Name of Operation: WOLF MEADOW KIDS:PLUS Facility ID: 1355084 Consultant: DEBORAH HOWELL Operation Type: Center Case Number: Visit Date: 4/24/2024 Number Present: 23 Completed Date: 4/24/2024 Age: From 5 To 11 Total Minutes: 85 Time In: 03:10 PM Time Out: 04:35 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance for applicable child care requirements during a Routine Unannounced visit. Your last ACV was conducted on December 13, 2023. Your facility’s compliance history score prior to today’s visit was 99%. You, Tiera Cannon, Program Coordinator, assisted me with the visit. I observed your Five-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, daily schedule, activity plan, staff-child ratio worksheet, First Aid poster, tobacco free policy signage and evacuation plans posted. Today children were adequately supervised, enhanced staff-child ratios minus one (1), adequate approved space use and permit restrictions were observed being maintained during today’s visit. Activity areas were set up in the gym with materials including blocks, manipulatives, art, and games. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces and in good repair. Children were actively engaged in outdoor play, indoor play and observed during transitions and personal care routines. There are no children currently enrolled who have medications or Medical Action Plans. Inspections/Drills/Logs Sanitation Inspection- November 27, 2023, with a Superior rating and five demerits. Fire Inspection- September 24, 2023 Playground Inspection- March 26, 2024 Fire Drill- March 18, 2024 Emergency Drill- March 21, 2024 Incident Log- Completed as required. No new staff have been hired since my last visit. I was able to verify current CPR/FA certifications, Criminal Background Checks, BSAC, and Recognizing and Responding to Suspicions of Child Abuse and Neglect. Your signature on all forms served and will serve as verification the information provided was accurate and complete. There was one violation observed or cited during the visit. Violation Number Comment Rule 832 There was no written emergency medical care (EMC) plan. There was not an emergency medical care plan posted for review. 10A NCAC 09 .0802(a) Technical Assistance was provided on the following: 1. Create a folder of all required postings and set a calendar reminder to review them monthly, or the same day as the playground inspection or fire drill. This way if they have gotten lost or removed you will have a copy to use as a replacement. Since you have some 5th grade children this could be one of the classroom responsibilities they have each month. Compliance Plan: There is no compliance letter due at this time. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
