ODELL ELEMENTARY KIDS: PLUS Facility ID: 13000528 Consultant: DEBORAH HOWELL Operation Type: Center Case Number: Visit Date: 4/29/2024 Number Present: 28 Completed Date: 4/29/2024 Age: From 8 To 11 Total Minutes: 105 Time In: 03:05 PM Time Out: 04:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance for applicable child care requirements during a Routine Unannounced visit. Your last ACV was conducted on September 28, 2023. Your facility’s compliance history score prior to today’s visit was 100%. You, Amanda Motley, Program Coordinator, assisted me with the visit. I observed your Five-Star License, NC Child Care Summary of the Law, current menu, Sanitation placard, Emergency Medical Care Plan, daily schedule, activity plan, staff-child ratio worksheet, First Aid poster, tobacco free policy signage and evacuation plans posted as required. Children were adequately supervised, enhanced staff-child ratios minus one, adequate approved space use and permit restrictions including enhanced space requirements were maintained during today’s visit. The children were engaged in free play, homework, and outdoor play. Activity areas were set up in the cafeteria with materials including blocks, manipulatives, art, and games. I observed toys and equipment to be of sufficient quantity. All toys and equipment were developmentally appropriate in all spaces and in good repair. I monitored three emergency medications and Medical Action Plans. Inspections/Drills/Logs Sanitation Inspection- November 29, 2023 with a Superior rating and zero demerits Fire Inspection- December 6, 2023 Playground Inspection- April 15, 2024 Fire Drill- April 23, 2024 Emergency Drill- March 13, 2024 Incident Log- Completed as required. I monitored one file of a new staff who has been hired since my last visit. I was able to verify current CPR/FA certifications, Criminal Background Checks, BSAC, and Recognizing and Responding to Suspicions of Child Abuse and Neglect for all remaining staff. Your signature on all forms served and will serve as verification the information provided was accurate and complete. There was one violation observed or cited during the visit. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. An emergency medication had expired in January 2024. 10A NCAC 09 .0803(1)(d) Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them to clarify or verify compliance. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
