This center currently operates with a four (4) star license issued on 6/25/2019. This program continues to meet enhanced ratio and enhanced space requirements. The last annual compliance visit was conducted on February 18, 2025. You, B. Shaver Johnson, was available and assisted with the visit today. Four (4) children were observed in care today ranging from ages zero (0) to two (2) years old. Children were engaged in free choice play, interacting with the caregivers, toileting and handwashing routines, and eating lunch. Lunch consisted of hot dogs, beans, fries, whole wheat bread, and milk. All interactions were nurturing and positive. The last sanitation inspection was completed on 4/7/2025 with a “Superior” classification. The last fire inspection was completed on 5/29/2025 and the center was approved for daytime care. I used the Annual Compliance Monitoring Checklist and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent (10%) of staff records and ten percent (10%) of children’s records and all program records were reviewed today. All staff records, children’s records and program records were found to be in compliance. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. No violations of child car requirements were documented today. Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Updated staff emergency information was not on file for review. .0701(a) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You, B. Shaver Johnson, was available and assisted with the visit today. This center currently operates with a four (4) star license issued on 6/25/2019. The last sanitation inspection was completed on 3/1/2024 with a “Superior” classification. The last fire inspection was completed on 5/31/2024 and the center was approved for daytime care. The last annual compliance visit was conducted on 3/7/2024. This program continues to meet enhanced ratio and enhanced space requirements. Six (6) children were observed in care today ranging from ages zero (0) to four (4) years old. Children were engaged in free choice play and interacting with the caregivers. All interactions were nurturing and positive. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. Due to technical difficulties, a handwritten visit summary was reviewed and left with you at the conclusion of the visit. A computer-generated visit summary was emailed to you. The following violations were documented today Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation.
