Your last annual compliance visit was conducted on March 12, 2024. You, Bethany Church, Lead Teacher, assisted with today's visit. Rated License Status Rated License Status: Your program currently has seven (7) points in Program Standards, seven (7) points in Education and one (1) Quality Point (has reduced staff/child ratios by one) for a total of fifteen (15) points and a five (5) star rated license effective February 28, 2019. The permit restrictions were in compliance including Daytime care only; Meets enhanced ratios minus one; Meets enhanced ratios; Meets enhanced space. Bethany Church requested to have an ECERS-R assessment via email on October 18, 2024. She completed the application for the reassessment and the rated license assessment request review on, November 6, 2024. The request for ECERS-R was submitted on October 24, 2024. The ECERS-R assessment was conducted on November 26, 2024. We reviewed your ECERS-R assessment conducted on November 26, 2024. Consultation was provided to you regarding the items that would be considered violations if observed by a representative of the Division of Child Development and Early Education. You received a score of 6.06 on the ECERS-R assessment. Bethany Church accepted the score during today’s visit. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during independent center play, teacher lead group time activities, transitions, rest time, outdoor gross motor activities, and departure. Fire Inspection: The most recent fire inspection was completed on November 12, 2024, and recorded. Sanitation Inspection: The last sanitation inspection was completed on November 15, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Playground Inspection: The last playground inspection was completed on February 17, 2025, and recorded. Emergency Drills: The last fire drill was completed on February 11, 2025, at 10:20am and recorded. The last lockdown drill was completed on January 21, 2025, and recorded. Files Reviewed: One (1) staff file was reviewed today. Two (2) children’s files were reviewed today. NC Pre-K/Developmental Day Requirements were monitored and observed to be in compliance during today’s visit. A checklist was used to note the requirements I monitored during today’s visit. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. We observed children in the indoor and outdoor learning environments during a teacher directed activity, free choice play, handwashing/bathroom routines, lunch, gross motor outdoor play, and nap. Appropriate interactions, supervision, and staff/child ratios were monitored and in compliance. Two Staff files were reviewed today using the DPI verification form. One violation was noted. Any violation(s) documented may impact the compliance history score. The center’s compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 93 %. The violation documented must be corrected immediately. On or before March 26, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information.
