I monitored the indoor environments for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers, First Aid Chart and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire and emergency procedures were monitored for compliance. The last fire drill was documented as completed on 11-3-25 at 3:25 pm with an evacuation time of two minutes and twenty-eight seconds. The last outdoor inspection was documented as completed on 11-21-25. A shelter in place drill was documented as conducted on 10-3-25 at 2:30 pm. Four preschool children were present and were observed during naptime. A meal or snack was not observe during the visit. A three star family child care home license with four points in education, two points in program and one quality point was issued to you on 11-8-23. Based upon observations made and information provided during the visit, you continue to meet the requirements previously met to obtain such a license. You stated during today's visit that you plan to retire in August, 2026. Therefore, a transition worksheet for the new rated license system was not completed. Violation Number Comment Rule 908 Health questionnaire was not completed annually. A current health questionnaire was not available for review. .1703(a)(1) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). A new qualification letter was not obtained prior to the 6-5-25 expiration date for the provider and one household member. G.S. 110-90.2 & .2703(n) & (o) 1831 A valid qualification letter was not on file and available for review at the facility. A valid letter was not available for the provider and one household member. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) At the time of today's visit, you and your husband had not obtained new qualification letters from the Criminal Background Check Unit. Based on the date that your last letters were issued, 6-5-20, new letters were due on or before 6-5-25 for both of you. As we discussed, you will have fifteen days from today's visit, or by 12-10-25, to obtain new letters for both of you. Upon receipt of the letters, please scan and email a copy of the letters to me to verify receipt. If you do not verify receipt of these letters by 12-20-25, your license may be placed on inactive status, which would mean you could not provide care in your home, until you verify receipt. During the visit, we reviewed the process for obtaining a new qualification letter, including completion of the necessary steps through the CBC Portal, payment to DCDEE for the necessary fees and having new electronic fingerprints completed. You attempted to log into the portal while I was present in your home and you were not able to get into the portal. I recommended that you contact the customer service number listed on the portal for assistance with logging in. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 12-10-25. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service. Today's visit also counts as your routine unannounced visit. Kareema King, Child Care Consultant, accompanied me on today's visit. During the visit, I monitored the indoor environments for compliance. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire, emergency procedures and the records for two newly enrolled children were also monitored for compliance. The last fire drill was documented as completed on 10-9-23 at 11:00 am with an evacuation time of 2 minutes and 39 seconds. An outdoor inspection was documented as completed on 10-9-23. A shelter in place drill was documented as conducted on 9-1-23 at 9:15 am. Five preschool children were present and were observed in free play activities. A meal/snack was not observed during the visit. A three year rated license assessment was completed during the visit. A three star license will be issued based upon the following point levels: Staff Education - Based upon the information currently available in the Workforce database, you have completed the NCECC coursework plus seven additional semester hours. Therefore, you are eligible for four points in education. Program Standards - You continue to meet minimum requirements and have completed a three month self study with the assistance of one of the staff members at the Partnership for Children of Johnston County. Therefore, you are eligible for two points in program standards. Quality Point - You stated during today's visit that you would like to continue serving no more than two children less than one year of age. Therefore, you are eligible for one quality point. Total Points - 4 (education) + 2 (program) + 1 (quality) = 7 points or a three star license. Your three star license will be printed in the Raleigh office and mailed to you. Please contact me if you do not receive your new license by 11-25-23. Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has.
