This center currently operates with a Five-Star license issued 4/24/18. As of 4/7/26, the facilities compliance score was one hundred (100) percent. A sanitation inspection was conducted on 12/9/25. This facility received a Superior classification and 4 demerits. This facility’s corporation status was verified current and active on the NC Department of the Secretary of State on 4/7/26. According to the Clean Classroom site, lead in water was up to date as of 10/25/23 and the facility is exempt for lead-based paint and asbestos testing. There are staff linked to this facility through the ABCMS system. The last fire inspection was completed on 3/27/25. I was greeted by Pam Conwell, Assistant Director. Ms. Conwell accompanied me on a walk-through of the center and the playgrounds. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. Children were participating in group activities, playing outside, and playing in centers. The staff/child interactions I observed were warm and caring. Outside I observed, there is plenty of room to run, jump and other vigorous play. They have a variety of materials and equipment available to children. Devin Walton, Administrator, provided me with the paperwork needed for the visit. Monthly fire drills and playground inspections were reviewed as well as quarterly disaster drills. The last fire drill was 3/2/26. The last disaster drill (shelter in place) was 2/10/26. The last playground inspection was January 2026. I reviewed staff files for two new employees. There were two violations cited during today's visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed 3/27/25. 10A NCAC 09 .0304(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection was completed January 2026. .0605(q) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) to achieve and maintain an eighteen-month compliance history score of at least seventy-five (75) percent. Any violations documented may impact the compliance history score. To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected. It must be submitted to me by 4/22/26 and include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, and information regarding how the violation was corrected. I provided technical assistance regarding: New Staff Training: For the two new staff members, they need to complete orientation in the first 6 weeks of employment, take the Recognizing and Responding to Child Maltreatment course within the first ninety days of employment and complete the health and safety trainings within the first year of employment. The Health and Safety trainings can be accessed through moodle on the NCDCDEE website. The facility plans to apply for QRIS using Pathway #1 by August 1, 2026. They have had mock rating scales conducted and are in the self-study phase. Elizabeth Childress, Child Care Consultant, accompanied me on this visit. A checklist was used to note requirements we monitored today. This facility has a 100% compliance score as of 10/6/25. The last annual compliance visit was on 10/17/24. This child care center currently operates with a Five-Star License, effective 4/24/18. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. A sanitation inspection was conducted on 5/7/25, with a superior classification and 2 demerits. A fire inspection was conducted on 3/27/25. Upon my arrival, we were greeted by, D Walton, Administrator for the program. Ms. Walton assisted us with the information required during the visit. We completed a walkthrough of the licensed spaces and noted attendance. Emergency drills and outdoor inspections were current and completed by trained staff. Required items were posted and accessible. A full assessment of applicable child care requirements was conducted during today’s visit. We completed a walk-through of the indoor and outdoor areas that the children use. Forty-nine children were present. I observed the children napping, playing in centers, playing outside and participating in group time. Teacher interactions were positive and nurturing. Medications were checked in each room and paperwork was correctly completed for each medication. We reviewed 5 children files. There were 2 violations of Child Care requirements observed on this visit Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical report on file prior to employment. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member did not have results of a TB test prior to employment. .0701(a) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. It is important to have all staff qualifications documented in NC Works as this is what will be used to determine education standards on the Star Rated License. During today's visit, staff in the infant classroom had their It's-SIDS training. We reviewed the rule regarding It's SIDS. As a reminder, any staff who work in the infant room need Its-SIDS within 60 days, however, someone with It's-SIDS always needs to be present in the infant room. Everyone in the classroom needs to be aware of the Safe Sleep Policy and Sleep charts. We discussed QRIS Pathways. You are currently leaning towards Pathway #1.
