Upon my arrival, I was greeted by staff member, T. Bridges. The administrator, D. Burrus, was present on site. I was unaccompanied as I completed a general walk-through of the facility which consisted of four (4) classrooms, the outdoor play areas, and the kitchen. A total of forty-four (44) children were present during today’s visit. I observed the children participating in free choice play, completing routine care tasks, and eating lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued on March 2, 2020. The NC Secretary of State website was reviewed on March 2, 2026, and Leap of Faith Academy LLC, was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was conducted on July 24, 2025. The last annual fire inspection was completed on November 17, 2025, and the last annual sanitation inspection was completed on July 30, 2025, and received a “Superior” classification with nine (9) demerits. The last documented monthly fire drill was conducted on February 3, 2026, the last documented quarterly lockdown drill and/or shelter-in-place drill was conducted on December 17, 2025, and the last documented monthly playground inspection was completed on March 2, 2026. During today’s visit, a partial assessment of the child care requirements was conducted. I observed all required postings, attendance logs, and safe sleep logs. I monitored the program records, equipment and furnishings, staff/child ratios & supervision, storage/administering of medication, and the outdoor area and equipment. I also reviewed a staff file for one (1) new staff member. This facility does not provide transportation. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. Two (2) diaper creams, Aquaphor with an expiration date of 12/2025 and Desitin with an expiration date of 02/2026 were administered to two (2) enrolled children after the expiration date. 10A NCAC 09 .0803(1)(d) COMPLIANCE HISTORY: Priors to today’s visit, the 18-month compliance history for this facility was 94%. A compliance verification letter is not required because the violation cited was corrected during today’s visit. TECHNICAL ASSISTANCE/CONSULTATION: As a reminder, you may want to consider developing a system to track medication expiration dates, as well as the dates of written authorizations for topical ointments and all prescribed and non-prescribed medications. Implementing such a system will help ensure that expired medications are not administered to any enrolled children. During today’s visit, I observed the additional bathroom that was installed in November 2025. Ms. Burrus stated that she is still in the process of obtaining an updated building inspection to reflect the change in capacity following the installation of the fourth bathroom. I reminded Ms. Burrus to submit to me a copy of the updated building inspection once she receives a copy from the building inspector. Thank you for your time today. The annual compliance monitoring checklist for child care centers was used to note requirements monitored today. Upon my arrival, I was greeted by three (3) staff members: D. Coxum, S. Brooks, and T. Lee-Bridges. The assistant administrator, Mr. Burrus, stated that the administrator, D. Burrus, was not present onsite. I was unaccompanied as I conducted a general walk- through of the facility which consisted of four (4) classrooms, the outdoor play areas, and the kitchen. A total of thirty-one (31) children were present during today’s visit. The children were observed during outdoor play, free choice, completing routine care tasks and eating lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a three-star rated license issued on March 2, 2020. The last annual fire inspection was completed on December 16, 2024, and the last annual sanitation inspection was completed on December 9, 2024, and received a “Superior” classification with six (6) demerits. The last documented monthly fire drill was conducted on June 2, 2025, the last documented quarterly lockdown drill and/or shelter- in- place drill was conducted on June 3, 2025, and the last monthly playground inspection was completed on July 3, 2025. During today’s visit, a full assessment of the child care requirements was conducted. I observed all required postings, attendance logs, and safe sleep logs. I monitored program requirements, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, and the outdoor area & equipment. This facility does not provide transportation. I also reviewed the staff files, and a sample of the children’s records. The following violations were observed during today’s visit: Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. Five (5) children between the ages of 12 and 24 months were grouped with children aged three years and older on the playground. 10A NCAC 09 .0713(a)(6) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member with a hire date of 1/15/2025 did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 99%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By August 7, 2025, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at tanya.herring@dhhs.nc.gov. If you elect to mail your letter, please ensure that you allow sufficient time so that the letter reaches me on time. My mailing address is: Tanya Herring 2201 Mail Service Center Raleigh, NC 27699-2200 Although the staff/child ratios violation was corrected during the visit, due to the nature of the violation cited during today’s visit, a follow-up visit will occur in the near future to ensure ongoing compliance has been maintained. TECHNICAL ASSISTANCE/CONSULTATION: Upon my arrival at the facility, I observed Mr. Burrus outside throwing away trash. He returned inside the building through the front entrance. I then observed three (3) caregivers exiting the side entrance with a group of children entering the playground. I made my way to the outdoor play space to begin monitoring and determined there was a total of thirteen (13) children present. One group of children consisted of five (5) one-year-olds and one (1) two-year-old, for a total of six (6). The other group of children consisted of four (4) two-year-olds, two (2) three-year-olds, and one (1) four-year-old, for a total of seven (7). Staff/child ratios were maintained for the number of children present, however, all thirteen (13) were grouped together in one space. This is a violation of Child Care Rule. .0713(a)(6). Outdoor space 2 was then monitored for compliance and no violations were observed. Once inside the building, I began monitoring the indoor spaces and was told by Mr. Burrus that Ms. Burrus was not onsite today. As a reminder, children can only be grouped together during the first and last operating hour of the day. During that time, you are still required to maintain staff/child ratios of the youngest child. Additionally, grouping younger children with older children may potentially lead to injuries or harm, as older children may engage in rougher play. Child care administrators and staff members must complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment unless the staff member completed the training within the year prior to their hire date. I encourage providers to implement a reliable tracking method such as a spreadsheet, digital calendar, or any other practical system to ensure that required trainings are completed within the 30, 60, and 90-day timeframes, as required by the rules. The Child Care Commission has recently approved the new rules for the Quality Rating and Improvement System (QRIS) Modernization, previously the star rated license assessment. As a reminder, all programs will be required to transition using this new system. A checklist was used to note the requirements I monitored today. The administrator assisted with today’s visit. Several therapists were present during parts of today’s visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plans, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers, and a summary of NC Child Care Law were all were all monitored during the visit. The storage of medication and hazardous products was also monitored for compliance. Staff and Children records were also reviewed today. Files for new staff were monitored along with a random selection of returning staff members. The information on the staff/training worksheets was verified for these files. Your signature on these forms will serve as verification the information provided is accurate and complete. A random selection of children’s records was reviewed, and the children record worksheet was shared with you and discussed today. I observed the children playing outdoors, engaged in free play, completing routine care tasks, and eating lunch. The meal served today consisted of ravioli, peas, pineapple, bread, and milk. Your last sanitation inspection was completed on February 8, 2023 with 6 demerits and a superior classification. The last fire drill was documented as conducted on August 7, 2023 at 9:00am. The log reflected that it took 1 minute and 23 seconds to evacuate 29 children and 5 adults. The most recent lockdown drill was conducted on June 14, 2023 at 10:00am. The last outdoor inspection was documented as completed on August 18, 2023. Your last fire inspection was completed on November 10, 2022. I verified the corporation status with the Secretary of State’s website as part of the visit. Any violations observed during today's visit were discussed with you even though the handwritten visit summary was used. The following violations were documented during today's visit. Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two staff members had not completed Recognizing and Responding to Suspicion of Child Maltreatment within the last five years. .1103(b) Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. The health and safety trainings must be completed every five years. The health and safety trainings have now been relocated from Pro-Solutions to Moodle on the DCDEE website and remain free of charge. I encouraged you to use the Health and Safety tracking log to ensure all topics are covered. I noted that both Medication Administration and Recognizing and Responding to Suspicions of Child Maltreatment are part of this series. The hold harmless state of Rated License Assessments has been extended, however you may request to have your rated license reassessed at any time. Therefore, it is imperative that you and your staff ensure all education information is updated in the WORKS database. I asked that you notify me if you are interested in completing the reassessment. We also discussed that the Governor lifted the State of Emergency related to COVID on August 15, 2022. With the lifting of the State of Emergency COVID signage is no longer required. You may take down the Health and Safety certificate issued at the beginning of the pandemic, as well as any other door signage related to COVID. Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice and payment is due in FULL within 30 days. Please closely monitor your mail and be on the lookout for your invoice. If you have not received your invoice by November 15th you should contact the Division at 1-800-850-0829. Please don’t delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Please be reminded that, failure to pay a license fee could also result in the facility's child care license being revoked. All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than August 30, 2023. The compliance letter may be sent to me via email, if you do not have access to email, please ensure that your compliance letter is mailed in a timely manner and that you allow for delays with the mail service. If your compliance letter is mailed, a handwritten signature must be included. Contact me if you have questions regarding correction of the violations documented during today’s visit. If you are unable to correct all violations within the specified timeframe please submit a request for an extension as soon as you determine you will not meet the imposed timeframe. This request should include a specific plan to correct the remaining violations and any time frames needed. This request will need to be approved by DCDEE management. A follow up visit may be conducted in the near future to verify compliance with violations documented during the visit.
