rested on linen-covered cots in the playroom. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A shed used to store paint was not locked, making it accessible to children when on the playground. .1719 (a)(7) 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. The battery-operated refrigerator thermometer was not operating. 10A NCAC 09 .1725(a)(10) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The operator's qualification letter expired on July 27, 2025. A five-year criminal background check was not completed prior to the expiration date, and a current qualification letter was not available until September 26, 2025. G.S. 110-90.2 & .2703(n) & (o) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, M. Johnson. We reviewed the visit summary and violation(s) documented during today’s visit. You corrected two of the three violations and only need to correct #810 – Refrigerator Thermometer. Correct this immediately. Send evidence of correction to me so that I receive it no later than 10/15/2025 Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how the violation has been corrected. Also include the following picture to verify compliance. Include a picture of the thermometer as described in the technical assistance information. Technical Assistance With Documented Violations: Storage of Hazardous Items - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. The substances most commonly involved in poison exposures of children are cosmetics and personal care products, cleaning substances, and medications. The shed that is accessible to children when they are outdoors was not fully locked. Cans of house paint were stored on the floor inside the door. The key lock was on the shed but was not fully locked. You stated that you will sometimes leave the shed unlocked over the weekend because your son comes over and assists with mowing your yard. Because of the rain, he had not been here this past weekend, and you forgot about relocking the lock. You locked it during the visit. Refrigerator Thermometer – Maintaining food in a refrigerator kept at 45 degrees or colder helps to slow the growth of bacteria, which prevents foodborne illness, spoilage, and food waste. A refrigerator thermometer helps you ensure that the refrigerator is maintaining cold temperatures. The battery-operated thermometer in the door of your refrigerator was not operating and could not show that your refrigerator was at 45 degrees or colder. You stated that the battery must have died, and you will either replace the battery or purchase a non-electronic refrigerator thermometer. For your compliance letter, state the date you purchase a new battery or thermometer and send a picture of the thermometer in your refrigerator, reflecting a temperature of 45 degrees or cooler. Criminal Background Check (CBC) – Criminal Background Checks ensure that child care operators, staff, or household members have not committed any crimes which could potentially make them unsafe to be around children. Because Abuse and Neglect registries are also checked, it helps to ensure that children are protected from physical, mental, and sexual abuse. Performing diligent background screenings also protects the child care facility against future legal challenges. Your CBC expired on 7/27/2025 and was not renewed until 9/26/2030. You stated that you had forgotten about it, but when informed that is was expired, you immediately completed an on-line application and fingerprints. Use your FCCH Verification form to track CBC due dates. Schedule five-year Criminal Background Checks so that you have plenty of time to complete fingerprints and resolve any possible errors. I observed your Qualifying Letter, dated 9/26/25. This item has been corrected so no further action is required except to maintain compliance with rules regarding Criminal Background Checks. You stated you had forgotten to lock the bedroom after storing a car seat inside the door. You corrected this during the visit. All medications (topical ointments and creams, OTC, and prescription) must be inaccessible to children. Diaper creams and lotions may be stored out of reach, on a shelf or in a cabinet at least five feet or higher. Emergency medications such as asthma inhalers, Epi-Pens, Diastat, and glucagon may be stored out of reach but easily accessible to a teacher in the event of a medical emergency. OTC and prescription medications must be locked. Check doors to bedrooms before children arrive to ensure they are locked. Best practice is to put all household medication is a locked cabinet or lock box. Attendance – Having accurate attendance is important to have as documentation if there is a question about children’s attendance related to payments, subsidy, or even parental disputes. More importantly it helps to accurately account for children in the event of an emergency where you might have to evacuate the home. Documentation of arrival was not available for two children present today. You stated you were busy when they arrived and forgot to write their arrival times. Documentation of arrival and departure helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Regardless of who does it, current, up-to-date attendance records must be available, and they must match what is submitted to other agencies for subsidy or food program requirements. Maintain attendance records for at least one year.
