Brittany Adams, Child Care Consultant, accompanied me on today’s visit. Courtney Harrison and Teresa Goodman assisted us with today’s visit. The last annual compliance visit was conducted on June 14, 2023. The last sanitation inspection was completed on January 22, 2024, with a “Superior” classification. The last fire inspection was conducted January 22, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was 94% percent prior to today’s visit. The center's one star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, teacher directed activities, story time, outdoor play and mealtime. Fire drills are being completed monthly as required. Emergency drills are being conducted every three months as required. Incident logs were monitored during today’s visit. Incident reports are being filed in children’s individual files. Playground inspections are being conducted monthly. Your playground meets child care safety standards. No concerns were observed. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. Emergency medications, medical action plans, ointments, and permission to administer forms were monitored during today’s visit and found to be in compliance. A selection of staff files and children’s files were monitored during today’s visit. Transportation was monitored during today’s visit and found to be in compliance. The following violation was documented today: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space C4 there was a diaper ointment without a Permission to Administer form. 10A NCAC 09 .0803(1)(a & b) Technical Assistance was provided on the following: We discussed removing ointments from the classroom after course of treatment is completed. Consultation was provided on the following: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. I conducted your last ACV on June 14, 2023. Your facility’s compliance history score prior to today’s visit was 94%. You, Teresa Goodman, Director, were off today, Candice Bralley, Education Director, assisted me with today’s visit. I observed your One Star License, Sanitation placard, Emergency Numbers, Emergency Medical Care Plan, tobacco free facility signage, CPR/FA posters and evacuation plans posted as required. I observed children adequately supervised during today’s visit. I observed adequate approved space used during today’s visit. I observed permit restrictions including “daytime care only”, during today’s visit. I observed children engaged in snack and teachers reading books, diapering, routine toileting practices, transitions, outdoor play and washing hands. I observed activity areas including books, blocks, manipulatives, dramatic play, art, and science. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces. I monitored all topical ointments/creams stored in locked storage in the classrooms. I monitored all permission to administer forms for each ointment/cream. I monitored six emergency prescription medications. I observed the medication administration permission forms and Medical Action Plans to be current, complete, and on file as required for all six prescription medications. I observed bathrooms stocked with necessary supplies with handwashing procedure signs posted as required. Fire and Sanitation inspections are occurring as required and copies are in your file. Outdoor inspections are occurring as required. Fire and Emergency drills are occurring as required. Your facility does provide transportation. I monitored this during the annual compliance visit on June 14, 2023. I monitored valid training of the following: CPR & First Aid training, ITS-SIDS training, BSAC training, and Recognizing & Responding to Suspicions of Child Maltreatment training for all staff hired since the last ACV and all existing staff. Your signature on all forms served and will serve as verification that the information provided was accurate and complete. The following violations were observed and cited during this visit: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In Space 9, a tube of Vaseline had "as needed" in the instructions instead of a measurable amount. 10A NCAC 09 .0803(1)(a & b) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In Spaces 1 and 10 the sleep charts had not been documented for the day and infants were observed sleeping. .0606(g) I provided technical assistance regarding the following topics: 1. Ensure permission to administer forms are completed with measurable amounts instead of “as needed”. Train ALL staff who may accept medication so they will understand the expectation and the reason for the rule. 2. Daily sleep checks are required to be conducted during the nap. Document the exact time the child falls asleep and the position. A kitchen timer is a great way to set that 15-minute reminder to get up and visually check each child and then document again the time, position and initial they were checked as the nap is taking place, not later in the day. Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance. Compliance History: You are required to maintain compliance with all applicable childcare rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all childcare facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
