Name of Operation: LITTLE BELIEVERS ACADEMY Facility ID: 51000824 Consultant: LEIGH MCKENZIE-LEE Operation Type: Center Case Number: Visit Date: 3/4/2024 Number Present: 59 Completed Date: 3/4/2024 Age: From 0 To 5 Total Minutes: 105 Time In: 10:15 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance of applicable child care requirements during your annual compliance visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. All new staff files and a percentage of existing staff files were reviewed and the information on the Staff and Training worksheets was verified. A sample of children's records were reviewed and the worksheet was completed. The last fire drill was documented as conducted on 2-23-24 at 9:51 am with an evacuation time of two minutes and eleven seconds. The last outdoor inspection was documented as conducted on 2-7-24. A shelter in place drill was documented as conducted on 2-8-24 at 10:16 am. An approved fire inspection was completed on 9-18-23. A sanitation inspection was last completed on 2-29-24 with sixteen demerits and a provisional classification. The children were observed in free play and circle time activities. Lunch, which was served during today's visit, consisted of chicken nuggets, mixed vegetables, strawberries, whole grain bread and milk. The incorporation status of Little Believer's Academy, Inc. was verified as current and active during the visit using the Secretary of State's website. A five star child care center licensed was issued for Little Believer's Academy, Inc. on 6-28-19 with seven points in education, six points in program standards and one quality point. Based on information provided and on observations made during the visit, you continue to meet the requirements previously met to obtain such a license. Prior to the visit, the owner of the center contacted me and submitted the necessary forms to request Environment Rating Scale assessments. This request will be submitted following the visit and you will be contacted to schedule the window for completion of the assessments. Please contact me if you have questions regarding the assessments. Upon receipt of the results, I will contact you to schedule a visit to review the results. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted in space 2 was dated for 2-26-24 to 3-1-24. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Three outlets in a power strip were not covered or in use in space 2. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A Magic Eraser was observed on a shelf in space 4, not in locked storage as required. .2820(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A first aid course was not completed every two years as required by one staff member with a hire date of 8-16-21. This staff member's First Aid expired in February, 2024. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A CPR course was not completed every two years as required by one staff member with a hire date of 8-16-21. This staff member's CPR expired in February, 2024. .1102(d) 9999 A violation was found for which there is no item number. A staff member's purse was observed on a cubby in space 2, not out of the reach of the children. This is a violation of a requirement in 15A NCAC 18A .2820(f) which states 'Employee purses and other personal effects shall be kept out of reach of children.' The staff member who's First Aid and CPR expired in February was registered for a class on the day of the visit. As we discussed, staff members must complete these required courses prior to the expiration date of their previous certification. You may choose to use a spreadsheet or dry erase board to track all due dates, such as CPR, First Aid, ITS-SIDS, CBC qualification letters, on-goin training and Health and Safety Trainings, for each staff member. ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 3-18-24. The two week timeframe for submission of your compliance letter is allowed for completion and submission of the letter. The timeframe is not provided for correction of the violations. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service.
