Name of Operation: A PLACE FOR KIDS HOME DAYCARE Facility ID: 33000446 Consultant: KESHIA HAYWARD Operation Type: Family CC Home Case Number: Visit Date: 1/13/2025 Number Present: 4 Completed Date: 1/13/2025 Age: From 1 To 4 Total Minutes: 135 Time In: 08:00 AM Time Out: 10:15 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor your program for compliance with applicable child care requirements during an annual compliance visit. S. Mitchell, Owner/Operator assisted with the visit. Four (4) children were observed in care today ranging from ages one to four years old. Children were engaged in morning activities, free-choice play, completing toileting, and hand washing routines, interacting with the caregiver, napping and eating breakfast. Breakfast consisted of noodles, apples and milk. Currently this program operates with a four-star license earning seven points in education, two points in program standards and one quality point for serving no more than two infant under age one. The following violations were documented. Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Arrival and departure records for the month of December 2024 were not available for review. .1721(e)(6) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Plastic bags were observed in a child's cubby and on the kitchen table. .1719(a)(18) 1953 Operator did not ensure child's hands were washed upon arrival at the home each day. Two children's hands were not washed upon arrival to the facility. .1725(a)(4) 1993 Vehicle used for travel was not free of hazards such as but not limited to, torn upholstery, broken windows, holes in the floor or roof, or tire treads of less than 2/32 of an inch. The two-front tires on the vehicle used to transport children did not meet the 2/32 of an inch as required. .1723(1) The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. The compliance verification letter must be received on or before January 27, 2025. The two-week time frame is established to allow you time for submitting your compliance verification letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware that any written information submitted by you regarding correction of violations documented during the visit is legal documentation. Therefore, it is important that all information submitted be accurate and truthful. Please be aware that if it is determined the information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's Star Rated License could be issued. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY: The program's compliance history was reviewed with the operator. The program’s compliance history was 92% percent as of January 12, 2025. OWNERSHIP STATUS: You are listed as the sole owner of this child care program. Prior to making any changes to the ownership status (forming a corporation, etc.), contact me to discuss change of ownership procedures. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your program are correct. If changes in your facility information occurs, please contact me at 252-214-2709 or email keshia.hayward@dhhs.nc.gov to discuss the changes and ensure the information is updated in the system. TRANSPORTATION: The tire on the 2016 Ford transit van used to transport children did not meet the 2/32 of an inch tread requirement as required. You stated you were aware tires were needed on the vehicle and plan to get new tires soon. I reminded you although you do not transport children on a consistent routine, the vehicle must always meet requirements in case an emergency arise, and the children must be transported from the facility. SAFE ENVIRONMENT: Plastic grocery bags were observed in a child’s cubby and on the kitchen table. You removed the plastic bag from the child’s cubby when prompted. We discussed checking daily to ensure the environment provided is safe for children in care. It was recommended talking to parents about potentially hazardous products and ask for assistance with ensuring the items brought are safe. HAND WASHING: Two children’s hands were not upon arrival. The children were allowed to play with materials when they entered the facility. I reminded you children hands should be washed upon arrival to the facility and prior to playing with materials and toys. Hand washing prevents the spread of germs and illnesses. ENROLLMENT: You have eight preschool children enrolled on first shift. You stated two of the children attend preschool programs and come in the afternoon. You added one of the children is your grandchild and come periodically. You shared if needed your mother will provide care at her home for your grandchild. You explained the children never all attend at the same time. I reminded you no more than five preschool children can be cared for at one time. CRIMINAL BACKGROUND REQUIREMENTS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
