Name of Operation: ARCHER'S PLAYHOUSE Facility ID: 13000604 Consultant: CAROLYN CONLEY Operation Type: Family CC Home Case Number: Visit Date: 2/15/2024 Number Present: 5 Completed Date: 2/15/2024 Age: From 0 To 9 Total Minutes: 130 Time In: 01:50 PM Time Out: 04:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of this visit is to monitor compliance with applicable child care requirements. Currently this center operates with a Four-Star license issued on August 23, 2023. The following restrictions are listed: firs and second shift; maximum of five preschool children at any time. You Alongee Archer, Owner, assisted me with the visit. Permit restrictions, supervision and capacity were observed in compliance during today’s visit. Your business, Archers Playhouse LLC, was checked in the NC Secretary of State website today and found to be current and active. If any changes to the business type need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. I observed the children during nap time, hand-washing routines, afternoon snack, departure procedures, arrival procedures, music and movement and free choice activities indoors. I observed positive interactions between you and the children. I used the "Annual Compliance Monitoring Checklist for Family Child Care Homes". I monitored the outdoor play area and outdoor inspections. I monitored the emergency drill report. You provide transportation when taking school aged children in the morning and periodic field trips. I was unable to monitor the vehicle today due to you caring for the children and unable to go outside with me. I was able to monitor the emergency information with photographs and permission to transport children. I encouraged you to keep a copy of the registration card and insurance information card with expiration dte in the “Transportation File”. The vehicle will be monitored during a future visit. I monitored the program files, your file and a portion of the children’s files. Two children have been enrolled since the last visit. The following forms were completed during today’s visit: -“VERFICIATION OF REQUIRED INFORMATION FOR OPERTOR AND ADDITIONAL CAREGIVERS” -“FCCH OPERATOR'S STATEMENTS OF RESPONSIBILITY” form. The following violation was observed during today’s visit. Violation Number Comment Rule 908 Health questionnaire was not completed annually. There was no documentation showing that the health questionnaire was completed annually. There was documentation from a previous visit that it was completed on 1/8/23. .1703(a)(1) TECHNICAL ASSITANCE: -You are required to complete the Health Questionnaire annually. It may be helpful to place a reminder on the calendar or your phone to remind you to complete this annually. The form can be found under “Provider Documents” on the DCDEE website. CONSULTATION: -We discussed that you are required to complete eight hours of in-service training annually. You will need to complete an additional 1.5 hours of training by February 22, 2024. You will be required to complete an additional eight hours of training by February 14, 2025 -We discussed that any time children are looking at any type of screen it will need to be documented on the screen time log. This includes during nap time if a child who has difficulty sleeping is looking at a tablet. -We discussed that you are required to monitor the EPR Plan annually. Add to your calendar as a reminder. Monitor it when you complete your Health Questionnaire or Professional Development Plan. Here is the link to access your EPR Plan: https://rmp.nc.gov/portal/portal.aspx#. You are also required to have a hard copy that is accessible at all times. -Your EPR plan needs to have the location of the Ready to Go File. A Ready to Go File means a collection of information on children, additional caregivers and the facility, to utilize, if an evacuation occurs. The file will need to include a copy of the Emergency Preparedness and Response Plan, contact information for individuals to pick-up children, each child's Application for Child Care, medication authorizations and instructions, any action plans for children with special health care needs, a list of any known food allergies of children and additional caregiver, additional caregiver contact information, Incident Report forms, an area map, and emergency telephone numbers. You have a copy of the “Ready to Go Kit Facility Checklist” developed by the NC Child Care and Safety Resource Center to use as a guide as well. -The summary of the law has been updated and can be found on the DCDEE website under “Provider Documents”. You will want to print it and post it and make sure to give it to any newly enrolled families. -The North Carolina child Care Resource and Referral Council (CCR&R) now has a challenging behavior helpline that staff can call and peak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in their classrooms: 1-888-600-1685 option 1. I also sent an email about this resource on April 4th. REQUIRED RESPONSE: Violations must be corrected immediately. You shall submit a written/typed, signed and dated statement to me at the address noted below or email me detailing how the violations were corrected with exact dates and details including how you plan to prevent reoccurrence of such violations which must be received by February 29, 2024. Failure to correct the violation(s) and send the written statement by the established deadline listed above may result in an unannounced follow-up visit or an administrative action may be recommended based on Child Care Rule 10A NCAC 09 .2200. The documentation was completed electronically and reviewed with you during the visit. I emailed a copy of today’s visit summary and the annual compliance checklist.
