ODELL KIDS:PLUS Facility ID: 13000353 Consultant: DEBORAH HOWELL Operation Type: Center Case Number: Visit Date: 9/22/2023 Number Present: 27 Completed Date: 9/22/2023 Age: From 5 To 8 Total Minutes: 110 Time In: 03:00 PM Time Out: 04:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance for applicable child care requirements during an annual compliance visit. I used the “Annual Compliance Monitoring Visit Checklist for Child Care Centers" during today's visit. Your last ACV was conducted on October 18, 2022. Your facility’s compliance history score prior to today’s visit was 97%. You, Amanda Motley, Program Coordinator, were at another site today. Aruna Nemani, Program Coordinator/Group Leader assisted me with the visit. Michelle Barnette, Program Coordinator at another site came to assist as well. I observed your Five-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, Emergency Medical Care Plan, daily schedule, activity plan, staff-child ratio worksheet, First Aid poster, tobacco free policy signage and evacuation plans posted as required. I observed children adequately supervised during today’s visit. I observed enhanced staff-child ratios minus one (1) maintained during today’s visit. I observed adequate approved space use and permit restrictions including enhanced space requirements maintained during today’s visit. I observed children toileting, hand washing, snack, outdoor play, indoor play, and transitions. I observed activity areas set up in the cafeteria with materials including blocks, manipulatives, art, and games. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces and in good repair. I monitored two emergency medications and Medical Action Plans. I observed your incident log. I monitored general safety throughout the licensed spaces. I observed the outdoor area used by children. Playground area does not meet childcare safety standards. Fire and sanitation inspections are occurring as required. Copies of each are in your file. Fire and emergency drills are occurring and documented as required. You stated that you do not provide transportation. I observed the current Public-School Records Offsite Verification Forms for staff and children. I monitored a random selection of four children’s files. I monitored two staff files. Your signature on all forms will serve as verification that the information provided was accurate and complete. There was one violation observed or cited during the visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. One substitute staff did not have their file available for review upon request. G.S. 110-91( 9) Compliance Plan: There is no compliance letter due at this time. Technical Assistance was provided on the following: 1. Remind substitute staff to always have their file in their physical possession in case of an emergency. It may be beneficial to maintain substitute files at each facility so they are easily accessible. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
