The annual compliance monitoring checklist for FCCH was used to note requirements monitored today. Upon my arrival, I was greeted by the operator, A. Mimms I was unaccompanied as I completed a general walk-through of the indoor and outdoor learning environments. During today’s visit, a total of three children (3) were present at the time of arrival. A short time later, a fourth child arrived during the visit. The children were observed during lunch, completing routine care tasks, and naptime. I also observed both caregivers engaging in positive and nurturing interactions with the children. There were a variety of age-appropriate toys and books in the indoor and outdoor learning environments. Today’s lunch was in compliance with the Meal Pattern requirements. LICENSE STATUS: This FCCH currently operates with a five-star rated license issued on August 20, 2024. REQUIRED DRILLS/INSPECTIONS: The last annual compliance visit was completed on March 5, 2025. The last documented monthly fire drill was conducted on February 9, 2026, the last documented quarterly lockdown drill and/or shelter-in-place drill was conducted on January 20, 2026, and the last documented monthly playground inspection was completed on February 10, 2026. During today’s visit, a full assessment of the child care requirements was conducted. I observed all required postings, and attendance logs. There are no infants currently enrolled, so safe sleep logs were not required. I monitored program/space requirements, equipment and furnishings, capacity and supervision, storage and/or administering of medication. I also reviewed the staff file for the operator and additional caregiver. The operator and her additional caregiver were in compliance with the training requirements including CPR, first Aid, and SIDS. The qualification letter for Ms. Mimms and her additional caregiver, M. Tatum, was on file and available for review. The Verification of Required Information for Operator and Additional Caregivers form and the Operator’s Statements of Responsibility form were completed during today’s visit. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Enrolled child J.S., with a date of enrollment of 10/24/2023, did not have a completed health assessment on file within 30 days of enrollment, as required by the rule. GS 110-91(1); 10A NCAC 09.1721(a)(1) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this FCCH was 92%. COMPLIANCE LETTER: All violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By March 9, 2026, please send me a letter addressing each violation, how the violations were corrected, and how compliance will be maintained in the future. Maintaining these records on file is important, as they provide valuable insight pertaining to each child’s physical and developmental needs. This information helps caregivers effectively plan individualized activities that support each child’s growth and development. Additionally, having up-to-date medical information ensures providers are aware of any communicable diseases or health conditions that may require special care, particularly in situations where this information may not have been previously communicated by the parent. During today’s visit, I reviewed all three (3) pathways of the new Quality Rating and Improvement System (QRIS) modernization with you and completed the QRIS conversational template. Although you are not required to complete the rated license assessment process until August 2027, I provided an overview of the key differences between the program standards for all three pathways to help guide your decision in the future. The annual compliance monitoring checklist for FCCH was used to note requirements monitored today. Upon arrival, I was greeted by the owner/operator, A. Mimms. I was unaccompanied as I completed a general walk-through of the indoor and outdoor environments. A total of three (3) children were present during today’s visit. The children were observed participating in free choice play, completing routine care tasks, and eating lunch. I also observed the caregiver engaging in positive and nurturing interactions with the children. There were a variety of appropriate toys and books in the space designated for child care. Today’s lunch was in compliance with the meal pattern requirements. LICENSE STATUS: This FCCH currently operates with a five-star rated license issued on August 20, 2024. REQUIRED DRILLS/INSPECTIONS: The last annual compliance visit was completed on March 19, 2024. The most recent monthly fire drill and monthly playground inspections were completed on February 24, 2025, and the most recent quarterly lockdown/shelter-in-place drill was conducted on January 10, 2025. During today’s visit, a full assessment of the child care requirements was conducted. I observed all required postings, attendance logs, and safe sleep logs. I monitored program/space requirements, equipment and furnishings, capacity & supervision, storage and/or administering of medication, the outdoor area & equipment, and the operator’s file and children’s records. Ms. Mimms and her additional caregiver are in compliance with the required trainings, including CPR, First Aid, and SIDS. The qualification letter for Ms. Mimms and her additional caregiver was in compliance. The operator stated that she does not have any pets, nor does she provide transportation. The Verification of Required Information for Operator and Additional Caregivers form and the Operator’s Statements of Responsibility form were completed during today’s visit. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 1810 Operator did not retain the safe sleep acknowledgement in the child's records as long as the child was enrolled. One child, N.H., did not have a signed statement on file acknowledging receipt of the safe sleep policy. 10A NCAC 09 .1724(c) COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this FCCH was 98%. CRIMINAL BACKGROUND CHECK PORTAL: As stated in the North Carolina General Statute 110-90.2 & .2703(r), child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete the Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the referenced training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been granted, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers who were hired or moved into the child care facility within five business days.
