Upon my arrival, I was greeted by staff member, J. Gonzalez. Ms. Gonzalez stated that the administrator, L. Webster, was not present. I was unaccompanied as I completed a general walk-through of the indoor and outdoor learning environments which consisted of the indoor and outdoor learning environments, and the kitchen. A short time later, Ms. Webster arrived and assisted with the visit. A total of twenty-six (26) children were present during the visit. The children were observed during outdoor play and completing routine care tasks. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility is currently operating with a three-star rated license issued on December 20, 2022. The NC Secretary of State website was reviewed on June 3, 2026, and Kids Only Pre-School, Inc. was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was completed on October 16, 2025. The last annual fire inspection was completed September 30, 2025, and the last annual sanitation inspection was completed on June 2, 2026, and received a “Provisional” classification with ten (10) demerits. The last documented monthly fire drill was conducted on May 19, 2026, the last documented quarterly lockdown/shelter-in-place drill was conducted on March 2, 2026, and the last documented playground inspection was completed on May 20, 2026. During today’s visit, a partial assessment of the child care requirements was conducted. I observed all required postings, and attendance logs. This facility is not licensed to provide care for infants. So, safe sleep logs are not required. I monitored program records, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, the outdoor areas and equipment. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The center received a “Provisional” sanitation inspection on June 2, 2026. 10A NCAC 09 .0304(b) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 98%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By June 17, 2026, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. Ms. Webster stated that the repair technician present was in the process of correcting the issues related to the water temperature. I observed handwashing posters displayed in all restrooms, including the restroom located in the hallway. Additionally, Ms. Webster stated that all personal bags and other personal items are stored in the storage area of the building next door. QUALITY RATING IMPROVEMENT SYSTEM (QRIS): During today’s visit, I briefly reviewed the requirements outlined in Pathway 2: Classroom and Instructional Quality Pathway for a three-star rated license. I verified that Ms. Webster is currently implementing a curriculum that meets the requirements outlined in the child care rules. I also reminded Ms. Webster that all staff members including administrators, lead teachers, teachers, program coordinators, and/or group leaders must create DCDEE Works account and submit their official transcripts to the workforce education unit to have their education evaluated in WORKS. Please note that providers without verifiable education may result in a reduction in stars at the facility. Thank you for your time today. The annual compliance monitoring checklist for child care centers was used to note requirements monitored today. Upon my arrival, I was greeted by the owner/operator, L. Webster. I was unaccompanied as I completed a general walk-through of the indoor and outdoor learning environments which consisted of three (3) classrooms in building #1, and one (1) classroom in building #2, the outdoor play areas, and the kitchen. A total of twenty-nine (29) children were present during today’s visit. The children were observed during outdoor play, teacher-directed activities, and completing routine care tasks. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility is currently operating with a three-star rated license issued on December 20, 2022. I also reviewed a sample of the staff files and children’s records. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member with a hire date of August 11, 2025, did not obtain a medical report until August 13, 2025. 10A NCAC 09 .0701(a) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 98%. A compliance verification letter is not required because the violation observed and documented was corrected during the visit. TECHNICAL ASSISTANCE/CONSULTATION: During today’s visit, transportation requirements were monitored. Ms. Webster reported that a 2007 IC bus will be used to transport children to extracurricular activities such as field trips. The vehicle has a current license plate (BMY-1330), registration, and insurance through Progressive Southeastern Insurance Company (exp: December 28, 2025). All applicable transportation requirements have been met. Therefore, transportation has been approved. Please take a moment to review Section .0803(7) of the Child Care Rules, which outlines specific requirements for administering medications in child care centers including over-the-counter medications such as topical ointments, diaper creams, lotions, insect repellent, and sunscreen. This section details the required information that must be included in medication authorization forms. Child care licensing requirements are established to ensure a safe and healthy environment for all children in care. As a child care provider, it is important that you are familiar with all licensing requirements applicable to your program. Maintaining compliance with these laws and rules is a key responsibility and requires regular review of the child care regulations. During today’s visit, we reviewed the Quality Rating and Improvement System (QRIS) Modernization for licensed child care centers and completed the QRIS conversational template. I provided an overview of the key differences between the program standards for the various pathways to help guide your decision. A total of thirty-seven (37) children were present during today’s visit. The children were observed during free choice play, completing routine care tasks, and preparing for lunch. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This facility is currently operating with a three-star rated license issued on December 20, 2022. The NC Secretary of State website was reviewed on May 15, 2025, and Kids Only Pre-School, Inc. was listed as current and active. REQUIRED INSPECTIONS/DRILLS: The last annual compliance visit was completed on November 4, 2024. The last annual fire inspection was completed August 6, 2024, and the last annual sanitation inspection was completed on August 7, 2024, and received a “Superior” classification with two (2) demerits. The most recent documented monthly fire drill was conducted on April 25, 2025, the most recent documented quarterly lockdown/shelter-in-place drill was conducted on May 8, 2025, and the most recent documented playground inspection was completed on May 5, 2025. During today’s visit, a partial assessment of the child care requirements was conducted. I observed all required postings, and attendance logs. This facility is not licensed to provide care for infants. So, safe sleep logs are not required. I monitored program records, equipment and furnishings, staff/child ratios & supervision, storage and/or administering of medication, the outdoor areas and equipment. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #1, an electrical outlet not in use was not covered with safety plugs. 10A NCAC 09 .0604(c) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 99%. A compliance verification letter is not required because the violation observed and documented was corrected during the visit. TECHNICAL ASSISTANCE/CONSULTATION: All electrical outlets and power strips in spaces used by children must have safety measures to protect against electrical hazards. This includes using safety outlet covers or safety plugs when not in use unless the outlets are located behind furniture or equipment that cannot be moved by a child. The Child Care Commission is tentatively scheduled to adopt the new rules for the Quality Rating and Improvement System (QRIS) Modernization, previously the star rated license assessment. Once the rules are adopted, the hold harmless for the rated license assessments will end and all programs will be required to transition. Before the new rules are adopted, please encourage all staff to create a WORKS account using their NCID credentials. The staff members should include administrators, lead teachers, teachers, program coordinators, and/or group leaders. Once the WORKS account is created, staff must submit their official transcripts. Providers without verifiable education may result in a reduction in stars at the facility. The most recent annual sanitation inspection was completed on August 7, 2024, and received a Superior classification with two (2) demerits. The last documented monthly fire drill was conducted on October 12, 2024, the last documented quarterly lockdown drill/shelter-in-place drill was conducted on August 12, 2024, and the last documented monthly playground inspection was completed on October 22, 2024. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings and attendance logs. This facility is not licensed to provide care for infants; therefore, safe sleep logs are not required. I also monitored program records, equipment and furnishings, staff/child ratios & supervision, medications, outdoor area, and equipment. I also reviewed all the staff’s records and a sample of children’s records. This facility does not provide transportation. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). I stated to Ms. Webster that Ms. Ward has fifteen (15) days from today’s visit to submit the necessary documents to obtain a new qualification letter. If no letter has been obtained by November 19, 2024, Ms. Ward may not return until such a time that a new qualification letter is obtained and submitted. As a reminder, criminal background check qualification letters are valid for five years from the date of issuance. Child care providers may apply for the 5-year renewal six (6) months prior to the expiration. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. ABCMS allows administrators to see the real-time background check status of staff members, run printable report of the staff roster to assist with compliance visits, and see new background check applicants and add to the staff roster. To get started, complete the ABCMS Child Care Provider Portal training in Moodle. It is a video tutorial followed by a few questions. Thank you for your time today.
