Facility ID: 82000174 Consultant: TANYA HERRING Operation Type: Center Case Number: Visit Date: 1/16/2025 Number Present: 52 Completed Date: 1/16/2025 Age: From 1 To 5 Total Minutes: 188 Time In: 09:52 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable child care requirements during your routine unannounced visit. Upon my arrival, I was greeted by the owner/operator, J. Parker. I was unaccompanied as I conducted a general walk- through of the facility which consisted of the six (6) classrooms, the outdoor play areas, and the kitchen. A total of fifty-two (52) children were present during today’s visit. The children were observed participating in free choice play, routine care and having lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility currently operates with a two-star rated license issued on April 11, 2018. The NC Secretary of State website was reviewed on January 16, 2025, and Toddlers Preschool, Inc., was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was conducted on July 25, 2024. The last annual fire inspection was completed on April 16, 2024, and the last annual sanitation inspection was completed on August 6, 2024. The last documented monthly fire drill was conducted on December 23, 2024, the last documented quarterly lockdown/shelter in place drills was conducted on October 20, 2024, and the last monthly playground inspection was completed on January 9, 2025. During today’s visit, a partial assessment of the child care requirements was conducted. I observed all required postings and attendance logs. There are no infants currently enrolled, so safe sleep logs were not monitored. I monitored the program records, equipment and furnishings, staff/child ratios & supervision, storage/administering of medication, and the outdoor area and equipment. This facility does not provide transportation. I also reviewed a staff file for one (1) new staff member. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A blanket permission form to administer albuterol to one (1) child had expired on 11-12-24. .0803(12) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member with a hire date of 11-25-24 did not have a TB test or screening on file that was less than 12 months. .0701(a) 9999 A violation was found for which there is no item number. A bag of crackers and cereal was not stored in an approved, clean, tightly covered, storage container once the original package had been opened. This is a violation of 15A NCAC 18A .2806(a). COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 97%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By January 30, 2025, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at tanya.herring@dhhs.nc.gov. If you elect to mail your letter, please ensure that you allow sufficient time so that the letter reaches me on time. This will prevent you from having expired medications, and authorization forms which are especially important when it comes to administering an emergency medication needed for a chronic condition such as allergies, asthma, and seizures. Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization has expired must be returned to the child’s parents or discarded within 72 hours. CRIMINAL BACKGROUND CHECK PORTAL: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete the Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the referenced training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been granted, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
