Name of Operation: CLEVELAND ELEMENTARY SCHOOL Facility ID: 51000822 Consultant: LEIGH MCKENZIE-LEE Operation Type: Center Case Number: Visit Date: 3/13/2025 Number Present: 12 Completed Date: 3/13/2025 Age: From 4 To 5 Total Minutes: 85 Time In: 11:35 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance of applicable child care requirements during your annual compliance visit. I monitored the indoor and outdoor environments for compliance with applicable child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Files for new staff members and a percentage of existing staff were reviewed during the visit and the information on the Staff and Training worksheet was verified. A sample of children's records were reviewed during the visit and the worksheet was completed. A fire drill was documented as completed on 3-10-25 at 9:15 am with an evacuation time of one minute and thirty seconds. A lockdown drill was documented as conducted on 1-16-25 at 9:28 am. An outdoor inspection was completed on 3-3-25. An approved fire inspection was completed on 1-14-25. A sanitation inspection was last completed on 10-11-24 with two demerits and a superior classification. The children were observed during naptime. A four star child care center license with four points in education, seven points in program standards and one quality point was issued to you on 3-5-2020. Based on observations made and information provided during the visit, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet in space D408 was not covered or in use. 10A NCAC 09 .0604(c) The violation documented was corrected during the visit; therefore, a compliance letter was required as a follow up to the visit. Upon my arrival, I was greeted and escorted to the PreK classroom by front office staff. Once in the classroom, I was assisted by lead teacher, J. Reichart. LICENSE STATUS: Currently, this this child care center operates with a 4- star rated license issued March 7, 2023. A total of twenty- eight (28) children were present during today’s visit. Children were observed during circle time, completing routine tasks and having lunch. The meal was in compliance with meal pattern requirements. I completed a staff and training worksheet for three (3) new staff members. I also observed a variety of toys, books, and materials for the children to explore throughout the day. During today’s visit, a partial assessment of the Child Care Requirements was conducted. I observed all required postings, attendance logs and completed a review of all program records. Any items not in compliance are listed in the violations section below. REQUIRED INSPECTIONS: The last annual compliance visit was completed on 04/18/23. The last documented fire drill was conducted on 12/13/23. The last documented shelter in place/lockdown drill was conducted on 12/11/23, and the last documented monthly outdoor play inspection was conducted on 12/13/21/23. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was conducted on 1/9/2023. 10A NCAC 09 .0304(a) Compliance Verification Letter: The violations documented during today’s visit. Violations must be corrected immediately. A compliance verification letter must be submitted to me no later than 1/24/2024. In your letter, please include statements addressing how and when the violations were corrected and how compliance will be maintained moving forward. The compliance letter must also include the facility ID number. You can submit the letter to me at Kareema.King@dhhs.nc.gov. COMPLIANCE HISTORY: Prior to today’s visit, the 18- month compliance history for this facility was 95%. Child care licensing requirements are established to ensure a safe and healthy environment; therefore, it is important for you to be knowledgeable of all the licensing rules and requirements that apply to your program.
