A rated license assessment visit was completed during today’s visit. The checklist was used to note the requirements I monitored today. During the visit the Administrator. S. Carr accompanied me on the walk- through of the facility and assisted with today’s visit. LICENSE STATUS: Currently this facility operates with a four-star license issued June 9, 2021, earning seven (7) points in the education component, two (2) points in program standards, and one (1) quality point based on staff benefits and parent involvement. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, lesson plans had themes on trees, clothing, balls, and helping hands. Three (3) classrooms of children were observed playing outside. The other six (6) classrooms were observed as the children were playing in activity areas. The lunch menu met the meal pattern requirements. Your last fire drill was completed 9/10/25 and your last monthly outdoor playground inspection was dated 10/3/25. Your last EPR drill was a shelter-in-place completed 9/8/25. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last fire inspection was dated 3/27/25. Your last sanitation inspection was completed 4/14/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. During the visit you stated you were only using one bus at this time to transport children. The bus license plate was BFR-9518. The bus inspection expires in December 2025, and the insurance expires July 1, 2026. Prior to today’s visit, the 18- month compliance history for this facility was 99%. The following violation was documented and corrected during the visit. Violation Number Comment Rule 115 A summary of the NC Child Care Law was not posted in a prominent place in the center. The most recent copy of summary of the NC Child Care Law was not posted in the facility. G.S. 110-102 Childcare licensing requirements are established to ensure a safe and healthy childcare environment. RATED LICENSE ASSESSMENT: The facility operates as an office of Head Start Grantee site and has chosen Pathway 3: Accreditation and Head Start Pathway for a five -star license. The program meets the Head Start and Early Head Start standards including the staff/child ratios. DHHS Memo on Funding and Enrollment Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May1, 2024 deadline. Facilities that have not enrolled should complete the online process by November 1, 2024. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Required water testing for child care centers under Rule 15A NCAC 18A .2816 is not affected by the November 1 deadline. The allegation are as follows: There is a concern that a child was not adequately supervised. Upon arrival I was greeted by the Administrator/ Ms. Carr, who assisted me with today’s visit. I explained there was a complaint, and I reviewed the allegation with you, and you were given the opportunity to ask questions and provide information. Child Care Consultant Tanya Herring accompanied me on today’s visit. A computer-generated document will be sent to you. During today’s visit I discussed the allegation: You stated you had self- reported the incident to the DCDEE Raleigh office on 4/15/24. You stated on 4/12/24 two staff members with a ratio of 2:9 four-year-old children were in their classroom with the children and a therapist who was working with one child inside of the classroom. You stated the therapist told the two teachers she was finished with the child and the child was on the small children’s couch in the classroom. You stated a child in the classroom had an accident and everyone was rushed to the bathroom. You stated it is Telamon’s policy to have 2 staff members present if a child has an accident. Both staff members realized they were missing a child and went back to see about the child and saw the child sitting on the couch. The child was okay and was not upset. You stated you contacted the parent of the child about the incident. Today you provided me with the qualifying letters for the two staff members, the therapist, along with the child’s file to review. You stated the child was still enrolled and attending your facility. The child was not present during the time of today’s visit. The Director, Ms. Carr, stated she had spoken with both staff members, and they had stated it was an accident that should not have happened. You stated the staff understand the importance of adequately supervising the children. You stated you have a policy where the staff count the children every 30 minutes and check to make sure they are present. You stated this information in the classroom in a booklet. You stated a staff meeting was held on Thursday April 18, 2024, to review child care rules along with Telamon active supervision policies. During today’s visit I observed the classroom where the incident occurred, there were 8 children present with 2 staff members in the classroom today. The children were being counted and supervised during today’s observation. I spoke with one of the members in the allegation you stated the same as the Administrator regarding the incident. The other staff member in the allegation was absent today. Limited monitoring of the child care requirements occurred during today’s complaint visit. There were 118 children present today ranging from 1 to 5 years of age in the facility. Based on the information gathered during today’s visit, video footage reviewed, and interviews, the allegation was substantiated. One violation was documented today. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On April 12, 2024 a child was left unattended in a classroom for approximately six minutes. .1801(a)(1-5) Please send me a written statement describing how and when the above violation was corrected. The written statement must be received no later than May 6, 2024. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service. Please note that the two- week timeframe is established to allow time for submission of your letter, not for correcting the violations. All violations should be corrected immediately or as soon as possible. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance. (2) staff must interact with the children while moving about the indoor or outdoor area. (3) staff must know where each child is located and be aware of the children's activities at all times. (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child Please be reminded that children must be adequately supervised at all times. During today’s visit I suggested doing a name to face check throughout the day to make sure all children are accounted for. Please be reminded the confirmed allegations of non-compliance can lead to administrative action. A follow-up visit may be conducted to monitor compliance in the future. Today you stated that the last day for children to attend this school year will be May 16, 2024 and for the teachers last day will be May 20, 2024. . Upon arrival I was greeted by the administrator, Ms. Carr, who assisted me with today’s visit. A computer-generated visit summary will be provided to you within two (2) business days. The allegation is as follows: There is a concern that staff did not interact with children in a nurturing and caring manner. Limited monitoring of the child care requirements occurred during today’s visit. A total of 98 children were present ranging from 1 to 5 years of age. I discussed the allegation with you, and you were given an opportunity to ask questions and provide additional information. You stated that on 11/29/23 there was an incident between two children outside on the playground. You stated that their teacher took out her cell phone and recorded a video of the two children in a physical altercation. The video was then sent to the parents of the two children to show them how they were acting. You stated you were not aware of the video until it was brought to your attention on 11/30/23. You stated that on that same date, the staff member who recorded the video was suspended. On 12/1/23, the teacher was terminated due to failure to comply with the facilities policies and procedures. You stated that you took immediate action once you became aware of the incident. I observed the classroom designated for the two children who were in the video. The caregivers were attentive to the children’s needs and were interacting positively with the children in the classroom. Ms. Carr stated the needs of the children are met daily and that staff interactions are in compliance with state and federal requirements. You stated the staff have training each August before you start a new school year focusing on the nurture and care of children. Based on the information obtained and statements made by staff, the allegation was substantiated. The following violation was cited during today’s visit: Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A teacher video two children in a physical altercation rather than intervening and demonstrating appropriate behavior. G.S. 110-91(10) Due to the nature of the violation cited, a follow-up visit may be conducted to monitor compliance. In addition, a compliance letter is due to me within two (2) weeks of today’s visit. By December 28, 2023, please send me a written statement describing how the violation was corrected and how compliance will be maintained in the future.
