Ms. Brittany Adams, Child care consultant, accompanied me on today’s visit. You, Ms. Lorrie Carriker, director and Alice Johnson, assistant director, assisted us with the visit today. Your program currently operates with a GS-110-106 Notice of Compliance effective January 19, 2007. Your last Annual Compliance visit was conducted on January 17, 2023. There are new staff . New staff files were monitored, and a sampling of other staff files were monitored today. A sampling of children files was monitored. We visited all indoor and outdoor spaces used by children. The Annual Compliance Monitoring Checklist for child care centers was used during today’s visit. Medications were monitored during today’s visit . Children were observed during infant feeding, diaper changing, group time, free play activities, staff child interactions, transitions, daily routines, and lunch. Today lunch consisted of meatballs, potatoes, green beans, mixed fruit, bread, and milk. You stated you provide transportation. I observed your transportation bus for the after schoolers. I observed and monitored the child safety restraints, the vehicle inspections, and insurance, first-aid kits, emergency information on children, staff information, and all information needed for compliance. Your last sanitation inspection was conducted on April 19, 2023. You received a superior classification with 9 demerits. Your last fire inspection was conducted on September 15, 2023. Your last fire drill was conducted on December 28, 2023. Your last emergency drill was a Shelter in Place Drill, and it was conducted on December 28, 2023. A playground inspection was conducted on December 29, 2023. The following violation(s) were observed and cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Three children that were in the hall outside their classroom unsupervised. Teacher did not know the students were playing in the hall. .1801(a)(1-5) 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. In two classrooms the staff/child ratio sheet did not have the correct age group. .0713(a)(10), (c) & (f)(3); .2818(e) 847 Parent's medication authorization did not include required information. One medication form was expired and others did not have the correct amount of cream to apply to child. 10A NCAC 09 .0803(4)(6-9) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Two emergency forms for children did not have a photograph of student on form. 10A NCAC 09 .1003(d) 1301 Center did not maintain a record of daily attendance. In the classroom for the 108 the attendance sheet noted there were eleven in the classroom and twelve children marked present on the attendance sheet. GS 110-91(9) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Playground area for the Pre-school and School age needs mulch added. .0605(k)(1-4) Technical Assistance: 1. Playground inspections are to be completed every month. In the playground inspections the mulch should be measured to make sure it is meeting the depth of the loose surfacing based on critical height of the equipment. Playground inspections are done for the safety of the children and to help maintain the safety of the playground areas. All things that are marked unsafe on this form need to be fixed as soon as possible. 2.Attendance should be done when a child enters a classroom. The teacher in the classroom should mark the child is present and they should also mark on the attendance sheet if the child is removed from the class and placed in another classroom. This helps the teacher keep up with the number of children that are always in attendance in the classroom. The teacher should make sure all children in the classroom are marked present. 3. Emergency Information on the bus and van is important. All emergency information for all children that ride the bus or van needs to be on the vehicle. The emergency information must include a photograph of the student with the emergency information. The administration staff and bus driver should look over and review all emergency forms and make sure they have the correct information and check every few months to make sure the information is current. 4.Children must be adequately supervised at all times. Children must be supervised by a teacher at all times. Children cannot be in any area unattended. Children were in the hall playing unattended. It was discussed with the administrative staff that the children were in the hall unattended. The administrator understands this is a violation and stated that the teacher is a new teacher to this classroom. It was suggested that the classroom doors stay closed when children are in the classroom so children will not wander in the hall area to play. It is suggested that teachers get training from the local Partnership for Children on supervision and classroom management. Consultation: 1.It was discussed with administrative staff that during my visit I noticed that the shelves with toys and materials have been turned to the walls, so the children do not have assess to the materials. It was explained to the administrative staff that this could be a violation in the future. Children must have access to all materials in the classroom. 489: For children under three years of age, hands-on experiences, including familiar and new activities, were not provided to enable the infant or toddler to learn about himself and the world both. 487: For children under three years of age, materials were not made available to children on a daily basis. Above are the violations that might be cited. 2. Popcorn kernels were in a teacher’s cabinet in the four-year-old classroom. This was discussed with the director, and they were removed. This can be a choking hazard for children. It was suggested that the popcorn not be kept in the classrooms if it is in the building. All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance.
