Name of Operation: WINECOFF KIDS:PLUS Facility ID: 1355090 Consultant: DEBORAH HOWELL Operation Type: Center Case Number: Visit Date: 10/9/2024 Number Present: 26 Completed Date: 10/9/2024 Age: From 5 To 10 Total Minutes: 120 Time In: 03:00 PM Time Out: 05:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance for applicable childcare requirements during an annual compliance visit. I used the “Annual Compliance Monitoring Visit Checklist for Child Care Centers" during today's visit. Your last ACV was conducted on November 13, 2023. Your facility’s compliance history score prior to today’s visit was 100%. You, Sharon Hooker, Program Coordinator, assisted me with the visit. I observed your Five-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, Emergency Medical Care Plan, daily schedule, activity plan, staff-child ratio worksheet, First Aid poster, tobacco free policy signage and evacuation plans posted as required. Today, children were adequately supervised, enhanced staff-child ratios minus one was maintained, adequate approved space was used, and permit restrictions including enhanced space requirements were maintained during today’s visit. I observed children having snack, outdoor play, hand washing and play in activity areas. Today’s snack consisted of Nutrigrain bars and milk. I observed activity areas set up in the cafeteria with materials including blocks, manipulatives, art, and games. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces and in good repair. I monitored medications stored on site. I observed the playground used by children. Playground area does not meet childcare safety standards as noted on the permit restrictions. Inspections/Drills/Logs Sanitation Inspection- December 5, 2023 Fire Inspection- September 21, 2023, this inspection is past due. You stated the school did not pass and the inspector would return on October 18th, 2024. Please send me a copy of the inspection once received. Playground Inspection- September 23, 2024 Fire Drill- September 25, 2024 Emergency Drill- August 26, 2024 Incident Log- Completed as required. I observed the current Public-School Records Offsite Verification Forms for staff and children. I monitored a random selection of four children’s files. No new staff have been hired since my last visit. I monitored both staff files for on-going training hours, and other requirements. Your signature on all forms served and will serve as verification the information provided was accurate and complete. There was one violation observed or cited during the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection occurred in September of 2023. 10A NCAC 09 .0304(a) Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance. Please Note: If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. If you cannot meet the requirements by this date, you shall contact me with a timeline of the corrections. In some cases, this timeline may be extended. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
