Name of Operation: MARGIE HILL'S DAY CARE HOME Facility ID: 82000156 Consultant: TANYA HERRING Operation Type: Family CC Home Case Number: Visit Date: 11/13/2024 Number Present: 5 Completed Date: 11/13/2024 Age: From 0 To 2 Total Minutes: 190 Time In: 12:35 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor applicable child care requirements during your annual compliance visit to your family child care home (FCCH). The annual compliance monitoring checklist for FCCH was used to note requirements monitored today. Upon arrival, I was greeted by the owner/operator, M. Hill. I was unaccompanied as I completed a general walk-through of the indoor and outdoor environments. A total of five (5) children were present during today’s visit. I observed four (4) children resting during naptime, and the caregiver engaging in positive and nurturing interactions with an infant while she was eating. Today’s snack was in compliance with the Meal Pattern requirements. LICENSE STATUS: This FCCH currently operates with a one-star rated license issued on November 15, 2015. REQUIRED DRILLS/INSPECTIONS: The last annual compliance visit was completed on November 29, 2023. The last annual fire inspection was completed on October 31, 2024. The last documented monthly fire drill was conducted on October 14, 2024, the last documented quarterly lockdown/shelter-in-place drill was conducted on August 29, 2024, and the last documented monthly playground inspection was completed on October 14, 2024. During today’s visit, a full assessment of child care requirements was conducted. I observed all required postings, attendance logs, and safe sleep logs. I monitored program/space requirements, equipment and furnishings, capacity & supervision, storage and/or administering of medication, outdoor area & equipment, the operator’s file, and children’s records. Ms. Hill is in compliance with the required trainings, including CPR, First Aid, and SIDS. The qualification letter for Ms. Hill was in compliance. The operator stated she does not have any pets, nor does she provide transportation. She also stated she does not have an additional caregiver. The Verification of Required Information for Operator and Additional Caregivers form and the Operator’s Statements of Responsibility form were completed during today’s visit. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Cans of furniture polish and disinfectant spray were stored on top of a shelf. .1719 (a)(7) 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. One child, A.K, did not have a written feeding schedule on file. .1706(i) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. One child, K.I, did not have a signed statement on file acknowledging receipt of the Summary of NC Child Care Law. GS 110-102 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The caregiver did not complete visual checks on the two (2) sleeping infants aged 6 months and 11 months, and documentation was not maintained on file for the last 30 days . .1724(a)(8)&(f) 1828 The Family Child Care Home operator did not discuss the operational policies with parents on or before the child's first day of attendance. Operational policies were not discussed with the parents of all five (5) children prior to the first day of attendance. 10A NCAC 09. 1715(b) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. Three (3) children, A.K, C.B, and K.I, did not have the SBS/AHT policy on file. .1726(b)&(c) COMMENT: COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this FCCH was 100%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By November 27, 2024, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at tanya.herring@dhhs.nc.gov. If you elect to mail your letter, please ensure that you allow sufficient time so that the letter reaches me on time. My mailing address is: Tanya Herring 2201 Mail Service Center Raleigh, NC 27699-2200 We discussed additional time is available to meet your on-going training requirements. You were reminded that fifteen (15) training hours must be obtained by November 29, 2024. Failure to obtain the number of hours or submit documentation of the required hours may result in additional violations being documented. You must submit the remaining hours to me electronically or by mail. TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, safe sleep logs were not in compliance with child care rules and requirements. These rules are extremely important and have been established to help reduce the risk of sudden infant death syndrome (SIDS) while infants are in child care facilities. Caregivers must check on sleeping infants at least every 15 minutes as required by Child Care Rule .1724 (a)(7). The rule goes on to discuss safe sleep practices for infants under the age of 12 months. ANNUAL LICENSE FEES: Invoices will be emailed to the facility addresses on file by November 30, 2024. Online payments will be due by December 31, 2024. In accordance with North Carolina General Statute § 110-90(1a), licensed child care facilities are assessed an annual license fee. The amount of your annual license fee is based on the licensed capacity printed on your license as of October 1, 2024, not the number of children enrolled at your facility.
