This visit was conducted with Ann Pugh, headmaster, and Paige Brewer, administrator. Your program currently operates with a Five Star license. Your program Restrictions include first shift only, children in care on ground level only, no children in diapers, meets all enhanced requirements, plus reduced ratios minus one. We discussed the pathway options during the visit- Program Assessment - Pathway 1, Classroom and Instructional Quality - Pathway 2, OR Accreditation and Head Start Pathway. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five-star licensed facilities where four-year-old children are enrolled. You are currently using the Experience Preschool Curriculum. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 6/23/2026 and Agape Faith Church was listed as current and active. The program’s compliance history was 88% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in group time with the teacher reading and incorporating the lesson plan. I observed materials and activities that were available to the children in the classrooms. Y The last annual compliance visit was conducted on 7/9/2025. A sanitation inspection was completed 4/22/2026 with a Superior classification. The last fire inspection was conducted 2/26/2026. Program records and required postings were monitored. A fire drill was conducted on 6/2/2026. A shelter in place drill was documented on 6/2/2026. An outdoor inspection was documented on 6/5/2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today.Hazardous items were stored in locked cabinets. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In Space 7, there were three areas of chipping paint on the walls that need repair. I emailed the current approved list during the visit today. • Annual evaluations of staff are required to be on file. • Medical/TB should be on file for new staff prior to employment. • Walls should be in good repair and repaired as needed. Paige Brewer, administrator, accompanied me during a walk-through of the facility. During the visit, I spoke with Ms. Brewer, and an additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 209 Children used space that was not approved. The administrator took a child into a unlicensed space used for the school that is located on the premises. GS 110-91(1)&(4-5) 904 Child was handled roughly. On 2/5/25, a staff member forcefully pulled a 4-year-old child down the hall by one arm, while the child cried. .1803(a)(1) 910 Children were disciplined by leaving them alone in a room separated from staff. A staff member placed a 3 year old child in a bathroom, closed the door, and would not allow the child out of the bathroom. .1803(a)(2) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. On more than one occasion, a staff member spoke to multiple 3-year-old children with an aggressive tone to correct behavior, yelled and threatened the children, and called children inappropriate names. .1803(a)(9) 1877 A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk. On multiple occasions staff members restrained children by wrapping their arms and/or legs around the child, not allowing the child to move as a form of discipline for not listening or the child acting out. .1803(a)(10) 9995 Children shall not be in the kitchen except when participating in a supervised activity. On more than one occasion, the administrator took children into the kitchen while the administrator prepared lunch, a violation of Sanitation Rule 15A NCAC 18A .2808 (e). All violations documented above must be corrected immediately. A written, dated, and signed letter of compliance must be submitted to me within one week, by (3/6/25), describing how and when the violations were corrected and how compliance will be maintained in the future. Paige Brewer, Administrator, assisted me with the visit. Your program currently operates with a five-star license effective 2/20/23. Restrictions include day time care only, children in care on ground level only, no children in diapers, meets all enhanced requirements plus reduced ratios minus one. The NC Secretary of State website was reviewed on 2/24/2025 and Agape Faith Church was listed as current and active. The program’s compliance history was 93% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. adequate during the visit. I observed positive interactions today. I observed children eating lunch and working directly with staff members. A file for four new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPA/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded 1/7/2025. A fire drill was recorded 2/18/2025. A shelter-in-place drill for the facility was recorded 12/20/2025. The most recent fire inspection was on 1/22/2024. I observed a sanitation inspection was last conducted on 8/22/24 earning a Superior classification. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was conducted on 1/22/2024. 10A NCAC 09 .0304(a) 1757 A valid qualification letter was not on file and available to review at the facility. One new staff member did not have a printed qualification letter on file. Paige Brewer, Administrator, assisted me with the visit. Your program currently operates with a five-star license effective 2/20/23. Restrictions include day time care only, children in care on ground level only, no children in diapers, meets all enhanced requirements plus reduced ratios minus one. The program’s compliance history was 91% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in an art activity, in center play and working directly with staff members. A file for one new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPA/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items was monitored today. A playground inspection was recorded 2/19/2025. A fire drill was recorded 2/12/2024. A shelter-in-place drill for the facility was recorded 12/11/2023. The most recent fire inspection was on 1/22/2024. I observed a sanitation inspection was last conducted on 8/22/23 earning a Superior classification. The following violation was cited during today’s visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One over-the-counter ointment in Space 1 had an expired permission. 10A NCAC 09 .0803(4)(6-9) Technical Assistance: · The permission for topical ointments are good for one year. Additional information: Ms. Brewer stated that they would like to add an additional classroom for school-age care. A building inspection, fire inspection and sanitation inspection will be needed. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on August 7, 2023 and Agape Faith Church was listed as current and active. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in an art activity, group time, and routine care. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Experience Preschool. A sanitation inspection was completed on 9/22/2023 with a Superior classification. The last fire inspection was conducted 2/19/2023. Program records and required postings were monitored. A fire drill was conducted on 7/10/2023. A shelter in place drill was documented on 6/5/2023. An outdoor inspection was documented on 7/5/2023. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation for field trips only. During today’s visit, I monitored field trip information. I observed first aid kits and fire extinguishers on the vehicle. I monitored two vehicles today. I observed two 2016 Ford Vans-EFJ-2721 and EFJ-2722. The vehicles were clean and in good repair and had adequate tire tread. Both vehicles were current on inspections and insurance. Your next inspection date on these vehicles is due October 2023 and the insurance is valid through May 2024. The following violations were cited during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in Space 2. GS 110-91(12); .0508(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet.
