L. Square, Principal and C. Rivers, Lead Teacher, assisted me with the visit. Your program currently operates with a five-star license, earning 7 points in the education component, 7 points in the program standards component (meeting reduced ratios) and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You stated the approved curriculum used is the Creative Curriculum, Teaching Strategies Gold. Creative Curriculum books, activity plans and other resources were observed today. The last annual compliance visit was conducted on December 4, 2024. The last sanitation inspection was completed May 8, 2025, with a “Superior” classification. The last fire inspection was conducted March 31, 2025, and your facility was approved for daytime care only. A walk-through of the facility was conducted. One group of children were observed. Children participated in free-choice play, outdoor play, eating breakfast, completing morning routines, including hand washing and toileting and interacting with teachers. Breakfast consisted of doughnuts, peaches, orange or apple juice, and milk. Bertie County Schools is listed as the current owner of this program. Prior to making in changes in the program’s ownership status, contact me to discuss change of ownership procedures. The following violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Four bolts were protruding from the black border enclosing equipment on the playground used by children. An open can of Ginger Ale soda with an open was on a teacher’s desk and accessible to children. 10A NCAC 09 .0601(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Written authorization was not on file for an EpiPen kept onsite for one preschool child enrolled. 10A NCAC 09 .0803(1)(a & b) The violations documented must be corrected immediately. MEDICATION REQUIREMENTS: An EpiPen is kept onsite for one preschool child enrolled. Based on my review of the child’s record and conversation with the teacher, an authorization to administer medication form was not on file for review. An action plan completed by the primary health care provider was on file which indicated “complete a medication administration form if the medication listed below is to be provided by the child care”. I reminded you that permission to administer medication forms must be completed and kept on file for any medication that will be given to children while in care. It was recommended that you provide the permission form to the parent or guardian for completion and placement in the child’s file. Additionally, I recommend reviewing documentation related to medical authorization to ensure all required information is included and compliance is maintained. QRIS CONVERSTATION: During the visit, I spoke with B/ Holley, NC Pre-K coordinator regarding the QRIS plan for this program. Due to travel time considerations for Ms. Holley, we agreed to schedule a technical assistance visit to complete the QRIS conversation. I asked Ms. Holley to review her schedule and provide available dates for a technical assistance/training visit. Ms. Holley stated she would submit the date today. ANNUAL CHILDCARE IMMUNIZATION REPORT DEADLINE: The 2025-2026 Annual Child Care Immunization Reporting Portal is now open, and reports must be submitted electronically by midnight, November 15, 2025. Instructions and worksheets are available for download and may be used to collect data needed before entering your individual school information online. A. Watford, Assistant Teacher, assisted me with today’s visit. The visit was reviewed with Bobbi Holley, Director of NC Pre-k. Your program currently operates with a five-star license, issued 6/16/19 earning 7 points in the education component, 7 points in the program standards component (meeting enhanced ratios minus one and enhanced space) and 1 quality point for: enhanced policies approved and staff benefits package. Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. North Carolina Rated License Assessment Project - NCRLAP’s mission is to promote the quality of child care by consistently and reliably assessing environments for the North Carolina Star Rated License. They collaborate with professionals in the early childhood education field to foster the development and learning of young children. Their website, ncrlap.org, offers resources, trainings, videos, worksheets, and help to get ready for the new ITERS-3, ECERS-3, and FCCRS-3 assessment tools that will be used in the new QRIS system. The center's compliance history was reviewed with the operator. The program’s compliance history was 96 % as of 3/17/25. A walk-through of the classroom was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in group time, free play in activity areas, transitions and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. No new staff have been hires since the previous visit. The providers moved around the room during free play and spoke with the children about their play. Later the children went outside and played in the fenced playground space. Hands were washed as required. Lunch was observed and consisted of a cheese tortilla, broccoli, 100% fruit juice Jello, and milk. The following violation(s) were documented: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was completed on 4/9/24 and marked as unsatisfactory for item #21 - the smoke detector/fire alarm system maintained with annual test reports provided. A second inspection was completed on 12/3/24 and was also marked as unsatisfactory for the same item. Documentation of a satisfactory inspection was not on file. 10A NCAC 09 .0304(a) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The stationary dome shaped climbing structure was located over grass. .0605(j) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A bottle of hand sanitizer was stored on top of the cubbies and a bottle of Cherry Blossom hand cream was located on the teacher’s desk. Both items were stored less than 5 feet above the floor. .2820(b) 1041 Prior to employment a Criminal Background Check was not completed. The substitute working in the classroom did not have documentation of a criminal background check on file. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. G.S. 110-90.2(b) & (d) & .2703(e) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The plan available for review was dated 2017. .0607(e) Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. The most recent versions of child care laws and rules and environmental health rules in North Carolina, the Items Number Listing (can be used as a detailed checklist of required items) and What’s New information are available on the DCDEE website, www.ncchildcare.ncdhhs.gov. Check the designated facility email at least weekly to stay current with communication and email blasts from DCDEE. If your email is not the facility email, sign up for email blasts on the What’s New page for general information and communication from DCDEE. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 4/1/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Fire Inspection – Child care facilities must receive a satisfactory fire inspection at least annually. If the inspection marks any item as not in compliance, the item must be corrected immediately and a new inspection completed to document compliance. A fire inspection was completed on 4/9/24 and marked as unsatisfactory for item #21 - the smoke detector/fire alarm system maintained with annual test reports provided. A second inspection was completed on 12/3/24 and was also marked as unsatisfactory for the same item. To ensure the safety of the children in care, this must be corrected immediately. Per the maintenance personnel, the item has been corrected and he will contact the fire inspector and request another inspection to verify compliance. Criminal Background Checks (CBC) and Qualifying Letters – Criminal background checks help to ensure children are safe with the adults who provide care for them. All staff who work with preschool children must complete a criminal background check through the Division. This includes substitutes, therapist who work with children outside of the classroom, and bus drivers who transport the children. Christine Bland was working as a substitute in the NC Pre-K classroom and did not have verification of a criminal background check on file and available for review. Unlike many criminal background checks completed by other organizations, ours also checks the Child Maltreatment Registry and National Sex Offender Registry. The substitute can complete working today but may not be used in the classroom again until documentation of a current qualifying letter through the Division of Child Development and Early Education is on file. Emergency Preparedness and Response (EPR) Requirements – Your Emergency Preparedness and Response (EPR) Plan needs to be reviewed and updated annually. The plan available for review was dated 2017. Documentation that the update had been completed in the Risk Management Portal was not on file. You stated that the plan had been reviewed in the Risk Management Portal but you had not documented the review. When you complete your review, document the date on the last page, click on publish to save any changes, and print the current copy. The current plan must be placed in your Ready to Go File along with your children’s applications, staff emergency information, blank incident log forms, and a map of the area. Please see the Ready to Go check list to ensure all information is included. A copy of the updated plan must be submitted with your compliance verification plan. Playground Safety - Regular outdoor inspections are critical to prevent deterioration of equipment and accumulation of hazardous materials in the play. It also ensures that appropriate repairs are made as soon as possible. The stationary dome shaped climbing structure was located over grass. All stationery climbing structures over 18 inches high must be installed over sufficient surfacing to protect children if they fall. The structure also had large patches of rust and peeling paint. The structure also did not have a 6 foot fall zone. On one side there was 34 inches between the base of the structure and the plastic barrier around the perimeter. This equipment must be repaired or replaced and sufficient surfacing added. Sufficient surfacing is 6 inches in depth. The equipment must also have a 6 foot fall zone on all sides. You stated that the playground had been moved and at one time had surfacing. Storage of Hazardous Materials - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. Any product under pressure and any product that includes more than one warning must be in locked storage. All other items must be stored at least 5 feet above the floor. A bottle of hand sanitizer was stored on top of the cubbies and a bottle of Cherry Blossom hand cream was located on the teacher’s desk. These products are not required to be in locked storage but must be kept a minimum distance of 5 feet from the finished floor. The provider corrected this area of non-compliance by placing the products in the locked cabinets over the sink. I emailed a copy of the Hazardous Items Storage for Childcare document today. It can be found on the Division's Website underneath the Provider tab/Provider Documents and Forms for Family Child Care Homes.
