You, Stephanie Sullivan, Site Coordinator, assisted me with today’s visit. The last annual compliance visit was conducted on October 22, 2024. Your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 100% prior to today’s visit. The center's five star rated license, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in teacher directed activities indoors, free choice activities, routine care taking, and meal time. Fire drills are being completed monthly as required. Emergency drills are being conducted every three months as required. Incident logs were monitored during today’s visit. Incident reports are being filed in children’s individual files. Playground inspections are being conducted monthly. Materials, toys, and equipment throughout the facility was of sufficient quantity, developmentally appropriate, and in good repair. I monitored two emergency medications, medical action plans and permission to administer forms. One staff file and six children’s files were monitored during today’s visit. This program does not provide transportation. One violation was documented today: Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. For a staff member hired on 3/23/20 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics .1103(b) Technical Assistance: I highly recommend printing off all trainings that are taken, documenting them on the appropriate sheets and having them on file and available for review. Please ensure the staff member hired on 3/23/20 completes all health and safety trainings and all trainings are submitted to me when the compliance letter is submitted. On or before September 18, 2025, I must receive a compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. You, Jatanna Bryant, Site Coordinator assisted me with the visit. Your program currently operates with a five-star license, issued January 22, 2018. The last annual compliance visit was conducted on January 12, 2023. The program’s compliance history prior to today’s visit was 100%. The Kids: Plus, program is operated by Cabarrus County Schools. Fire and Sanitation inspections are occurring as required. Fire and Emergency Drills are occurring as required. I monitored each indoor and outdoor space. I monitored two emergency medications and accompanying documentation. Today I observed children, engaged in transitions, snack, outdoor play, personal care routines, and free play. The snack was observed and consisted of Goldfish crackers and fruit juice. I monitored a random selection of six children’s files. I monitored files of two staff who had been hired since the last ACV. I monitored a random selection of one existing staff file. One violation was observed and cited during today’s visit. Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff did not have her medical health questionnaire in her medical file. .0701(d) Technical Assistance was provided on the following: 1. Allow staff to place their own forms into their confidential envelopes to ensure they do not get mixed up with someone else. No Compliance Letter is required at this time. Compliance History: Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
