    Name of Operation: TEENA'S FAMILY CHILDCARE, LLC Facility ID: 51000895 Consultant: LEIGH MCKENZIE-LEE Operation Type: Family CC Home Case Number: Visit Date: 6/4/2025 Number Present: 3 Completed Date: 6/4/2025 Age: From 2 To 4 Total Minutes: 115 Time In: 01:05 PM Time Out: 03:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance of applicable family child care home requirements during a routine unannounced visit. I monitored the indoor environments for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers, First Aid Chart and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire and emergency procedures were monitored for compliance. Three preschool children, including your two preschool children, were present and were observed during naptime. A four star family child care home license with seven points in education, two points in program and one quality point was issued to you on 2-28-21. Based upon observations made and information provided during the visit, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. Medication authorization was not available for the medications administered to a school age child in care, including Clonidine, Oxcarbazepin, Quetiapine, Guanfacine, Vitamin D and Benedryl. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 917 Record of all medication given was not kept on file for each child during the course of treatment and for six months following the end of treatment. Written documentation for medication administered to a school age child from 5-7-25 to date of visit was not available for review. 10A NCAC 09 .1720(c)(6) 1401 Prescribed medications were not stored in their original containers in which they are dispensed with the pharmacy label. Medications administered to a school age child were stored in a pill box, not in the pharmacy bottles. Three different types of medication were stored in the pill box, which was in locked storage, at the time of the visit. 10A NCAC 09 .1720(c)(1)&(2) 1416 Questionable medication or instructions concerning medication was administered without signed, written dosage instructions from a licensed physician or authorized health professional. The provider administered unknown medications to a school age child in care. .1720(b)(5) 1417 Over-the-counter medications were not stored in the manufacturer's original packaging on which the child's name is written or labeled and was not accompanied by written instructions specifying required information. Over the counter medications were not stored in the manufacturer's original packaging. .1720(b)(4)(A-E) 1423 Operator did not record required information when administering prescription or over-the-counter medication. Written documentation of medications administered to a school age child in care was not completed each time he was administered prescription or over the counter medication. .1720(b)(13)(A-E) During the visit, we discussed the importance of ensuring the all medication be provided to you in the original container. If the medication is prescription, it must be provided to you in the bottle it was dispensed in at the pharmacy. Over the counter medications must be in the bottle or box it was packages in by the manufacturer. At the time of the visit, a variety of medications were observed in a pill box. The following medications were listed as medications the child takes on his medical form: Clonidine, Oxcarbazepin, Quetiapine, Guanfacine, Vitamin D and Benedryl. Authorization to administer this medication was not available as the child's guardian had not completed these forms. You must have this authorization prior to administering medication to a child in care. During the visit you stated that this child missed a dosage of one of the medications earlier in the day. You stated that the medication was provided by his guardian; however, you failed to give the medication to the child. You also stated that you had not been completing written documentation when the medication has been administered to the child as you were not certain about the name of the medication you were giving to the child. I recommended that you discuss the child's medication needs with his guardian to determine if any or all of these medications can be administered by the child's guardian before care and/or after care to reduce the medications you are administering to him while in your care. We discussed the safety concerns for administering unknown medications to a child while in your care. You were not made aware of the type of medication, the dosage, any possible side effects or symptoms of a negative reaction to these medications. You also cannot be certain that the prescription medications were prescribed to this child. In the future, all child care requirements pertaining to administering medication must be met before you can take possession of a medication or administer the medication to a child in your care. During the visit, we reviewed section .1720 and .1721 of the child care requirements. Please refer to these sections for additional information on medication administration and on the documentation requirements for medication. We also reviewed the Medication Error form that you must complete for failure to administer his medication earlier in the day. Please obtain his guardian's signature on this form and provide the guardian a copy of the completed form. Please contact me if you have additional questions or need further assistance with medication requirements. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 6-18-25. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service. I monitored the indoor and outdoor environments for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, first aid chart, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire, emergency procedures and the children's records were also monitored for compliance. The last fire drill was documented as completed on 11-20-24 at 8:08 am with an evacuation time of fifteen seconds. The last outdoor inspection was documented as completed on 11-20-24. A shelter in place drill was documented as conducted on 11-20-24 at 8:39 am. Your one year old and three year old children were present at the time of visit; both children were sleeping during naptime. A four star family child care home license with seven points in education, two points in program standards and one quality point was issued to you on 2-28-21. Based on the information provided and on observations made, you continue to meet the standards previously met to obtain such a license. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. A First Aid course was last completed by the provider on 11-12-22; therefore, a new course was due on or before 11-12-24. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. A CPR course was last completed by the provider on 11-12-22; therefore, a new course was due on or before 11-12-24. .1703(a)(3) 716 Electrical outlets not in use were not covered. Two outlets in the child care area were not covered or in use. 10A NCAC .1719(a)(27) 1831 A valid qualification letter was not on file and available for review at the facility. The qualification letter issued 4-30-24 for one of your household members was not available for review. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) At the time of the visit, you could not locate the qualification letter issued for your mother, Shirlyn Banks. I verified that a letter was issued for her on 4-30-24. You attempted to log in to the ABCMS system to print a new letter but received an error message. I recommended that you contact the BCB Unit at the phone number or email address listed in the error message for additional assistance. To correct this violation, please include a copy of her letter along with verification of completion of First Aid and CPR.
