    Name of Operation: KAREN'S LITTLE HOUSE OF JOY, LLC Facility ID: 51000651 Consultant: LEIGH MCKENZIE-LEE Operation Type: Center Case Number: Visit Date: 11/12/2024 Number Present: 5 Completed Date: 11/12/2024 Age: From 1 To 4 Total Minutes: 70 Time In: 10:20 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's unannounced visit monitor compliance of applicable child care requirements during a routine unannounced visit. I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. The storage of medication and hazardous products was also monitored for compliance during today's visit. The last fire drill was documented as completed on 11-1-24 at 9:30 am with an evacuation time of fifteen seconds. The last outdoor inspection was documented as completed on 11-1-24. A lockdown drill was documented as conducted on 10-1-24 9:00 am. An approved fire inspection was completed on 8-29-24. A sanitation inspection was last completed on 6-26-24 with zero demerits and an superior classification. The children were observed during free play. A meal or snack was not observed during the visit. A four star center in a residence license was issued to you on 6-17-19 with seven points in staff education, three points in program standards and one quality point. Based on the information provided and on observations made during the visit, you continue to meet the requirements previously met to obtain such a license. During the visit, we discussed the extension of the hold harmless status with regards to rated license until a new QRIS has been implements. We also discussed the changed from use of the FCCERS-R to the FCCERS 3 beginning in early 2025. Please contact me if you would like to request a rated license reassessment, including an Environment Rating Scale assessment prior to the implementation of the new QRIS. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was previously completed on 8-7-23. A new inspection was not completed until 8-24-24. 10A NCAC 09 .0304(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A variety of cleaning products were stored in the space under your staircase; the lock on the door to the space was not unlocked at the time of my arrival. .2820(b) Prior to today's visit, you completed an addition to your child care space that is used for office space. I received written approval from Environmental Health for the addition. During today's visit, I obtained a copy of the Certificate of Occupancy from the Johnston County Inspections department. To approve the space to be used by the children, you will need to obtain a building inspection for child care. You may obtain a copy of this inspection form on our website. As we discussed during the 4-16-24 visit and again today, you will need to hire an architect or engineer to complete the inspection form as the Johnston County Inspections Department will not complete these inspections. I will consult with our Lead Consultant on who in the county can complete these inspections and send that information to you by email. You also added space to your outdoor play area, creating a space for your school age children to play. In the space, you have two non-stationary play structures, which do not require surfacing or fall zones. A violation was documented during the visit for failure to obtain an approved fire inspection on or before the date of the previous inspection. You stated that you contacted the inspector's office the month prior to the due date of the inspection to request that an inspection be completed or or before the date of you last inspection. You stated that the inspector indicated during the inspection that he "forgot" you. You also stated that he explained that based on their requirements, he had until the end of the month to complete the inspection. This is different from child care requirements, which state that an inspection must be completed annually. I will request further clarification on this and reach out to the inspector's office if needed. The violations documented were corrected during the visit; therefore a compliance letter was not required as a follow-up to the visit.
