Your program currently operates with One Star license effective 8/5/2022. Restrictions include daytime care, maximum of five preschool children, children in care on ground level only, and fireplace not in use during operating hours. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS – • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Ms. Casey stated the only persons living in the home with her are her husband and daughter. Be reminded that you and your husband and daughter, must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. A walk-through of the child care area was completed today. Indoor and outdoor areas were observed. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play, napping, and eating snack. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 1/26/2026. A fire drill was recorded 1/26/2026. A shelter-in -place drill for the facility was recorded on 12/17/2025. There are no pets reported in the home. Ms. Casey does not currently provide transportation. The following violations were cited at today’s visit. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The operator's first aid training was not from an approved course. .1703(a)(2) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator and household members were not linked to the ABCMS portal. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. There are no pets reported in the home. The outdoor area was observed today. The door to the outside fence had been taken down for repair. Ms. Casey stated it was loose and the latch needed to be repaired. She has an additional covered play area for outdoor play. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. I observed a 2022 Honda Pilot used for transportation. It is current on insurance and the inspection. She primarily transports during the school year from preschool to her family child care home. She has emergency information, first aid kit, and fire extinguisher as well as current car seats. The following violation was cited during today’s visit: Violation Number Comment Rule 709 Equipment and toys were not in good repair and developmentally appropriate. The gate on the outside fence was not attached and needs to be repaired. Additional Information: - I will contact you when I receive additional information on requirements for additional transportation providers. -Since February 2023, the NC Child Care Commission has been working toward revising the Quality Rating Improvement System (QRIS) as required by Session Law 2023-40. Listening to and communicating with early childhood professionals, families, communities and businesses is a critical component to ensure a successful QRIS modernization. In 2023, the Commission conducted 8 in-person listening sessions and 2 webinars; provided a QR code survey and offered a QRIS email account to collect feedback. Thus, informed by national research, trends and outcomes, and input from more than 1800 NC early childhood education professionals, the Commission formed recommendations for change, such as: Offering a choice of three pathways to demonstrate quality and earn a 2-5 Star Rated License. Updates to education standards for early childhood educators to expand options; and Adds standards for both educators and facilities that support continuous quality improvement and family/community engagement practices that promote communication and collaboration. In addition, on July 8, 2024, Governor Roy Cooper signed Session Law 2024-34, which includes Quality Rating Improvement System (QRIS) Modernization. This law established three pathways to earn star ratings, directed the Commission to continue with rulemaking to support the pathways and allowed the following while rulemaking occurs: Extension of Hold Harmless provisions - star rated license assessments and reassessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments continue to be required for new child care programs. Accreditation Licensure - Child care programs currently accredited through the approved accrediting bodies may move to a 3- or 5-star license if they are not already at this star level. Providers will need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Updated Program Assessment Tool – Beginning on February 1, 2025, child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. This updated edition includes language and interaction measures, a need consistently identified by early childhood professionals during listening sessions. She will get all transportation forms signed before transporting this fall. I observed the 2022 Honda Pilot today with license plate FDZ-1497. The following violation was cited during today’s visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. I stated to Ms. Casey that the local zoning ordinance for Forsyth County required that family child care homes have a four foot fence with a minimum area of 100 square feet. I will contact zoning to get additional information on fencing requirement. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The local zoning requirement of a four foot fence with a 100 square feet per child was not maintained. Part of the fence was removed from the premises. GS 110-91 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 7/26/2023. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to jennifer.davis@dhhs.nc.gov. This visit was conducted by Jennifer Davis, childcare consultant with Connie Casey, Operator. Your program currently operates with a One Star license. Restrictions include daytime care only, maximum of five preschool children, children in care on ground level only and fireplace not used during operating hours. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 100% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. Upon arrival, capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in free play, and routine care. I observed developmentally appropriate materials and activities were available to the children in the licensed space. The last annual compliance visit was conducted on 7/20/2022. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed owner status of this facility.
