I monitored the indoor environment for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS and health questionnaire were monitored for compliance. The children were observed while eating lunch, which consisted of chicken nuggets, french fries, apples and apple juice. The last fire drill was documented as completed on 3-27-26 at 4:00 pm with an evacuation time of thirty seconds. The last outdoor inspection was documented a completed on 3-27-26. The last lockdown drill was documented as conducted on 1-3-26. You are currently licensed as a one star family child care home. If you leave items outside that you need for the children or yourself, to maintain supervision, you must take the children back out with you. The children cannot ever be left alone in the facility, even for a brief amount of time. I observed that three of the children's files (DK, MK and JB) did not have a fully completed application. Please ensure that all areas on the application are either completed or marked as NA. One child's file (DW) was missing a medical and immunization, and one child's file (RB) was missing the verification of receipt of the summary statement. Name of Operation: KATHY'S DAY CARE HOME Facility ID: 51000104 Consultant: LEIGH MCKENZIE-LEE Operation Type: Family CC Home Case Number: Visit Date: 10/3/2023 Number Present: 6 Completed Date: 10/3/2023 Age: From 1 To 11 Total Minutes: 40 Time In: 11:30 AM Time Out: 12:10 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance of applicable family child care home requirements during your annual compliance visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Smoke detectors, fire extinguishers, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, toys and materials and the bathroom the children use were all monitored for compliance. The permit, schedule, SIDS information, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. Qualifying letters, CPR/ first aid, ITS-SIDS, health questionnaire, emergency procedures and the children's records were also monitored for compliance. The last fire drill was documented as completed on 9-25-23 at 4:30 pm with an evacuation time of thirty seconds. The last outdoor inspection was documented as completed on 9-5-23. A lockdown drill was documented as completed on 7-30-23 at 11:30 am. Five preschool children and one school age child were present and were observed playing outside and washing their hands as they returned inside. A one star family child care home license was issued to you on 9-18-06. Please contact me if you have questions or are interested in applying for additional stars. Violation Number Comment Rule 720 Adequate supervision was not provided for children in the Family Child Care Home. The provider exited the facility, going out to the playground to get a child's bottle, leave all six children inside unsupervised. .1711(a) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A completed medical or health assessment was not available in one child's file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Immunization records were not available in one child's file. GS 110-91(1); .1721(a)(2) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. Verification that the summary was provided was not available in one child's file. GS 110-102 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. The application was not fully completed in three of the children's files. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) 1981 When juice was served, it was not 100% fruit juice and/or was not limited to 6 ounces per day, for all ages. A child was observed drinking Nature's Twist, which is not 100% juice. .1706(u)(7) A violation for failure to adequately supervise the children was documented. After you and the children came inside and everyone washed their hands, I observed you going back to the playground and you stated that you left a child's bottle outside. When this was discussed with you, you stated that you thought it was fine to go back out since I was there with the children. I explained to you that you did not ask me about supervising the children and that I cannot be responsible for providing supervision during visits. If you leave items outside that you need for the children or yourself, to maintain supervision, you must take the children back out with you. The children cannot ever be left alone in the facility, even for a brief amount of time. I observed that three of the children's files (DK, MK and JB) did not have a fully completed application. Please ensure that all areas on the application are either completed or marked as NA. One child's file (DW) was missing a medical and immunization, and one child's file (RB) was missing the verification of receipt of the summary statement. During the visit, I observed the school age child drinking Nature's Twist, which is not 100% juice. A violation was documented as children can only be served 100% juice. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 10-17-23. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service.
