Your program currently operates with a five-star license, issued 7/31/19, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced requirements and reduced ratios) and 1 quality point for the educational option for all lead teachers have an AAS or higher in ECE/CD. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of 4/15/25. Corporation Status: Per your statements and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. No children were present during the visit. The classroom space is organized in learning centers including library, dramatic play, blocks, and art. Files for new staff were reviewed. Fire and sanitation inspections remain current. The following violations were observed. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. Two screws were protruding from the top of the portable climbing structure, and one bolt was protruding from the wall of the bridge on the structure. .0601(c) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Staff employed on 2/24/25 had a medical report on file dated 3/20/25. 10A NCAC 09 .0701(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 30, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The protruding screws and bolt were not noted on March 24, 2025. You stated your staff had not notified you of the issue. You stated you will speak to the staff about how to complete the playground inspections and being mindful of all areas. You were able to call maintenance personnel, who removed the screws and secure the bolt during the visit. Be sure to complete daily inspections of the outdoor area, prior to being used by children, address all hazards immediately to ensure the safety of children. Staff Medical Reports- Knowing if a staff member is physically and mentally fit to care for children is a vital part of ensuring the health and safety of children. Each staff member must have a medical report on file on or before the first day of employment. Staff employed on 2/24/25 had a medical report on file dated 3/20/25. You shared with me that you thought it was due within thirty days of employment. Staff may use a medical assessment that was completed less than 12 months prior to the date of employment. This facility operates with a Five (5) Star License issued May 23, 2022, earning seven (7) points in the Education Component, six (6) points in the Program Component (meeting enhanced space and ratios) and one (1) Quality point. The last sanitation inspection was completed on April 11,2024, earning a “Superior” classification. The last fire inspection was completed on June 14, 2024, and was approved for day and night care. The facility had a fire inspection completed on April 23, 2024 that yielded an “Unsatisfactory” result and had to have another inspection completed. However, the inspection reports were not sent to the consultant until after the last inspection had expired. The last annual compliance visit was conducted on August 23, 2023. Ms. T. Melendez, Owner/Operator and D. Cherry, Lead teacher were present and available to assist. Two (2) children were present age seven (7) and ten (10) years. The children were engaged in free play activities, teacher directed activities, and eating lunch. Lunch consisted of ham sandwiches, carrots, pears , and milk. All staff interacted with children in a positive and nurturing manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records were monitored. All staff records and children’s records were found to be in compliance. One violation was documented for program record keeping/annual inspections. Any violations observed today were discussed with you and documented on the visit summary reviewed with you at the end of the visit. The following violations were documented on today’s visit. This facility operates with a Five (5) Star License issued May 23, 2022, earning seven (7) points in the Education Component, six (6) points in the Program Component (meeting enhanced space and ratios) and one (1) Quality point. The last sanitation inspection was completed on April 18, 2023, earning a “Superior” classification. The last fire inspection was completed on April 24, 2023, and was approved for day and night care. The last annual compliance visit was conducted on August 25, 2023. D. Cherry, Lead teacher was present and available to assist. Two (2) school age children were present. The children were observed in toileting and handwashing routines, eating snacks, and free choice activities. The snack consisted of pretzels and 100% fruit juice. All interactions between the staff and children were positive. This program serves school age children only. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. Staff and Training worksheet was completed for two (2) new staff members. Violations cited for incomplete staff records for both employees. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. Any violations observed were reviewed and a computer-generated visit summary was left with you at the conclusion of the visit. The following violations of child care requirements were documented. G.S. 110-91( 9) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Grass was overgrown and not kept in a manner conducive to children being able to play safely in the outdoor environment. 15A NCAC 18A .2832(a) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fencing in the outdoor play area was not a minimum of 4 feet in height. OUTDOOR PLAY AREA During my observation of the outdoor play area, grass was overgrown and was not kept in a manner conducive to children being able to play safely in the environment. We discussed the outdoor premises should be kept in a manner which does not encourage vermin. The grass was being cut during the visit. We discussed digging out the bottom of the fence, pulling the fencing up, or adding tubing to the top of the fence to adjust the height to the required minimum height of 4 feet. RESOURCES HEALTH AND SAFETY TRAININGS DCDEE has partnered with the NC Child Care Health and Safety Resource Center to offer health and safety trainings in MOODLE. These trainings will remain at no charge for staff. The contract with ProSolutions ended on September 30, 2020. Health and safety trainings are due every five (5) years, including the Recognizing and Responding to Suspicions of Child Maltreatment. The Recognizing and Responding to Suspicions of Child Maltreatment training must be completed via the Prevent Child Abuse NC website. The enrollment process and completion of course materials tend to work best with a windows laptop or desktop using Firefox, but other devices and Google Chrome should also work. You can access the health and safety trainings available in Moodle and Frequently Asked Questions by logging in using NCID and completing the following steps: • From the Moodle home page, students need to log-in with their NCID and password. The log-in is in the upper right corner of the screen. If you do not have a current log-in you will need to create one. • After log-in, the student should scroll to the bottom of the page. In the ‘Search courses’ box, the student should type in the short name “Year 2023 CCDF” and then click go. The student should not register for prior year courses as they are no longer available. • After completing the course search for “Year 2023 CCDF”, the student should be able to register for the course. • Once registered, the student will then need to use the “ADDITIONAL DIRECTIONS” for step-by-step instructions. The Infant Toddler Educator AWARD$ program has recently gone through some exciting programmatic updates. The original AWARD$ program was only available to teachers working full-time in B-3 classrooms that had an AA in ECE or higher degree and were employed by 4- and 5-star programs. The education requirement has been reduced as an incentive to support teachers seeking to increase their education in any star rated licensed program. To be eligible for AWARD$ Plus, applicants must: • Work at least 30 hours/week with infants, ones, or twos. • Earn at or below $20/hour. • Work in a licensed child care center or home. • Have at least 12 semester hours of birth to five focused coursework. INFANT-TODDLER EDUCATOR AWARD$ PLUS PROGRAM Infant-Toddler Educator AWARD$® Plus is a salary supplement program, available in all NC counties, designed to increase the education, retention and compensation of teachers and family childcare educators working with our youngest children. The Infant Toddler Educator AWARD$ program has recently gone through some exciting programmatic updates. The original AWARD$ program was only available to teachers working full-time in B-3 classrooms that had an AA in ECE or higher degree and were employed by 4- and 5-star programs. The education requirement has been reduced as an incentive to support teachers seeking to increase their education in any star rated licensed program. To be eligible for AWARD$ Plus, applicants must: • Work at least 30 hours/week with infants, ones, or twos. • Earn at or below $20/hour. • Work in a licensed child care center or home. • Have at least 12 semester hours of birth to five focused coursework.
