Name of Operation: CLAYTON HEAD START Facility ID: 51000121 Consultant: LEIGH MCKENZIE-LEE Operation Type: Center Case Number: Visit Date: 12/10/2025 Number Present: 10 Completed Date: 12/10/2025 Age: From 4 To 5 Total Minutes: 65 Time In: 10:30 AM Time Out: 11:35 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance of applicable child care requirements during your annual compliance visit and to complete a rated license assessment. I monitored the indoor and outdoor environments for compliance with applicable child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, daily schedule, staff/child ratios and a summary of NC Child Care Law were all posted. The storage of medication and hazardous products was also monitored for compliance. A sample of children's records were monitored. New staff files were reviewed during the and the information on the Staff and Training worksheet was verified. A fire drill was documented as completed on 11-10-25 at 12:05 pm with an evacuation time of forty three seconds. A shelter in place drill was documented as conducted on 11-19-25 at 9:40 am. An outdoor inspection as documented as completed on 11-14-25. An approved fire inspection was completed on 9-8-25. A sanitation inspection was completed on 10-30-25 with three demerits and a superior classification. The children were observed during circle time and playing outside. A meal or snack was not observed during the visit. The incorporation status of Johnston Lee Harnett Community Action, Inc. was verified during the visit using the Secretary of State's website. Prior to today's visit, an Application for Assessment for a Rated License for Centers was submitted for the facility. Based on new child care requirements, head start programs are eligible for a five star license under the Accreditation and Head Start Pathway. Along with the application, the following documents were also submitted as required: Request to Use Accreditation and Head Start Licensure Pathway to Earn a Star Rated License, your Notice of Award verifying your status as a head start site and the staff/child ratios your are currently meeting. A new five star license will be printed and mailed to your following today's visit. Please contact me if you do not receive the new license in the mail by 1-15-26. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet by the children's bathroom was not covered or in use. 10A NCAC 09 .0604(c) The violation that was documented was corrected during the visit. I monitored the indoor and outdoor environments for compliance with applicable child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, daily schedule, staff/child ratios and a summary of NC Child Care Law were all posted. A fire drill was documented as completed on 12-5-24 at 9:25 am with an evacuation time of thirty-four seconds. An outdoor inspection as documented as completed on 1-8-25. A shelter in place drill was documented as conducted on 11-18-24 at 9:25 am. An approved fire inspection was completed on 9-10-24. A sanitation inspection was completed on 10-4-24 with zero demerits and a superior classification. The children were observed during free play, washing their hands and eating lunch, which consisted of roasted potatoes, yogurt, blueberries and granola and milk. The incorporation status of Johnston Lee Harnett Community Action, Inc. was verified as current and active during the visit using the Secretary of State's website. A four star child care center license with seven points in education, two points in program standards and one quality point was issued on 11-18-16. Based on observations made and information provided during the visit, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was previously completed on 9-6-23, thus a new inspection was due on or before 9-6-24. A new inspection was completed on 9-10-24. 10A NCAC 09 .0304(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Fluoridated toothpaste was stored in an unlocked top cabinet. 15A NCAC 18A .2820(d) The violations documented were marked as corrected during the visit; therefore, a compliance letter was not required as a follow up to the visit. A sanitation inspection was completed on 10-5-23 with zero demerits and a superior classification. The children were observed playing outside, washing their hands and eating lunch, which consisted of beef tips with rice, broccoli, tangerine freeze and milk. The incorporation status of Johnston Lee Harnett Community Action, Inc. was verified during the visit using the Secretary of State's website. A four star child care center license with seven points in education, two points in program standards and one quality point was issued on 11-18-16. Based on observations made and information provided during the visit, you continue to meet the requirements previously met to obtain such a license. During the visit, we discussed your upcoming rated license year, which runs from 7-1-24 to 6-31-25. We discussed the importance of submitting education information promptly for all new staff and for existing staff who complete additional semester hours. We discussed the assistance offered through the Partnership for Children of Johnston County and the option to have the Environment Rating Scale (ERS) assessments completed at any time between now and your rated license assessment. If you do not score as well as you would like on the ERS assessment during the first assessments, you would be eligible for additional assessments during your rated license year. Please contact me if you have questions pertaining to the rated license assessment process.
