A checklist was used to note the requirements I monitored today. This facility currently operates with a Five-Star License issued. This facility has a 100% compliance score as of 1/13/26. The last annual compliance visit was on 1/30/25. The last sanitation inspection occurred on 1/6/26 and was a superior classification with 13 demerits. Criminal background checks were checked in the ABCMS system. Upon my arrival, I was greeted by the office staff who took me to the NC-PreK classroom. I informed Tonja Walters, Lead Teacher, of the purpose of today’s visit. Ms. Walters gave me the necessary paperwork I needed to complete today’s visit. The last fire drill was conducted 12/10/25. The last emergency drill (shelter in place) was conducted on 9/11/25. The last playground inspection was completed on 9/8/25. Sixteen (16) children were present during today’s visit. This is a DPI program operated by Guilford County Schools. During today’s visit, I observed children participating in gross motor activities and a book reading during group time, drawing buildings with crayons and eating lunch. All medication and hazardous items were out of the children's reach. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were reviewed during today’s visit. Your signature on these forms will serve as verification of the information provided is accurate and complete. There were three violations cited during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There were three electrical outlets in the classroom that were not covered. 10A NCAC 09 .0604(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection was completed 9/8/25. .0605(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter in place drill was conducted 9/11/25. .0604(u);.0302(d)(8) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by 1/28/26. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected and information regarding how the violation was corrected. I provided technical assistance regarding: Shelter-in-place and lockdown drills need to be completed once a quarter. If the Elementary School conducts this it can be counted. If the school does not conduct a lockdown/shelter-in-place drill within three months, the NC-PreK class can conduct their own. Playground inspections need to be conducted monthly. You can do these the same day as the monthly fire drill to help you remember. QRIS Modernization Pathways: The Pathway Discussion Template was discussed and completed. Since the facility has received a new rated license permit 10/23/25, they will not need to go through the pathway assessment until 2028.
