I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. The storage of medication and hazardous products was also monitored for compliance. Two new staff member were hired since the 5-8-24 annual compliance visit. In addition, two substitutes were present in the classrooms during the visit. Files for these staff member were reviewed during the visit. A fire drill was documented as conducted on 11-18-24 at 2:30 pm with an evacuation time of one minute and fifty-two seconds. A lock down drill was documented as conducted on 10-17-24 at 10:20 am. A playground inspection was last completed on 10-14-24. An approved fire inspection was completed on 1-4-24. A sanitation inspection was last completed on 11-6-24 with zero demerits and a superior classification. The children were observed participating in free play activities A meal or snack was not observed during the visit. A five star child care center license was issued to you on 3-21-16 with five points in education, seven points in program and one quality point. Based on observations made and on information provided, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A playground inspection was not completed for November, 2024. .0605(q) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Verification of review of the EMC plan was not available for two staff members with hire dates of 10-1-24 and 9-30-24. 10A NCAC 09 .0802(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Current emergency information was not available for the two substitutes present in the classrooms. .0701(a) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Verification of review of the EPR plan was not available for two staff members with hire dates of 10-1-24 and 9-30-24. .0607(f) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Verification of review of the policy was not available for one staff member with hire date of 10-1-24. .0608(d)(1-4) ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. G.S. 110-90.2(b) 1755 Certified developmental day center staff/child ratios and group sizes were not met. Thirteen children were observed on the playground with two staff members, while the third staff member was inside the classroom. Later in the visit, eleven children were outside with one staff member, while the other two staff members were inside. A one to six staff/child ratio was not maintained. .2902 (f) 1757 A valid qualification letter was not on file and available to review at the facility. A qualification letter was not available for the substitute in space 512. G.S. 110-90.2(b) & (d) & .2703(e) Upon arrival for the visit, one of the teacher assistants in space 122 was inside of the classroom. Thirteen children were present with you and the other teacher assistant on the playground. Based on the developmental day certification on your license, a one to six staff/child ratio is required to be maintained at all times. While I was outside on the playground, this same teacher assistant walked back into the classroom, again leaving thirteen children with two staff members. Later in the visit, while you and one of the children were inside, the other teacher assistant brought a child into use the restroom, leaving eleven children outside with one staff member. I encouraged you to discuss the required ratios with both teacher assistants. I also encourage you to discuss what must be done to maintain compliance, such as bringing the number of children in with them to maintain ratios on the playground like you did when you brought one child in with you to assist me. This type of violation directly impacts the safety of the children while in your care and must be corrected immediately. Consecutive violations of staff/child ratio requirements may lead to the issuance of an administrative action. While in space 512, I determined that a long term substitute who was present during the visit, has not completed a criminal background check and was present without a qualification letter. This process must be completed prior to employment. Staff members without a qualification letter cannot be present in the classroom. I discussed this with the teacher in this classroom and the substitute. Based in information provided, the principal and assistant principal were aware of this requirement for staff members in the licensed preschool classrooms. In addition, the Director of the Preschool Department is aware of this requirement. Prior to the conclusion of the visit, I spoke with the principal and explained that this substitute could not remain in the classroom. She stated that there were other staff members on site at the school with a current qualification letter. A staff member with a qualification letter issued on 8-21-19 was moved from another classroom to classroom 512 and the substitute was moved to a classroom for older children that is not licensed by DCDEE. I will follow up with the Preschool Department regarding this violation. Consecutive visits with violation of criminal background check violations, as well as willfully disregarding child care requirements, may lead to the issuance of an administrative action. ****Child care licensing requirements are established to ensure a safe and healthy child care environment.
