    Name of Operation: MISS CANDY'S PLAYLAND Facility ID: 13000478 Consultant: DEBORAH HOWELL Operation Type: Family CC Home Case Number: Visit Date: 4/17/2024 Number Present: 4 Completed Date: 4/17/2024 Age: From 2 To 4 Total Minutes: 120 Time In: 09:30 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance with applicable child care requirements during an annual compliance visit. Your last ACV was conducted on May 16, 2023. Your facility’s compliance history score prior to today’s visit was 96%. You, Candy Davis, Owner/Operator, assisted me with today’s visit. I observed your Four Star License, NC Child Care Summary of the Law, emergency phone numbers, emergency procedures plan, activity plan, daily schedule, up to date attendance sheet and First Aid information sheet posted in your child care room. Upon arrival, I observed two children playing, two more children arrived during the visit. I observed your childcare area set up on the ground floor of your home. There are age-appropriate materials accessible to the children. The bathroom is adjacent to your kitchen and designated for child care use was clean and stocked with necessary supplies. Children were adequately supervised, staff-child ratios were maintained. I observed adequate approved space use and permit restrictions including “daytime care”, “maximum for 5 preschool children at any time”, “no more than two infants under age one”, ‘children in care on ground level only”, and “fireplace/woodstove not used during operating hours” maintained during today’s visit. You stated that you do not allow screen time for the children. Inspections/Drills/Logs Playground Inspection- April 1, 2024 Fire Drill- April 1, 2024 Emergency Drill- March 1, 2024 Incident Log-Maintained as required. You reported you have no pets in the home. The Verification of Required Information for Operator and Additional Caregivers form was completed during today’s visit, and a copy was given to you. You completed a FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form. I monitored all ongoing required training hours, criminal record checks, IT’S SIDS, CPR/FA, and found all required records to be complete and valid as required. I monitored children’s files today. Two children’s files had an incomplete application, they did not contain all the required information. Your signature on all forms served and will serve as verification that the information provided was accurate and complete. The following violations were observed and cited during today’s visit: Violation Number Comment Rule 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation. One child's application did not include the choice of physician and another child's application did not include the hospital preference. 10A NCAC 09 .1721(a)(3)(G)(H) Technical Assistance was provided for the following: 1. Ensure applications are updated each year, review prior to accepting applications to monitor for completion of all areas of the application. Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them to clarify or verify compliance. For your convenience, your letter may be sent by email to: Deborah.k.howell@dhhs.nc.gov I must receive your compliance letter no later than May 1, 2024. Please Note: If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. If you cannot meet the requirements by this date, you shall contact me with a timeline of the corrections. In some cases, this timeline may be extended. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Compliance History: You are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score.
