    Name of Operation: New Season Kids Facility ID: 13000639 Consultant: DEBORAH HOWELL Operation Type: Center Case Number: Visit Date: 5/4/2026 Number Present: 7 Completed Date: 5/4/2026 Age: From 4 To 5 Total Minutes: 180 Time In: 09:30 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor your program for compliance with all applicable child care requirements during the first temporary period visit (TTP). I was greeted by you, Latoya Mason, Owner/Operator and you assisted me with today’s visit. This facility was issued a temporary license on March 9, 2026, with the following restrictions: daytime care only, no cooking allowed in building, cannot enroll children younger than 2.5 years of age, and meets enhanced space. You stated that there are nine children enrolled, and there were seven children present today. All children’s files were monitored. All staff files were monitored. We discussed that you would need to complete the Staff and Training Worksheets for all staff and have them current and available for review by DCDEE staff. All indoor and outdoor areas of the facility were monitored for general safety. Inspections/Drills/Logs Sanitation Inspection-April 16, 2026, with a superior rating and 3 demerits Fire Inspection-February 18, 2026 Playground Inspection-April 14, 2026 Fire Drill-April 15, 2026 Emergency Drill March 18, 2026 Incident Log-Provided to you EPR Plan-In process for this location I discussed the rated license process with you. The following violations were observed: Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. There was not an incident log available for review. .0802(g)(1-6) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Staff were not linked to the facility in ABCMS. G.S. 110-90.2 & .2703(r) TECHNICAL ASSITANCE: 1. I provided you with an Incident Log and reviewed the rule requirement for maintaining the form. This violation was marked as corrected during the visit. 2. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Please submit a written, signed, and dated statement to me at the email below detailing the steps taken to correct each violation. Your letter must include the following: 1. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance. For your convenience, your letter may be sent by email to: Deborah.k.howell@dhhs.nc.gov I must receive your letter of compliance no later than June 18, 2025. Please Note: If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. If you cannot meet the requirements by this date, you should contact me with a timeline for the corrections. In some cases, this timeline may be extended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. CONSULTATION: -All staff must have their current education evaluated by the WORKS Unit in Raleigh.
