I monitored the indoor environment for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were posted. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire and emergency procedures were monitored for compliance. The last fire drill was documented as completed on 4-22-26 at 3:30 pm with an evacuation time of one minute. The last outdoor inspection was documented at completed on 4-24-26. A lockdown drill was documented as completed on 3-20-26 at 11:00 am. Five preschool children were present and were observed in free play activities and eating lunch, which consisted of sliced turkey, cheese sticks, pickles and carrots, strawberries, whole grain cracker and milk. A four star family child care home license was used on 1-23-19 with seven points in education, two points in program and one quality point. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. A well water test was last completed on 12-28-23; therefore a new test was due on or before 12-28-25. 10A NCAC 09 .1725(a)(1) 908 Health questionnaire was not completed annually. A health questionnaire was last completed on 1-1-25. .1703(a)(1) During the visit, we reviewed the changes to the rated license requirements that were effected July 1, 2025. You will be required to apply for a new rated license under these new requirements. We reviewed the various options, including the Program Assessment Pathway and the Classroom and Instructional Quality Pathway, and you stated that you will apply for a new license under the Classroom and Instructional Quality Pathway. The documents you will need to complete and submit to me to apply for a new rated license were reviewed and left with you: Application for Rated License - Please complete the top portion of the form, select Classroom and Instructional Quality Pathway and sign and date the form. Continuous Quality Improvement Plan - Please complete the form, identifying one goal, steps to meet that goal and the timeframe for completion. Family and Community Engagement - Please complete the form and provide documentation on how you are meeting the items listed at the top of the form for the two star level. You must also submit documentation regarding how you are meeting the two other options you are meeting from the various options listed on page 2. Curriculum, observations and additional training hours - To be eligible for a four star license under the pathway you have selected, you will need to use an approved curriculum for all ages of children in care, conduct annual assessments of each child in care using an approved formative assessment tool and share the results with the child's parent and complete five additional on-going training hours per year. You can find a list of approved curriculum and formative assessment tools on the DCDEE website at https://ncchildcare.ncdhhs.gov/. As we discussed, the Early Learning Matters is an approved curriculum option that is free of charge. There is also a free formative assessment called Desired Results Developmental Profile (DRDP). Before your annual visit, please determine which curriculum and formative assessment tool you will implement, the timeframe for completion of child assessments and how the information will be shared with the parents. Lastly, based on your on-going training requirement, you will be required to complete a total of thirteen on-going training hours annually. This will be monitored during annual compliance visits beginning in 2027. Please contact me if you need assistance with completion of the forms. As we discussed, you will need to apply for a new rated license by September. Your rated license will be reassessed during your annual compliance visit. Prior to today's visit, you contacted me by email regarding difficulties with completing your new criminal background check. Please contact Serena Luke in the CBC Unit at serena.luke@dhhs.nc.gov to schedule a time to speak with her by phone. Ms. Luke will assist you with this process. You will contact her by phone at 919-814-0175. You are due for a new check by 8-12-26 and your household member is due by 8-26-26. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Please send me a written statement describing how and when the above violation was corrected. The written statement must be received no later than 6-10-26. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. Based on observations made and information provided, you continue to meet the requirements previously met to obtain such a license.
