You, Tinora Jones, Owner/Operator, assisted me with today’s visit. Your last annual compliance visit was on June 12, 2025. The facility is owned/operated by Tinora Jones. As the owner/operator, you are required by Child Care Rule .0102 (19) to reside in the home where the license is issued and according to General Statute 110-91(8) to be the full-time caregiver to the children enrolled at the family child care home. Your program’s compliance history was 96% prior to today’s visit. Your program currently operates with a one star license. I visited indoor spaces only today. I observed children engaged in free choice activities, staff/child interaction, meal time and rest time. I monitored hazardous product storage, medication storage, general safety, and discipline practices. I monitored program records and staff record files. I monitored health and safety requirements. I monitored supervision of children, capacity, approved space use, and permit restrictions. The following violations were cited during today’s visit. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Operator did not have a current criminal background check on file. Operator's criminal background check expired August 2025. G.S. 110-90.2 & .2703(n) & (o) 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). A child (DOB: 2/23/23) did not have a permission to administer form on file for an emergency medication on site. 10A NCAC 09 .1720(b)(4) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Operator did not complete Recognizing & Responding to Suspicions of Child Maltreatment training every five years. The operator completed the last Recognizing & Responding to Suspicions of Child Maltreatment training September 2020. .1703(d)(2) 2056 Documentation of the operator's Emergency Preparedness and Response plan was not completed and/or maintained on the template provided by the Division of Emergency Management. Today you showed me the electronic copy of your EPR plan which listed today’s date. You were unable to print off the documents for this plan. I informed you that going forward the EPR plan needs to be printed and available for review. This plan is used in case of an emergency and should be easily accessible if you need to leave your facility. Please print this plan as soon as possible. Consultation: Please ensure you always have the most recent copy of the Child Care Summary of Laws posted. Please ensure you continue posting a list of all children in your program with allergies. Children's special diets or food allergies shall be posted in the food preparation area and in the child's eating area. Please ensure when posting the safe sleep document all items on the document are completed, and make updates and changes when necessary. As we discussed during the last visit, please ensure your first aid kit contains gloves and band aids. During today’s visit I reviewed your first aid kit and it contained one band aid and one glove. Please continue ensuring you are washing your hands before and after handling food and feeding the children and continue ensuring all children's hands are washed before and after they are fed per child care rules. We reviewed QRIS pathways 1-3 today, and you stated at this time you are not going to choose a pathway but you will consider it. You also stated that you may remain a one star facility. I informed you about the QRIS Modernization page on the DCDEE website under the “What’s New” tab, and informed you about section .3200 in child care rules which contains all rules pertaining to QRIS. A three star licensed is required to be eligible to receive DSS subsidy. The violation(s) documented must be corrected immediately. On or before November 11, 2025, I must receive a compliance letter that describes accurately and in detail, how and when the violations were corrected. Your program’s compliance history was 100% as of June 11, 2025 and was reviewed with you today. Your program currently operates with a one star license. I visited indoor and outdoor spaces today. I observed children engaged in free choice and teacher directed activities, staff/child interaction, routine care taking and meal time. Food was prepared in the kitchen. I monitored hazardous product storage, medication storage, general safety, and discipline practices. I monitored program records, staff files and children's files. This facility does not provide transportation. I monitored health and safety requirements. I monitored supervision of children, capacity, approved space use, and permit restrictions. One violation was cited during today’s visit. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The pet (cane corso) in the home did not have a written record of up-to-date pet vaccinations. .1719(b)(1) Technical Assistance: Please ensure your pet receives their vaccine within two weeks from today’s visit. The last vaccine expired three weeks ago on May 20, 2025. Consultation: Please continue reviewing medical action plans upon receipt to ensure plans are filled out in their entirety, including but not limited to the doctor’s information and the specific brand of medication. Please add band aids to your first aid kit. Please complete the remaining on going training hours by June 20, 2025. NEW QRIS MODERNIZATION PLAN: The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Please be reminded any ERS assessment completed on February 1, 2025 and after will be assessed using the ERS-3 versions.
